2 Introduction The St. Luke s University Health Network s Warren Campus has conducted a community health needs assessment (CHNA) pursuant to the requirement of Section 501 (r) of the Internal Revenue Code. The study was completed in 2012 and the project was completed with the assistance of independent contractors who are recognized experts in the fields of community health assessment and data mining. St. Luke s Warren Campus commissioned AMK Healthcare Consulting, LLC (AMK), in association with New Solutions, Inc. AMK was established in 2010 by Valerie Sellers, president and CEO. Ms. Sellers holds a Bachelor s degree in Industrial and Labor Relations and a Master s degree in Health Administration both from Cornell University. Before founding AMK, Ms. Sellers served as Senior Vice President for Health Planning and Research at the New Jersey Hospital Association for more than 17 years. New Solutions New Solutions, Inc. (NSI) is a management consulting company formed in The collective expertise of the NSI team includes health administrators, market researchers, business and financial analysts, strategic planners and marketing professionals, epidemiologists, database experts, and decision support technologists. Lead consultants have an average of 20+ years of health care experience. On the St. Luke s Warren Campus CHNA project, the lead participant from NSI was principal, Nancy Erickson, a key management advisor to many New Jersey hospitals senior administrators for more than 20 years. Ms. Erickson holds a Bachelor s degree in Sociology from Drew University and a Master s degree from Rutgers University, where she specialized in health planning, policy and management. The results of the survey are based on four separate components: 1. A random telephone survey of 719 households in the hospital s primary service area, which includes Warren County and its neighboring community of Easton, Pennsylvania. 2. An analysis of existing community health measures and secondary data. 3. Three focus groups conducted by AMK with key patient populations in the Warren Campus service area. 4. Structured interviews with identified key informants in the community
3 St. Luke s University Health Network Mission Statement The mission of St. Luke's University Health Network is an unwavering commitment to excellence as we care for the sick and injured; educate physicians, nurses and other health care providers; and improve access to care in the communities we serve, regardless of a patient's ability to pay for health care
4 Service Area The community served by the Hospital is defined based on the geographic origins of the Hospital s inpatient and observations. The Primary Service Area (PSA) is the geographic area from which the largest portion of the Hospital s inpatients and observations originate. The Secondary Service Area (SSA) is where an additional population of the Hospital s inpatients and observations reside. The PSA is comprised of seven zip codes in Warren County. The SSA is comprised of one zip code in nearby Pennsylvania. In 2011, when the Community Health Needs Assessment data was collected, the Hospital s PSA/SSA combined included approximately 115,000 people and is projected to increase 2.1% by Identification of Community Health Needs The CHNA data were reviewed by AMK Consulting and New Solutions along with a Strategic Planning committee consisting of key community leaders and hospital personnel, and the following needs were identified: - 4 -
5 Aging population with changing health care needs o Example: almost 30% of population over age 65 has not had pneumonia vaccine o Transportation to and from appointments is challenging Elevated high school dropout rates and poverty rates High rates of chronic disease, especially o Heart Disease o Cancer o Diabetes o COPD o High cholesterol o Unintentional Injury Lack of health insurance coverage for many o Even those with insurance feel specialist care is unavailable Inappropriate ER use o Cellulitis o ENT conditions sore throats and colds o Dental problems o Asthma Access to Care for Primary Care, Specialist care and Dental services o Primary Care Physicians below state average o Lack of pre-natal care, obstetrics and gynecology o Orthopaedic services Communication Strategies for community to be aware of available services Lifestyle behaviors o Tobacco use o Obesity, Nutrition and Physical Activity o Drug and Alcohol Use and Abuse Needs being directly addressed: The priority community health needs to be addressed through the Warren Community Health Implementation Plan include: - 5 -
6 Increased access to primary and specialty care, including orthopaedics, neurology, women s health and cardiology Obesity, Nutrition and Physical Activity Wellness and Chronic Disease Prevention, including diabetes Mental Illness and Substance Abuse (Behavioral health) Smoking Cessation Elder Health Communication Strategies The implementation strategies for each of these priorities will include steps to improve and coordinate communication to assure seamless continuum of services. Needs that will not be directly addressed at this time include: 1. Elevated high school drop-out rates and poverty rates We are firmly committed to the advancement of Phillipsburg, New Jersey, and we demonstrate that commitment continuously by supporting civic initiatives. In 2012 and 2013, we provided $10,000 to keep the town s public swimming pool open and available for local youths. In addition, we are planning to place AED (defibrillators) in every entity within the Phillipsburg Area School District. By providing lifesaving equipment and enhancing quality of life for children, we are hoping to positively encourage positive behaviors. Additionally, St. Luke s Warren provides millions of dollars in charity care each year for those who unable to pay for health care services. In 2012, that number was. 2. Lack of health insurance for many St. Luke s Warren Campus is actively working with health care insurance providers to develop cooperative relationships that will make insurance coverage financially feasible for more people. Part of this effort is in response to the Health Insurance Exchange initiative that is a vital component of the Affordable Care Act. Our hospital feels it can be most useful by remaining involved in the transition process as the ACA unfolds. 3. Communications strategies - 6 -
7 St. Luke s Warren is an active participant in the Warren County Community Health Improvement Coalition along with representatives from the Warren County Department of Public Health and other health and welfare-related agencies throughout the county. Together, we continue to work toward improving the dissemination of information. Our goal is to 1) educate the community about the types of programs and services available; and 2) to help community members access those services. Community Resources SLUHN Warren Campus has identified the following agencies and programs as potential collaborators for the Implementation Plan: For all priority areas in the Community Health Needs Assessment, the following agencies will be key partners for the implementation plan: All priority health needs Warren County Health Department Warren County Department of Education Phillipsburg Area School District Phillipsburg School-Based Youth Services Phillipsburg Education Foundation Warren Hills Regional School District United Way of Northern New Jersey Additional partners for individual priority health needs include: Obesity, Nutrition and Physical Activity; Wellness and Chronic Disease Prevention, including Diabetes American Cancer Society American Diabetes Association American Heart Association American Red Cross, North Jersey Region Local grocery stores Local Restaurants participating in St. Luke s Heart Smart program Million Hearts (CMS Campaign for heart attack and stroke) NORWESCAP (Northwest New Jersey Community Action Partnership) Public Health Nursing Agency Tobacco-Free for a Healthy New Jersey - 7 -
