JOB #: 029MEM MINISTRY: Senior Living Courtyard DEPT: Residential / AL REPORTS TO: Memory Care Manager SUPERVISES: None PROPOSED FLSA STATUS: EXEMPT: NONEXEMPT: X (HR TO DETERMINE) Access to PHI: Level 2 May access minimum necessary PHI GENERAL SUMMARY: The primary function of the Dementia Activity Coordinator is to develop, implement and maintain a program of activities for the dementia residents through fostering an environment of social, mental, physical and spiritual wholeness. This position is an integral part of the senior community mission of providing joy and purpose to the lives of our residents. ESSENTIAL JOB DUTIES: (Responsibilities, Accountabilities, and Competencies; May not include all duties of this job) Job Duties 1. Program: Specialized dementia training. Facilitate resident programs and activities with a special focus on dementia care/memory support. Effectively communicate and interact with adults in the early to mid-stages of dementia. Listen for new ideas or concerns of residents and their families. Provide program support by using verbal and pictorial promotional materials. Maintain a balanced activity program designed to provide mental, spiritual, social, and physical opportunities for residents. Supervise and coordinate staff and volunteers who provide leadership and support for our programs and activities. Provide information and education on subjects of special interest to our senior residents and their families, such as Alzheimer s nutrition, eercise, insurance, voting, driving, transportation, volunteering, etc. Encourage and coordinate activities and programs which help bring resident community together. Communicate monthly, weekly activity schedule via posted calendars including electronic. Collaborate with Nurses, Caregivers and activity staff to maimize resident participation in the program. Assist in evaluating new residents who may benefit from specialized programming. Participate in programs and trips from time to time in order to develop rapport with residents and to evaluate the quality of the activity. 2. Administrative and Financial:

2 Page 2 Recommend the purchase of equipment and supplies in order to implement or maintain programs. Attend administrative, staff and resident meetings as assigned. Evaluate and provide input to the Administrator regarding program needs and prepare an annual budget to support scheduled events. 3. Work collaboratively with supervisor, coworkers, residents and families. 4. Perform other related duties as assigned. SUPERVISORY RESPONSIBILITIES: None ESSENTIAL QUALIFICATIONS: (Any Equivalent Combination of Knowledge, Skills, Abilities, Education and Eperience) 1. Christian Commitment: Must have and maintain a strong personal faith in Christ. 2. Education: Associate degree in Education, geriatrics or healthcare from an accredited college or university recognized by the United States Department of Education or equivalent. An Activities Certification or equivalent working eperience on a year-for-year basis can be substituted. 3. Eperience: Previous eperience working or volunteering with older adults. Eperience working with residents with Alzheimer s or dementia. 4. Licensure/Certification: 5. Software and Equipment Used: Proficient user of Microsoft Office Suite, such as Word, Ecel and Publisher. 6 Other Considerations: Enjoy working with seniors. Good interpersonal skills. Ecellent written and verbal communication skills with the ability to read, write, communicate in and interpret English at a functional level. Ability to find solutions to resident problems/concerns. Ability to work in an environment with constant interruptions. Fleible work hours; some evenings or weekend hours are required to support marketing events and maintain census goals. Display ecellent organizational skills. Follow schedules effectively and publicize events well in advance. Maintain good listening skills with a positive Christian attitude concerning life. Ability to be diplomatic, tactful, and courteous when dealing with residents and others. Ability to use digital camera and post pictures for resident boards. Ability to work collaboratively with supervisor, coworkers, residents and families. PREFERRED QUALIFICATIONS:

Education: Completed Dementia and Mental Health training as required by DSHS under Boarding Home Training Requirements. 2. Eperience: Activities with seniors. 3. Licensure/Certification: Dementia Care Certification, current Food Handlers Permit, current CPR and First Aid card. 4. Software and Equipment Used: Proficient user of Outlook and PowerPoint. 5.
