2 Page 2 of 6 This leaflet has been produced to help answer some of the questions that you and those who care for you may have about Stockport Specialist Palliative Care Service. Palliative Care aims to improve the quality of life of people (including carers) affected by serious illness. It can include people with cancer or other illnesses such as heart, lung, liver or neurological conditions. How we may help Working with other professionals involved in your care we aim to: Provide advice on symptoms that may be concerning you e.g. pain, breathlessness and sickness Provide emotional support for you and your family / carers in order to help you manage your illness Provide time for you or your family/carers to discuss concerns or questions about your illness and treatment and plan for the future when appropriate Help with complex discharge arrangements Help to aim and achieve your preferred place of care at home, hospital, hospice or care home Help with care at end of life Provide information about social, practical and financial issues Provide advice on other support services that are available Who we are The Stockport Specialist Palliative Care Team includes specialist nurses (often called Macmillan Nurses), Doctors, a Social Worker, Pharmacists and a Dietitian. All members of the team have specialist training and experience in caring for patients who have an illness that may not be curable, those with advanced conditions and people reaching the last few weeks of their life. The clinical team is supported by an administration team. Members of the team can visit and support patients and families at home, in hospital, in care homes, in medical clinics or nurse led clinics. One of the team will be your first point of contact (keyworker) for the service. However, you may receive support from other members of the team. 3 Page 3 of 6 Frequently asked questions: If I am at home, who should I contact if I have a problem? In the first instance please contact your GP or your District Nurse within normal working hours. Out of hours you can contact the on call doctors at Mastercall or out of hours District Nursing Team. I have seen the adverts for Macmillan Support is this the same as a Macmillan Nurse? The adverts you see are advertising available support from the Macmillan Charity not just the support that Macmillan Nurses can give. In addition to Macmillan Nurse support the Charity also provide a 24 hour advice line number There is a Macmillan Cancer Information Centre based at Beechwood Cancer Care Centre, Chelford Grove, Adswood telephone number Within Stockport there is also a Macmillan Welfare Rights Officer who can be contacted on Some people do not need to see a Macmillan Nurse or member of the Specialist Palliative Care Team. Although you have a Macmillan Nurse or member of the Specialist Palliative Care Team involved in your care, the majority of your support will be given by your GP or District Nurse. How often will you visit? This will vary depending on the level of support you require and will be discussed with you during your visits. Once I have been seen by a Macmillan Nurse will I be visited regularly? The Macmillan Nurse will agree with you how often she will visit. At times in the community, advice and support will be via telephone rather than face to face. If regular visits are not needed but things change and you have a problem please contact your GP and District Nurse. Once I have been seen by a Macmillan Nurse or Member of the Specialist Palliative Care Team will they always be involved in my care? You will have been referred to the team because you have specific needs that you require support for. Once these needs have been addressed you will be discharged from our care. However, if you require our help in the future you can be referred back to us by your doctor or nurse. Alternatively, you or your family can contact us directly. Students & Security Students may sometimes be on placement with the service. Patients will always be asked to consent to students being present or providing any care/treatment. All our staff wear identification badges and you should check this before allowing someone into your home.
4 Page 4 of 6 Confidentiality At each assessment with you, notes will be made and key information will be shared with other professionals involved in your care to support your ongoing care. You can be confident that your personal health records will be stored securely and remain confidential. For information about accessing your health records please speak to a member of staff. Contact us The Stockport Specialist Palliative Care Team are available to contact 7 days a week You can contact the team by telephoning: Community Specialist Palliative Care Team If you are unable to speak to someone please leave a message on the answerphone and we will return your call although it may not be the same day. At weekend and bank holidays messages will be taken off the answer phone and will be responded to if urgent.
