2 Step #1 If you haven t already done so, apply to the College of Business. Link: business/admission requirements/ Catalog year can be found on your degree audit (Page 1, near your name and major). GPA and credits earned information can also be found on your degree audit (Pages 1 & 2). If you re currently taking a class, do not enter a grade. On the right in this section, only enter the number of courses completed. In this example, courses completed are 3. Don t forget to click submit!

3 University of Wisconsin Eau Claire College of Business Health Care Administration Program Application for Admission and Cohort Assignment The UWEC Health Care Administration (HCAD) program offers a premiere education, combined with an advanced experiential practicum the final year. Applicants selected for participation in Step the program #2 are expected to demonstrate experience and a passion for wanting to make a difference in people s lives. If you have a head for business and a heart for caring, this program and future career may be a good fit for you. Selection decisions for the program include consideration of GPA, health care and leadership experience, drive/motivation/passion of the student, future career goals, and availability in the desired practicum cohort. Achievement of a grade of C or better is required in all HCAD major coursework, and a minimum GPA of 2.75 are required to apply. These are also graduation requirements. Applicants below these requirements may be considered for a provisional admittance on a case by case basis, if space allows. What do you bring or have to offer the HCAD program and your future classmates? Why do you want to receive a degree in Health Care Administration? What have you done to prepare yourself for a future in Health Care Administration (work or volunteer experiences, books or articles you ve read, activities you ve participated in, etc.)? What do you see yourself doing in five to ten years? We want to know how your participation in the program will add value to you, your classmates, the program, and the field.

They are the courses you ll take while on Practicum.

6 Step #5 Print Essay Reminder Your Essay: Your essay (and application) will be evaluated on how well thought out and written it is, as well as how you answer the following questions (be concise, the essay should not exceed two typed pages): What do you bring or have to offer the HCAD program and your future classmates? Why do you want to receive a degree in Health Care Administration? What have you done to prepare yourself for a future in Health Care Administration (work or volunteer experiences, books or articles you ve read, activities you ve participated in, etc.)? What do you see yourself doing in five to ten years? We want to know how your participation in the program will add value to you, your classmates, the program, and the field. Appointments for EACH TYPE available throughout the day: Business Presentations Brief (15 min) appointments: assistance with elevator speeches, PowerPoint slides, content organization, etc. Business Presentations Extended (25 min) appointments: assistance with interview practice and individual/group presentations. Business Writing (15 min) appointments: assistance with resumes, cover letters, s, CoB reports, memos, etc.

7 Step #6 Packet Deadline The spring application deadline for the 2016 practicum cohort is Friday, March 6, Applications are due by 4pm in Schneider 400C. The application packet should include: Health Care Administration Program Application form Camps Planner signed by advisor Degree Audit from Camps (PDF version) Essay Prospective or pre HCAD students seeking more information about the HCAD program should attend one of the following information sessions: Wednesday, March 13, 2015, 3 4pm, CENT 2920 Friday, April 3, 2015, 12 1pm, SSS 105 Prospective or pre HCAD students planning to apply on March 6, 2015, for a practicum cohort should attend one of the following application preparation sessions: Walk In times (SSS 204), Friday, February 20 th 1 4pm Walk In times (SSS 204), Tuesday, February 24 th 1 4pm Walk In times (SSS 204), Friday, February 27 th 1 4pm
