1 Idaho State University Accelerated Nursing Undergraduate Degree Program ADDENDUM I Prerequisite Course Requirements for Idaho State University Accelerated Nursing Undergraduate Degree Program: Set A Prerequisite Courses: A. SET A prerequisite courses must be successfully completed prior to the application deadline, 9/15/14. B. Successful course completion is with a grade of C or higher. C. A minimum combined GPA of 3.0 for all SET A prerequisite courses is required for application eligibility. D. Only SET A prerequisite courses are included in the GPA calculation for admission ranking. E. If courses are taken outside of Idaho State University; official transcripts verifying successful completion must be submitted to the ISU Registrar s Office as part of the ISU application process. F. Documentation of Set A course completion must also be submitted with the School of Nursing Application (can be an unofficial transcript). 3 Idaho State University Accelerated Nursing Undergraduate Degree Program ADDENDUM II Application Requirements for Idaho State University School of Nursing Accelerated Nursing Program: Completion of baccalaureate degree from a regionally accredited college or university Bachelor s degree must be conferred and on official transcript submitted to Idaho State University and unofficial transcript submitted to the School of Nursing by application deadline. Application/acceptance to Idaho State University - Application to Idaho State University is required prior to applying to the Accelerated Nursing Program. Upon acceptance to ISU, a student identification number is assigned that is required on the Nursing application. Official transcripts from all colleges attended must be submitted to ISU Office of Admissions as part of the application process. The ISU application fee is $ Applications are available on the ISU website at or through ISU, Office of Admissions, Box 8270, Pocatello, ID , phone Note: the Accelerated Nursing Program is an undergraduate program and applicants should apply as Transfer Undergrad/Nursing Pre-Program. Accelerated Nursing Program Application - Detailed application information is available on the application website at The online application for 2015 admission will be open in June, The application deadline is September 15, 2014, 5:00 pm. There is a $50 non- refundable School of Nursing application fee. School of Nursing Academic Advising Appointment - All applicants must meet with the School of Nursing academic advisor for review of transcripts and assessment of admission criteria, either by phone or in person, no later than Friday, 9/5/14, 5:00 pm. Documentation of the advising appointment must be submitted with the application by completing the Advising Certification Form. Transcripts - In addition to the official transcripts submitted to ISU Office of Admissions (above), unofficial transcripts verifying completion of baccalaureate degree, Set A prerequisite course completion, and current enrollment in Set B prerequisite courses must be submitted with the School of Nursing application. 4 Petitioning Transfer Credits Petitions may be required for ISU School of Nursing to consider transfer course(s). A title that is different than the ISU course title specified, outofstate credits, or a combination of two or more courses that meet a prerequisite course description could require completion of a petition. The process for petitioning a class must be completed, approved by the School of Nursing, and on file before the application is submitted. Consult the School of Nursing Academic Advisor, well in advance of applying to the program regarding transfer equivalency and the petition process. Satisfactory Completion of Prerequisite Courses See Addendum I for specific courses. Test of Essential Academic Skills (TEAS V) - The TEAS V exam must be completed and scores submitted by the September application deadline. ISU administers the TEAS at ISU- Meridian, 1311 E. Central Drive, between June and September, dates/times indicated below. Photo ID and a check or cash in the amount of $55.00 are required at the time the exam is taken. Please call the Accelerated Nursing Program at to schedule your exam, or if you have questions about the TEAS. The TEAS exam may be taken only once. ISU-Meridian 2014 TEAS Exam Dates: Wednesday, June 11, :00 a.m. Monday, July 14, :00 p.m. Friday, August 1, :00 a.m. Wednesday, August 13, :00 p.m. Friday, September 5, :00 a.m. Background Investigation - Applicants must have a background investigation performed between May 15 and September 15 of the application year. The ISU-approved vender for criminal background checks is CertifiedBackground. It is an online service. Go to and click on Students. Enter package codes: id13 and id13wvmc and follow website instructions for account creation. The cost is $ A copy of the order confirmation (one page) from CertifiedBackground must be submitted with the School of Nursing application. No other background investigation will be accepted. Please note: the background check is required annually in the Nursing Program. Physical Exam Applicants must submit a completed Health Evaluation Form verifying that the applicant possesses the physical, cognitive, and sensory capacities to meet the requirements of being an active and capable student in the nursing program. Applicant s healthcare provider must sign the Health Evaluation Form which is part of the Accelerated Nursing Program application. 5 Current Immunization Status - Acceptance to the School of Nursing is conditional based upon submission of all required immunization verifications. Acceptance letters will specify the deadline for submission of immunization verification. Failure to submit required immunization documentation by the specified deadline will result in retraction of acceptance to the School of Nursing. Tuberculosis screening (TB) Current test results. Tdap vaccination for adults MMR - two documented doses or positive titer Hepatitis B - 3 dose series and positive titer. This process takes approximately 8 months, so please prepare ahead of the application due date. Varicella - two documented doses or positive titer Annual influenza vaccination required if admitted to the nursing program CPR Certification Applicants are required to provide proof of current American Heart Association Basic Life Support (CPR/AED) for the Healthcare Provider CPR certification. The School of Nursing ONLY accepts American Heart Association Basic Life Support (CPR/AED) for the Healthcare Provider. Other CPR certifications are not accepted. Note that advertising for CPR courses may include statements such as follows AHA guidelines but the language does not mean it is formal AHA course. (AHA HealthSaver certification is not accepted.) 6 Frequently Asked Questions Does it matter what field my current baccalaureate degree is in? No, it does not matter what field the Baccalaureate degree is in. Any Bachelor's degree from a regionally accredited school is acceptable. What factors are considered for admission to the School of Nursing (SON) Accelerated Nursing Program at ISU? Applicant selection is based upon: 1. Prerequisite GPA - The GPA used to rank applicants is calculated from the Set A prerequisite course grades. 2. Test of Essential Academic Skills (TEAS V) The Percentile Rank-Program score is ranked. 3. Personal interview Selected applicants may be contacted for a personal interview during the admission process. Interviews will be conducted in the Accelerated Nursing Department, and will be a determining factor in the ranking process. Telephone or internet video conference interview may be arranged if you are located out of state. Interviews do not guarantee admission to the Nursing Program. The results of the interview may exclude an applicant from acceptance, despite GPA and/or TEAS score. Do the prerequisite courses need to be taken at ISU? No, courses can be completed at and credits transferred from other accredited colleges/universities. Students may be required to petition the ISU School of Nursing for consideration of some transfer credits (see pertinent paragraphs below). How do I determine if the classes I have already taken are equivalent to the prerequisite requirements of the Accelerated Program? Unofficial transcripts can be ed to the School of Nursing for evaluation by the School of Nursing Academic Advisor to determine what, if any, courses need to be taken meet prerequisite requirements. Petitions are often required for courses with a title that is different from the ISU course title specified in the prerequisite list, occasionally required for out-of-state credits, and necessary if you have a combination of two or more courses that you believe meets one of the prerequisite courses. Advising appointments can be completed in person or over the phone. 7 Contact the School of Nursing Accelerated Program Academic Advisor to schedule an appointment to discuss your individual petition needs at (208) A course description and, preferably, a syllabus from the course(s) referenced in a petition will be requested. Does it matter when I complete program prerequisites? All Set A prerequisites must be complete and on transcript prior to the application deadline, and a minimum GPA of 3.0 in Set A prerequisites is required. Introduction to Pathobiology (or Pathophysiology) must be completed prior to applying but not more than 7 years prior to application. Pharmacotherapy for Nursing must be completed prior to beginning nursing courses but not more than 3 years prior to application. What do I need to know about the Test of Essential Academic Skills (TEAS)? The computerized TEAS V exam includes four parts: Reading, Mathematics, Science, and English Language Usage. A study guide for TEAS V is available if desired at The exam takes approximately 4 hours and your score is available upon completion. You will submit a copy of your score with your Accelerated Nursing Program application. The TEAS V exam may be taken only once. Applicants outside the Boise-Meridian area may contact ATI Testing at for testing sites. The TEAS V does not have to be taken at ISU-Meridian. However, applicants must select BSN as the Program Type when logging in to begin the TEAS V exam. Refer to Application Requirements for TEAS testing dates and additional details. How competitive is the admission process? The admission process is highly competitive. The program accepts 30 students each year. Where and when can I get an application? The application opens June 2014: Is an appointment with an advisor required? Yes. All applicants must meet with the SON academic advisor for review of transcripts and assessment of admission criteria, either by phone or in person, no later than Friday, 9/5/14, 5pm.
