A copy of the eligibility registration form issued pursuant to section 6 of the PAK Rules of Tender (see page 3). 2. An original of a letter duly signed by or on behalf of the Bidder stating that the Bidder agrees to be bound by the Rules of Tender (see APPENDIX C, pages 6 and 7). 3. Proof that the bid deposit has been transferred to PAK account (see page 5 for further details). 4.
