The use of the SMART (Specific, Measurable, Attainable, Relevant and Time-bound) format is preferred. What is the project s workplan? For example, for the WKKF-funded project activities, please describe who will do what, when, how, where, with whom, how frequently, and across what timeframe, etc. What are your strategies and efforts to secure funding beyond WKKF support? Please provide the project s funding sustainability plan. Additionally, include any information on the level or nature of risk(s) that have been identified for this project (strategic, reputational, organizational, legal, and/or programmatic) and how your organization plans to address/mitigate these risk(s). 1

Who are they and what are their roles/commitments? *Describe your organization s strategic approach to fulfill its mission and describe how the actual project you are requesting funding for fits into the overall strategic direction of your organization. *Project Team Diversity Please describe the policy work (if any) at the local, state, tribal, or federal level that will be included in WKKF-funded portion of this project. It is critical to indicate the type of policy (e.g. legislative, fiscal, system) and if WKKF funds will be used to support this work. If you are attempting to influence legislative or fiscal policies, how do you plan to advocate without lobbying? WKKF requires an evaluation report as part of the annual and final reporting process. An evaluation plan, describing evaluation activities for the project, is required as a component of the grant commitment, however it is not required for proposal review. ******************************************* If an evaluation plan is not yet developed, describe your organization s evaluation capacity here. If your project is funded, your program officer will provide additional information and a template to assist with developing a plan. This list should include any partners who are not providing funding for this project, and/or organizations you are working with. Briefly describe their role or commitment to the proposed project. In what ways is this organization best positioned to do the work toward achieving the desired project outcomes? How does the project fit into the organization s overall strategy? Please complete the chart for the activities being funded by the grant count each person only once based upon his/her role on the project. 3 Financial Due Diligence Questions As part of our financial due diligence process, the Foundation requires additional information related to your organization. Please respond to the questions below and provide the requested financial documentation. *Has the CEO, CFO and/or Executive Director for your organization changed within the last 2 years? Please explain. How often does your Board review the organization s financial reports? How often do other committees review the organization s financial reports? *Explain how your finance department is organized; list the name, title, qualifications and responsibilities for those primarily responsible for financial functions/reporting. *Select the option that applies to your finance staffing model. Within your organization, who is responsible for preparing checks and who is responsible for signing checks? How many check signers are required? *To help us determine what financial reports are required, please indicate if the aggregate amount of this funding request plus any currently active Kellogg grants is: Provide a brief explanation of position changes to executive staff in your organization.
