1 Transitional Housing Listing The Elizabeth Stone House P O Box 164 Jamaica Plain, MA The Elizabeth Stone House Transitional Housing Program provides housing for women and their children in need of temporary housing while they search for a permanent home. The Stone House serves women and families from the Roxbury, Dorchester, and Jamaica Plain area that are victims of abuse and or violence. Please contact them directly for more detailed program information. Abbey s House 52 High St Worcester, MA (505) Abby's House offers service enriched housing to women and children living in poverty. They have 78 units in total available at 3 separate locations. Most units are one bedroom apartments and there are a few 2 bedroom apartments available for women with children. Their application is available online on their website with links to applications and forms needed. You can print them out and bring them to Abby s house to begin the application process. Abby's House is located on High St just a few blocks south west of City Hall. Please contact them directly for more detailed information. Housing Families Inc. 350 Cross Street Malden, MA (781) Housing Families Inc. helps 400 homeless and extremely low-income people each day of these are homeless and formerly homeless children. Housing Families Inc. helps these children and their parents to grow and heal from their past traumas, including losing their home. Housing Families Inc. offers a range of academic and emotional supports (tutoring and counseling) which help children improve their grades and their self esteem. In addition to supportive services, they operate nearly 100 units of affordable housing and transitional shelter. Please contact them directly for more information about access to their services.
2 Lowell Transitional Living Center Middlesex Street Lowell, MA 018 (978) The Lowell Transitional Living Center provides: Emergency and Transitional shelter with a facility that houses 90 single adults in a supportive environment. Each person who stays at the LTLC begins a process that is geared toward ending their homelessness. Expanded Winter Beds in winter they expand their capacity by 45 beds so everyone in the Greater Lowell area has an opportunity to sleep indoors during the harsh weather months. The Bad Weather Bed program opens the shelter to those who may not normally come inside and offers the opportunity for our case managers to reach some of the most chronically homeless individuals. Supportive Housing the 12 single room apartments owned by the LTLC provides housing for individuals in the Supportive Housing Program. Renewal House Rev. Susan Chorley 10 Putnam St. Roxbury, MA (617) The Renewal House is a domestic violence shelter for individuals in crisis and their children. Since 1980, Renewal House has provided temporary emergency shelter and advocacy services to more than 1,000 individuals and families escaping domestic violence. The Renewal House offers emergency shelter for up to three months for qualified families. Information about renewal House can be accessed through the 24-hour confidential hotline: Some of the services offered at Renewal House are : Safety planning, In-house support groups (Self-Esteem and Wellness, Domestic Violence Support Group and Parenting Group),bilingual Spanish/English advocacy, English as a Learning Language (ELL) classes, health care advocacy,pastoral counseling, children's programs, counseling, referrals for job training, legal assistance and educational programs and community education.
3 Sojourner House 85 Rockland Street Roxbury, MA The Sojourner House has one community and has a total capacity of 24 people. Referrals are maintained according to a waitlist. Some of the services included are advocacy, food pantry, family life case management, transition to work and housing program. The following curfew restrictions are strictly enforced: Sunday-Thursday 9pm and Friday-Saturday 12am. Referrals from DTA are preferred but families can call Sojourner directly if rejected by DTA. Temporary Home for Women and Children 41 New Chardon Street Boston, MA Total Capacity: 15 families. Services: Advocacy, counseling; 3meals/day; limited Spanish; handicap accessible; medical care day program; transportation; clothing if available. Restrictions: Non-EA eligible; women & children only; no drugs or alcohol; income restriction. Admissions Process: Referral needed from a social service agency. Sheila Daniels/Edgewood Case Management in Transitional Housing for Individuals Women Only 55 Dimock St, Roxbury, MA ext Languages:English The Housing programs provides permanent housing residency for women and men in recovery who are living with multiple health issues and who are head of household with children. The Housing programs are located within a two mile radius of The Dimock Center in Roxbury. A Case Manager lives on-site and is available twenty-four hours a day, seven days a week. Additional Case Management services are available during the evening hours. The programs are supported by the Department of Public Health, Bureau of Substance Abuse Services.
4 Bridge Eliot Transitional Housing with Case Management for Individuals 47 West Street, Boston, MA Transitional Living Program Bridge s transitional living home provides housing for year olds. Residents can learn life skills, get help with finding permanent housing, getting a job and/or filling out school applications. Single Parent Home Bridge Eliot provides young pregnant women and single mothers with a safe place to call home. Moms can learn parenting and budgeting skills and get help finding permanent housing. Bridge staff assists with finding employment, daycare and/or attending school. Co-Op Apartments Our Co-op Apartments offer our young people increased freedom along with the support and caring of Bridge staff. Co-op apartments can be an important step on the road to independence. Project S.OA.R. (Stability, Opportunity, Achievement and Recovery) P.O. Box Boston, MA (617) ext Project S.O.A.R. provides transitional supportive housing for 70 homeless men and women (55 beds for men, 25 beds for women). Located on Long Island, this transitional housing program primarily serves guests who have jobs in the community or are enrolled in one of the shelter's job training or educational programs. The goal of Project S.O.A.R. is to facilitate the program participant's transition out of the shelter and into permanent housing. Eligibility: Homeless 18 years or older Employed, collecting benefits or enrolled in or awaiting enrollment in an educational or job training program. Posses a desire to transition into permanent housing Be able to participate in and contribute to communal living Demonstrate commitment to ongoing personal recovery A minimum of 30 days of sobriety
5 Safe Harbor Homeless Services Transitional Housing P.O. Box Boston, MA (617) ext The Safe Harbor Program serves 20 homeless men and women who are HIV positive and recovering from substance abuse. Safe Harbor offers a variety of services designed to assist the client in their journey towards independent lives in the community. Client services include: Individual counseling and education for HIV and substance abuse, on-site medical and mental health care, medication management support, case management and client advocacy, aftercare support and planning. Safe Haven Edith Nourse Rogers Memorial Hospital 200 Springs Road Bedford, MA (781) Jim Chaplin Safe Haven is a 20 bed transitional supported housing program for VA eligible Veterans who are chronically homeless and actively struggling with a substance abuse and/or a MH issue.. The program does not charge rent and allows a Veteran to stay there for up to 6 months. The Safe Haven Program has houses located in Boston and Framingham. Transportation will be provided for veteran s clinical needs. Ummi's Transitional Housing with Case Management for Individuals The Dimock Center 55 Dimock St, Roxbury, MA Ummi s is a Transitional Housing Program for 13 women (and their children up to 14 years of age) who are living with multiple health, mental health and addictions issues. The program model provides Intensive Case Management services to women and their families who meet the admission criteria. The program accepts pregnant women and women who are on Methadone maintenance. Women and their children may live at the program from 6 to 24 months. Aftercare services are available for one year following discharge.
6 Victory Programs Women s Hope Transitional House 12 Chamblet St. Dorchester Ma (617) Provides transitional housing for women who are living with HIV/AIDS and are in recovery from substance use disorders. Located in Dorchester the program can accommodate up to 7 women and residents can remain in the program for up to two years. Residents receive case management and recovery support services and assistance with locating permanent housing. The program goals are to provide a system of support while the residents move toward increased confidence, self sufficiency and independence. Victory Transitional house 3 Virginia Street Dorchester, MA (617) A supportive residential program designed to accommodate 6 single men living with HIV/AIDS and in recovery from substance use disorders. As with our Women's Hope Transitional Home, residents can remain in the program for up to two years. Through case management and addiction recovery support, the program seeks to empower each resident to maintain systems of support while preparing for independent living. A main goal for the residents of Victory Transitional House is to be able to move on to permanent housing settings. LARC (The Living and Recovery Community) Program Lemuel Shattuck Hospital 170 Morton St. 11 North Jamaica Plain MA (617) LARC is a structured 15 bed residential stabilization program for men and women with HIV/AIDS and histories of chronic addiction relapse. A LARC resident is typically homeless or at risk of becoming homeless due to alcoholism and/or drug dependency. All LARC residents are fully integrated into a program of recovery. LARC fully integrates persons involved in methadone treatment into the stabilization program. This program provides up to 90 days of specialized substance use disorder treatment and stabilization services. The Portis Family Home 226 South Huntington Avenue Jamaica Plain, MA (617) The Portis Family Home is residential shelter for homeless and very low-income women and their children. The program can accommodate up to seven families who are addressing substance use disorders. Many residents of The Portis Family Home have experienced domestic violence and are living with other chronic health conditions such as HIV/AIDS. The program provides intensive housing and vocational case management and recovery support services as the resident families transition to more independent and permanent housing settings.
7 (Victory Programs Continued) Yetman House Transitional Housing with Case Management for Individuals 26 Windmere Road, Dorchester, MA The Yetman House offers housing to women who have successfully completed an alcohol and/or drug treatment program. Through case management and addiction recovery support from peers, the program seeks to empower each resident to maintain systems of support while preparing for independent living. Joelyn s Family Home 1 Long Island Rd. Boston, Ma (617) Joelyn's Family Home - Located on Long Island in Boston Harbor Joelyn s Family Home serves homeless women who are addressing substance use disorders. This program provides 4 to 6 months of residential treatment services and can serve up to 42 residents. Victory House 566 Massachusetts Avenue Boston MA Phone: (617) Victory House offers 4 to 8 month residential substance abuse treatment program for men from all cultural backgrounds. Victory House has historically been committed to providing treatment to individuals who are severely physically or mentally impaired. We are also a leader in serving clients with HIV infection and AIDS. No one is denied admission to Victory House because of these attending health problems or needs. Shepard House 24 Windermere Road Dorchester, MA (617) Shepherd House is wheelchair accessible and smoke-free. Our six month program accommodates up to 24 women and provides a bridge between treatment and re-integration into community living, while learning to live drug and alcohol free.
