12 PHS-Connect and My EMR Care Coordination - Results Delivery Orders to PHS Lab CCD / C-CDA document sharing and retrieval Meaningful Use - Transitions of Care (via Direct Messaging) Allows for seamless communications from directly within practice EMR system to another physician EMR, to a Clinical Portal physician with an attached C-CDA Secure Messaging from Provider to Patients (via Direct Messaging) Patient Portal View, Download and Transmit Physician must contribute C-CDA to PHS-Connect and provision patient users with access to the MyPinnacleHealth patient portal If Patient is provisioned for MyPinnacleHealth and views any CCD/C-CDA from any clinical source during the reporting period, the provider receives credit 12
