2 Instructions For Completing Your Application For Admission To DMACC Thank you for applying to DMACC. Below are instructions to help you complete the application. Once your application is received, it will be processed as soon as possible. You should receive communication from us within 5 working days, informing you of your admission status. Current and Returning Students: If you have submitted an admission application in the past 3 years, you do not need to reapply for admission. Contact the DMACC Admissions office about your status. Admission procedures and entry requirements vary by program. Check our website for specific program details. ( Please note: Some programs accept a limited number of students. If you have met all of the program requirements and the program is full, you will be placed on a waiting list until a seat becomes available. Section 1...Personal Information (Questions 1 11) Question 3... If your high school or college records appear under a different name, please indicate that name under All Former Legal Last Name(s). Question Enter a valid address. DMACC will use this to communicate important information. Question 5... Enter your current living address. Question 6... Enter all numbers where you can be reached. By providing your cell phone number, you have provided DMACC with consent to send you text messages in conjunction with college business. Questions 9 & This information is required for federal and/or state reporting purposes. Single parent is a person who (1) is either pregnant or has custody or joint custody of one or more child/children; (2) is also unmarried or legally separated from a spouse. Displaced Homemaker is a person who has been a homemaker exclusively but now because of divorce, death or disability of a spouse, must prepare for paid employment. Students who indicate their first language is not English may be required to meet ESL admission requirements. Section 2...Planned Academic Program/Transfer Major (Questions 12 13) Question Program of Interest is sometimes referred to as a major course of study. If you are not sure about your program, visit with a counselor or advisor before applying. There is a list of programs and groups on page 6 of this application packet. Graduates are awarded the Associate in Arts or Associate in Science degree with a major in Liberal Arts. Students who have determined which profession they plan to enter should become familiar with the specific course requirements of the four-year institution to which they plan to transfer. Then with the help of an academic advisor or counselor, students can select the curriculum best suited to satisfy their particular transfer objectives. Section 3...Admission Information (Questions 14 20) Question Make sure the Program of Interest you select offers classes during the semester you plan to enter DMACC. Question Make sure the DMACC location you select offers your Program of Interest. (Refer to page 6 of this application packet.) Question Refer to the Admission Steps Section for Guest Student definition. Question Applicants seeking need-based financial aid are required to submit official high school or GED transcripts to DMACC before financial aid will be processed. Question Students who do not plan to receive a degree and check No will be categorized as a non-degree-seeking student and will not be eligible for financial aid. Question If you answered Yes to Question 18, see the definitions below for specific awards. The Program of Interest selected in Question 12 must agree with the award offered in the program. (Refer to page 6 of this application packet.) (AA) Associate in Arts Degree provides the courses of study equivalent to those offered to freshman- and sophomore-level students in most four-year colleges/universities. (AS) Associate in Science Degree provides a program of college-level courses designed to prepare students for transfer to a four-year college/university or for skills preparation for entry-level employment in a specific occupation where a bachelor s degree is usually needed. (AAS) Associate in Applied Science Degree is a two-year degree that may include fall and spring semesters plus summer semesters and is designed to develop the student s competence for employment in a specific occupational field. Page 2 (AGS) Associate in General Studies Degree provides students the opportunity to select their coursework to meet specific educational goals and interests. It is generally not designed to meet college transfer requirements.

Diploma provides students the opportunity to satisfactorily complete a career or technical program that is typically one year long (that may include three semesters) and includes at least one general education course in addition to courses designed to develop a person s technical skills and knowledge. Certificate of Specialization provides students the opportunity to satisfactorily complete a shorter course of study in a vocational or technical area, usually no longer than two semesters and usually designed to develop specific skills. Some certificate programs are too short to qualify for federal financial aid. Question If you plan to transfer to another college or university, please indicate, using the school s official name. Section 4...Iowa Residency Information (Question 22) The Iowa Department of Education determines the requirements for all Iowa community colleges, which may differ from those at state universities. In most instances, students will be considered residents of Iowa, for tuition and fee purposes, if they are permanently domiciled in Iowa and have resided in the state for a period of not less than 90 days prior to the start of the semester in which they plan to attend. A Request for Determination of Residency Status Form must be submitted before the first day of the semester along with documentation proving residency. Complete information may be found at Applicants who are not residents of Iowa are required to submit official transcripts showing high school or GED completion prior to acceptance at DMACC. If a degree was not obtained, students must pass the COMPASS Ability to Benefit exam. Official college transcripts may be used to meet this admission requirement as well. If you are not a resident of Iowa, provide proof of secondary education completion by submitting official high school or GED transcripts. If you have attended another college or university and have credit to be counted toward program requirements, submit official transcripts from those institutions. Section 5...Secondary Education Information (High School or GED) (Questions 23 26) Question List current or last high school attended. (If you have received your high school diploma from DMACC or another community college, enter the name of the college as high school last attended.) Section 6...College Education Information (Question 26) Question List all colleges (including DMACC) previously attended, beginning with the most recent, in order of attendance. TO ASSURE PROPER COMPLIANCE WITH STATE AND FEDERAL FINANCIAL AID REGULATIONS, STUDENTS WANTING TO RECEIVE FINANCIAL AID WHILE ATTENDING DMACC WHO HAVE ATTENDED OTHER INSTITUTIONS, WILL NOT HAVE THEIR AID PAPERS PROCESSED UNTIL ALL OF THEIR TRANSCRIPTS HAVE BEEN RECEIVED AND REVIEWED. AN OFFICIAL TRANSCRIPT FROM EACH INSTITUTION ATTENDED MUST BE SUBMITTED DIRECTLY TO THE DMACC ANKENY ADMISSIONS OFFICE. ALL DOCUMENTS SUBMITTED BECOME THE PROPERTY OF DMACC AND CANNOT BE RETURNED TO THE STUDENT OR SENT TO OTHER INSTITUTIONS. Admission Steps Assessment scores used for admission purposes must be (5) five years old or less from the test date. NEW STUDENTS: 1. Submit a completed DMACC application for admission. 2. Submit ACT or COMPASS exam results to DMACC (Full-Time or ESL only). 3. If you are not a resident of Iowa, provide proof of secondary education completion by submitting official high school or GED transcripts. 4. If you have attended another college or university and have credit to be counted toward program requirements, submit official transcripts from those institutions. 5. Complete any program prerequisites for your specific program. GUEST STUDENTS: STUDENTS WHOSE PRIMARY ENROLLMENT IS AT ANOTHER COLLEGE OR UNIVERSITY AND DO NOT PLAN TO ENROLL IN A PROGRAM OF STUDY AT DMACC. 1. Submit a completed DMACC Application for Admission. Always apply as Liberal Arts, no matter what your major. 2. Provide proof of enrollment such as an acceptance letter or valid student ID from primary school of attendance. (Note: Guest students are not eligible for financial aid and must remain part-time during fall and spring semesters.) HIGH SCHOOL STUDENTS: STUDENTS CURRENTLY ENROLLED IN HIGH SCHOOL WHO PLAN TO TAKE COURSES AT DMACC WHILE CURRENTLY ENROLLED IN HIGH SCHOOL. Juniors and Seniors Complete steps 1 and 2 below if enrolling as a parttime student. Complete steps 1, 2 and 3 if enrolling as a full-time student. Freshmen, Sophomores and Home-Schooled Students Complete steps 1, 2, 3 and 4.
