Transportation Reimbursement Form Pg 8. Evaluation Form Teachers and students should complete an evaluation form following the Utah Career Days event. 2 EVENT OVERVIEW EVENT PURPOSE The objective of Utah Career Days is to provide students (Jr. High/Middle School and High School) throughout the state with hands-on exposure to the diverse and exciting careers in the energy, manufacturing, construction, automotive and engineering industries. The Utah Career Days program includes interactive exhibits, industry displays and demonstration projects all inviting student involvement. It is hoped that increased awareness will help students, parents, teachers, counselors and the community at large become more aware of the opportunities available in these industries. EVENTS & ACTIVITIES There will be 3 area locations for rotation between. Be sure to move quickly when you are directed to move to the next location so that students will be able to have as much opportunity to see all the displays and activities as possible. All participants, including teachers and chaperones MUST adhere to the dress standards. CHAPERONES MUST STAY WITH STUDENT GROUPS AT ALL TIMES STUDENT RESPONSIBILITIES Follow behavior protocol Follow dress standards Follow safety standards DISTRICT/SCHOOL RESPONSIBILITIES Select students to attend Obtain district activity and transportation release forms Provide appropriate chaperones, 1:15 adult to student ratio minimum Provide transportation to the Utah Career Days event site, Davis County Legacy Events Center (map attached) Make arrangements for student lunches if needed (LUNCH MUST HAPPEN AWAY FROM THE UTAH CAREER DAYS SITE, EITHER ON THE BUS RIDE OR AT THE SCHOOL.) Collect teacher and student evaluations and return to Sherry Marchant, USOE, P.O. Box , Salt Lake City, Utah Provide Transportation Reimbursement forms with district bus invoices to: Pam Pyle, Utah Career Days, Utah Local Technical Program, Utah State University, 4111 Old Main Hill, Logan, Utah pamela.pyle@usu.edu Office: TEACHER RESPONSIBILITIES Prepare students for event Give student related assignments Provide time for wrap-up, reflection and evaluation Require the assignment from this packet or an assignment of your own design to help students stay on task. Review student responsibilities Behavioral requirements Dress requirements General requirements Collect student activity and transportation participation forms Stay with student group and ensure chaperones stay with student groups Maintain adult to student ratio, 1:15 minimum Provide general supervision for students and make sure students follow behavioral guidelines Make sure students wear appropriate safety equipment and clothing Make sure students rotate promptly when directed.

Buses will drop-off and pick-up at the south end of the parking lot.
5 ALL ATTENDEE REQUIREMENTS STUDENTS, TEACHERS, CHAPERONES, and VOLUNTEERS STUDENTS MAY NOT DRIVE THEMSELVES TO THE UTAH CAREER DAYS EVENT. NO EXCEPTIONS. STUDENTS MUST ARRIVE ON THE BUS WITH THEIR SCHOOL/CLASS. DRESS REQUIREMENTS Dress for the weather. Many of the Utah Career Days activities will be held outside. Wear closed toed shoes for safety. Wear long pants or Capri pants - no shorts, skirts or dresses. NO SAGGING OR BAGGING. Wear required safety equipment. Each student will be provided with gloves and safety glasses, when participating in an activity during the event. BEHAVIOR REQUIREMENTS Stay with your assigned group throughout the entire event. This includes both students and teachers. No rowdiness or goofing off. Unload and load bus in orderly and timely manner. Follow instruction given by the bus coordinator. Stay on marked paths and only in the areas designated for the Utah Career Days event. Be courteous to presenters. They are all volunteering their time and equipment to make this event happen. Follow rules for each designated area. POLICE/SAFTEY OFFICERS WILL BE ON PATROL IN EACH OF THE THREE AREAS. Throw trash away in designated areas. Don t leave it for someone else! GENERAL REQUIREMENTS AND INFORMATION Leave backpacks at schools. All students will receive a bag in which to collect information from vendors. Leave cell phones at home or school. Taking phone calls at the Utah Career Days event is not permitted. Bring a bottle of water and sunscreen with you to the Utah Career Days event. Have fun and learn something new!
