2 Dear Applicant: Thank you for your interest in State Fair Community College s Certified Medication Technician (CMT) Course. Students who choose to enroll in this course are admitted to the college on the same basis as other students, but admission to the college does not ensure admission into the CMT course. This course is a part of the MoHealthWINs grant, which means eligibility requirements must be met for a student to enroll in any course. To apply for the CMT course, each applicant must complete the following items. Checklist of Required Materials begins here: Please check off as you complete each task. For your application to be considered, each bulleted item must be completed accurately. This checklist is provided for your benefit and should not be submitted with your application packet. Step One: Meet State Fair Community College admission procedures: (If you are currently attending SFCC you should have already completed the tasks listed in this section.) Go to SFCC s website at and click How to Apply. o New students click New Student link, complete the online admissions form and request appropriate transcripts. o Follow remaining steps, submit placement scores and complete the Free Application for Federal Student Aid (FAFSA). SFCC s school code for the FAFSA application is This step is strongly recommended. Step Two: Complete MoHealthWINs eligibility application: Read through this entire application packet. o Complete eligibility form located at the end of this packet and submit completed form to Kara Wheeler. Application materials received after cut off dates will be considered for the next available class offered. After each cutoff date, all complete applications will be evaluated. All incomplete packets will be considered ineligible. The first 15 students with complete applications, who meet the MoHealthWINs eligibility requirements, will be invited to join the upcoming course. If student accepts, (s)he will be enrolled in the class by the MoHealthWINs advisor. Didactic classes are limited to 15 students. If a class if full and more qualified individuals have applied, they will be placed on a waiting list and given first priority to the next course offered. All students who enroll in the CMT course will be required to enroll in Toolbox 2.0 and to take the WorkKeys/NCRC, as part of the MoHealthWINs grant. They will be required to come to campus for an orientation day to complete this process and turn in necessary paperwork. Be sure to bring a photo ID to orientation. Participant Guidance: MoHealthWins is required to serve a target population which includes Trade Adjustment Assistance eligible participants, unemployed participants, underemployed participants and low skilled participants. There is flexibility to include a broad spectrum of individuals (as participants) from varied socioeconomic backgrounds for training purposes.
3 Trade Adjustment Assistance (TAA) An individual who has lost their job, from Trade certified employer, through no fault of their own (due to lack of work), as a result of foreign trade. USDOL is responsible for certifying qualifying employers as Trade Act eligible. The Trade Act program consists of Trade Adjustment Assistance (TAA) and Trade Readjustment Allowance (TRA) and is only administered by State merit staff. In Missouri, DOLIR/DES administers the TRA portion of the program and makes eligibility determinations (TRA Claims) along with TRA weekly benefits and other TAA associated payments. DED/DWD administers the TAA portion of the program that includes benefits and services such as training eligibility and approval. Unemployed An individual who has been terminated or laid off, or has received a notice of termination or layoff; employed at a facility at which the employer has made a general announcement the facility will close within 180 days; self-employed (including employment as a farmer, a rancher, or a fisherman) but unemployed as a result of general economic conditions or natural disasters; and a displaced homemaker. Underemployed An individual who is working part-time but desires full time employment, or is working in employment not commensurate with the individual s demonstrated level of educational and/or skill achievement. The local Workforce Investment Board negotiated average earnings target wage should be referenced to aid in eligibility determination. Low-Skilled An individual will be required to demonstrate college readiness for placement into college level coursework, (typically coursework numbered 100 level or above) as defined in the chart below of this guidance. Individuals scoring below the designated cut-off score for any area will be defined as Low- Skilled for the purposes of MoHealthWINs eligibility. All MoHealthWINs participants will be assessed through the use of WorkKeys/NCRC assessment in each of the following areas: Applied Mathematics, Locating Information, and Reading for Information. Individuals scoring below a level 5 for any NCRC/WorkKeys area will be defined as Low-Skilled for the purposes of MoHealthWINs eligibility. Since MoHealthWINs also serves GED seeking participants, low skilled adults as defined by the US Dept. of Education- students who, for whatever reason, are not enrolled in secondary school and lack the sufficient mastery of basic educational skills and do not possess a secondary school diploma or its recognized equivalent. Not having a high school diploma or its equivalent is by definition, low skilled. These participants will be assessed with a tool that will determine exactly what general, basic skills need remediation. Tests of Adult Basic Education (TABE) are currently used by high schools, MO Career Centers and Adult Education programs. This assessment test is aligned with the National Reporting Standards for education. Other Grant Participant Specifications: Veterans Priority The Jobs for Veterans Act (Pub. L ) requires priority of service to veterans and spouses of certain veterans for the receipt of employment, training, and placement services in any job training program directly funded, in whole or in part, by DOL. The regulations implementing this priority of service can be found at 20 CFR part1010. In circumstances where a grant recipient must choose between two qualified candidates for training or a service, one of whom is a veteran or eligible spouse, the Veterans Priority of Service provisions require that the grant recipient give the veteran or eligible spouse priority of service by
4 admitting him or her into the training program or providing service. To obtain priority of service a veteran or spouse must meet the program s eligibility requirements. Grantees must comply with DOL guidance on veterans priority. All community colleges and technical colleges are required to collect data for all MoHealthWINs participants including continuing education (noncredit) grant funded programs. The grant requires data collection on each participant for each area (Math, Reading and English) that indicates a participant s skill level (college-ready, one-level below college-level, two-levels below college-level, or three or more levels below college-level). These data elements will be important as we examine whether or not a student has made gains in skill level and will also help to explain the importance of entering skill level on degree and credential completion. The community colleges have all agreed to a set of common cut-off scores. Such cut-off scores should serve as the starting point for the definition of "Low Skilled" for credit seeking participants. An entering student who is deficient in one or more areas will be defined as "Low Skilled". Missouri Community College Readiness Standards Missouri community colleges have developed a common standard for college readiness. All Missouri community college students will be required to demonstrate college readiness for placement into college level coursework (typically coursework numbered 100 level or above) as defined in the chart below using ACT scores as noted. Concordant scores in other placement tests, as indicated below, may also be used for students who do not present an ACT score. Point considered when determining common placement scores included: The adopted college readiness standard must be based on research and best practice. The standard must align with other placement testing tools. Consideration should be given to the approach other states took in adopting a statewide standard. In its review of ACT based college readiness materials and publications, and in consultation with ACT staff and researchers, community colleges believe these common core standards will promote student success, provide access to college and support student learning. These changes began in the fall of The new testing scores are as follows: ACT COMPASS ASSET SAT Writing Critical Reading 450 Reading Critical Reading 460 Mathematics 23 Interm Test Math 460 Scores below these standards will result in development coursework. There is only one required course to complete the CNA program at SFCC. All students enrolled in this program are expected to complete the required course in 8 or 16 weeks. This class is required class for all students interested in furthering their education to receive a Nurse Aid skills certificate, professional certificate, or an AAS Health Care Specialist degree with an emphasis in Nurse Aide.
5 For program acceptance students will be required to complete: 1. MoHealthWINs eligibility form (last two pages of this application packet) Once accepted to the CMT program, students will be required to complete the following: 1. Since SFCC communicates with students through digital means and CMT courses are offered online as well as on ground, students are required to take a digital literacy assessment to test their current working knowledge of computers. The test consists of five modules, which are as follows: a. Basic Computer Use b. World Wide Web c. Windows 7 d. Using e. Microsoft Word All five sections of this test must be passed in order for the student to be enrolled in the CMT course. Students will be given two attempts at each section in order to pass. If one or more sections are not passed, the student will be required to take SS114 Computer Skills for College, a two-credit hour course offered at SFCC that is designed to build a foundation of basic computer skills necessary to be successful within an educational setting. Topics include basic computer functions, and functional navigation and practical application of Microsoft Word, Excel, PowerPoint, Internet, , mystar, SFCC Online and Elluminate. The cost of this class is not covered by the MoHealthWINs grant. It will be the responsibility of the student to pay for the course or use other financial aid received. The digital literacy assessment is currently offered at SFCC s Sedalia, Clinton and Boonville locations. To schedule an appointment to take the assessment, please use the following numbers: Sedalia: (660) Clinton: (660) Boonville: (660) The deadline for the spring and summer semesters to have this test taken are as follows: Spring first 8 weeks enrollment: Friday, Dec. 13 Spring second 8 weeks enrollment: Friday, March 7 Summer enrollment: May Students will be required to attend a full day of orientation held at the Sedalia campus before class begins. At orientation, you will be given guidance to your class, as well as completing required testing for the MoHealthWINs grant. This includes taking the WorkKeys/NCRC tests and registering at You will also turn in the following documentation that is required to be licensed as a CMT in the state of Missouri: a. A copy of a current TB skin test (It must be within the current year.) b. A signed permission slip to run a background check c. A copy of student s driver s license, Social Security card, and CNA certification. d. A letter of recommendation from a healthcare supervisor

TABE test completion with the following scores: Vocabulary Comprehension Combined Math At orientation, if students have turned in and completed all of the above, they will be enrolled in the CMT course at orientation and given a copy of their schedule so that they may receive an SFCC BLUECARD and purchase their textbooks. 7 Certified Medication Technician (CMT) Fee Schedule Because the CMT course is offered through MoHealthWINs*, tuition is free for all eligible participants. However, this does not include fees or additional expenses needed to complete the class. There will be expenses incurred in addition to class tuition and fees.
