Received: ; Accepted: ; Published: Abstract Refractory pelvic pain is one of the common obstinate diseases in gynecological department, which is difficult to be diagnosed and treated because of complicated etiology and various symptoms, the aim of treatment is to improve functions and relieve pain. It has become a serious clinical problem for gynecologist with obvious rising-up of incident rate in recent years. This review summarized the treatment programs and effect of western medicine therapies, Chinese medicine therapies and integrated Chinese medicine and western medicine therapies on refractory chronic pelvic pain associated with various gynecological diseases Keywords: Refractory Chronic Pelvic Pain; Etiology, Treatment; TCM Therapy Refractory chronic pelvic pain is a commonly seen gynecological disease manifested by non-menstrual pelvic pain lasting for more than 6 months, which needs medicine, surgery treatment and even repeated treatment because of disabled functions due to the disease. Refractory pelvic pain usually appears continuous or intermittent, irregular dull pain, dropping sensation, distension or stabbing pain in lower abdomen or lumbosacral area, or accompanies leukorrhagia, which usually is aggravated by fatigue, intercourse, cold and emotional stimulation. It may produces negative results for patient s life and psychology, even induces many adverse events such as ectopic pregnancy, infertility and irregular menstruation [1]. It is found by our further literature research that the illustrations on refractory pelvic pain are very similar to chronic pelvic pain in some articles. 2 characteristics such as slow development, complicated etiology, various symptoms, prolonged course, repeated attack and tough recovery. So we believe that no strict distinction exists between the conceptions of refractory pelvic pain and chronic pelvic pain, refractory pelvic pain could be taken as refractory chronic pelvic pain. 1. Etiology Research 1.1. Etiology in Western Medicine The etiology of refractory chronic pelvic pain is complicated, which is not only caused by gynecological and obstetrical diseases, but also induced by disorders in other systems including urinary tract system, digestive system, skeleton and muscular system and neuropsychology [1]. Meanwhile, it could be influenced by hormone changes, mental factors, personalities, social factors and abuse. This review mainly discusses the gynecological diseases which may induce refractory chronic pelvic pain including endometriosis [2], pelvic and abdominal conglutination, pelvic inflammatory diseases (PID) [3], pelvic congestion diseases (PCS) [4] and pelvic benign and malignant tumor. Among those causes, the following factors are possible to produce refractory chronic pelvic pain, such as endometriosis, PID, PCS, uterine adenomyosis, ovarian remnant syndrome, uterine cervix or cavity adhesion due to abortion and post-abortion infection. 1.2 Etiology and Pathogenesis in Chinese Medicine It is believed in Chinese medicine that pathogenic qi fails to attack the body if healthy qi is full inside, while healthy qi should be deficient if pathogenic qi invades the body. Healthy qi and pathogenic qi are opposite aspects closely related to disease occurrence, development and transformation. According to its main clinical appearances, refractory chronic pelvic pain should pertain to the diseases of heat invading blood chamber, abdominal mass, infertility, female abdominal pain, leucorrhea disorders in Chinese medicine, although there is no special disease name recorded for it in ancient Chinese medicine. The pathogenesis of Chinese medicine is mainly at the basic of unrecovered healthy qi and unrestored uterus after menstruation or delivery, firstly pathogenic wind, cold, dampness, heat or insects toxicity invade the body, and combat with qi and blood of Chong and Ren channels, then accumulate in the uterus, secondly prolonged disease exhausts qi and blood, and thirdly, doctor overuses cold and attacking herbs to damage healthy qi by mistake. At the foundation of deficient healthy qi, qi deficiency produces blood stasis, which combines with heat and accumulates in Chong, Ren and Dai meridians, and produces unsmooth qi and blood circulation in uterine vessels, and further blood stasis blocks uterus and uterine vessels in order to produce pain due to obstruction, or Page 41 以 发 病 缓 慢, 病 因 复 杂, 症 状 多 样, 病 程 迁 延 及 反 复 发 作, 经 久 难 愈 为 其 特 点 因 此, 难 治 性 盆 腔 痛 与 慢 性 盆 腔 痛 的 界 定 并 没 有 严 格 的 界 限, 难 治 性 盆 腔 痛 可 视 为 难 治 性 慢 性 盆 腔 痛 1. 病 因 研 究 1.1. 西 医 的 发 病 因 素 难 治 性 慢 性 盆 腔 痛 的 病 因 复 杂, 不 完 全 是 由 妇 产 科 疾 病 引 起, 还 包 括 其 他 系 统 如 泌 尿 系 统 消 化 系 统 骨 骼 肌 肉 及 精 神 神 经 等 方 面 的 疾 病 [1], 且 受 诸 如 激 素 变 化 心 理 性 格 社 会 因 素 及 受 虐 待 等 因 素 的 影 响 本 文 主 要 探 讨 引 起 难 治 性 慢 性 盆 腔 痛 的 妇 科 疾 病, 能 引 起 难 治 性 慢 性 盆 腔 痛 的 主 要 妇 科 疾 病, 其 中 包 括 子 宫 内 膜 异 位 症 盆 腹 腔 粘 连 盆 腔 炎 性 疾 病 盆 腔 静 脉 淤 血 综 合 征 和 盆 腔 良 恶 性 肿 瘤 等 在 诸 多 的 因 素 中, 子 宫 内 膜 异 位 症 [2] 慢 性 盆 腔 炎 性 疾 病 (PID)[3] 盆 腔 静 脉 淤 血 综 合 征 (PCS)[4] 以 及 如 子 宫 腺 肌 病 残 余 卵 巢 综 合 征, 人 工 流 产 引 起 宫 颈 或 宫 腔 粘 连 人 工 流 产 后 感 染 等 均 有 可 能 导 致 难 治 性 慢 性 盆 腔 痛 1.2 中 医 病 因 病 机 中 医 学 认 为 正 气 存 内, 邪 不 可 干 ; 邪 之 所 凑, 其 气 必 虚, 正 气 邪 气 是 与 疾 病 的 发 生 发 展 转 归 密 切 相 关 的 两 个 对 立 的 方 面 中 医 古 籍 中 并 无 本 病 名, 但 根 据 其 主 要 的 临 床 表 现 可 归 属 在 热 入 血 室 癥 瘕 不 孕 妇 人 腹 痛 带 下 病 等 病 证 中 本 病 发 病 机 理 主 要 是 经 行 产 后, 胞 门 未 闭, 正 气 未 复, 风 寒 湿 热, 或 虫 毒 之 邪 乘 虚 入 侵, 与 冲 任 气 血 相 搏, 蕴 结 胞 中 ; 或 因 患 者 素 体 虚 弱, 病 程 较 长, 迁 延 难 愈, 耗 散 气 血 ; 或 因 医 者 过 用 寒 凉 攻 伐 之 品, 使 正 气 更 虚, 病 势 缠 绵 正 气 不 足, 气 虚 则 血 滞, 瘀 热 互 结, 留 滞 冲 任 督 带, 使 胞 脉 气 血 不 畅, 瘀 阻 于 胞 宫 胞 脉, 不 通 则 痛 ; 气 血 虚 弱, 胞

3 qi and blood deficiency fails to notify the uterus and uterine vessels, malnutrition of uterus and vessels may produces pain. Professor Deng Gaopi believes that the main pathogenesis of this disease is deficient healthy qi and combination of blood stasis and heat, generally speaking, it is excessive pathogenic factors with deficient healthy qi [5]. Professor Wu Keming believes that the basic pathogenesis is the combination of damp-heat, blood stasis and qi stagnation [6]. In general, the disease location is in the lower energizer, pathogenic factors are dampness, heat, cold and blood stasis, the characteristics of disease mainly are excessive pathogenic factors and combination of deficiency and excess. 2. Treatment It is relatively difficult to treat refractory chronic pelvic pain and usually it is without ideal long-term result of treatment, and even repeated treatment are needed for recurrently attack, because of complicated etiology and involved multiple systems Western Medicine Treatment Medication Principles of medication contain the following aspects, combination of treating etiology and relieving pain, highlighting improvement on functions, utilizing psychotropic drugs to improve psychological condition if psychological causes take part in, cascade application of analgesics [7,8] Nerve Block Treatment for refractory chronic pelvic pain Nerve block therapy has taken great development in treating refractory chronic pelvic pain, with that painful symptom has been relieved for more and more patients. Currently, various nerve block surgeries could be selected, which contains nerve block with laparoscopy or laparotomy, nerve damage with focused ultrasounds. The general mechanism is to stop pain by blocking the transmission of corresponding nerve which produces pain and cutting off nerve reflex arc which conducts pain. Wang Yanan [9] got a certain effect in the treatment for refractory pelvic pain by blocking pre-sacral nerve with laparoscope. Many literatures recorded that urine neurotomy by laparoscope may obviously relieve pain. Jia Heping [10] treated refractory pelvic pain by improved superior hypogastric plexus block therapy and proved it s safe and effective. Wang Fang [11] treated 30 cases refractory pelvic pain by High Intensity Focused Ultrasound (HIFU) on presacral nerve and on disease location of uterine adenomyosis, follow-up visit for 1-18 months, the result showed that HIFU treatment is safe and effective without wound and side effect for refractory pelvic pain. 宫 胞 脉 失 养, 不 荣 则 痛, 从 而 发 为 痛 证 邓 高 丕 教 授 认 为 正 气 不 足, 瘀 热 互 结, 正 虚 邪 实 是 本 病 的 主 要 病 机 [5]; 吴 克 明 教 授 认 为 本 病 以 湿 热 瘀 结 夹 气 滞 证 为 基 本 病 机 [6] 综 之, 本 病 的 病 位 在 下 焦, 病 理 因 素 有 湿 热 寒 瘀, 病 性, 以 邪 实 为 主, 虚 实 夹 杂, 缠 绵 难 愈 2 治 疗 难 治 性 慢 性 盆 腔 痛 致 病 因 素 复 杂 涉 及 全 身 多 个 系 统, 因 此 治 疗 比 较 困 难, 长 期 疗 效 不 甚 满 意, 并 会 出 现 经 常 复 发 与 反 复 治 疗 的 现 象 临 床 上 多 从 以 下 几 方 面 进 行 治 疗 2.1. 西 医 治 疗 药 物 治 疗 药 物 治 疗 原 则 包 括 如 下 几 方 面 : 治 疗 病 因 与 缓 解 疼 痛 相 结 合 ; 强 调 功 能 改 善 ; 精 神 因 素 参 与 时 可 用 精 神 药 物 改 善 精 神 心 理 状 态 ; 阶 梯 性 使 用 镇 痛 药 [ 7,8 ] 神 经 阻 断 术 治 疗 难 治 性 慢 性 盆 腔 痛 近 年 来 神 经 阻 断 术 在 治 疗 难 治 性 慢 性 盆 腔 痛 方 面 有 了 长 足 的 发 展, 使 越 来 越 多 的 患 者 疼 痛 症 状 得 到 缓 解 目 前 神 经 阻 断 术 方 式 已 经 多 样 化, 纵 观 可 分 为 三 类 : 开 腹 手 术 ; 腹 腔 镜 下 神 经 阻 断 术 ; 聚 焦 超 声 神 经 毁 损 术 大 致 原 理 为 阻 断 引 起 疼 痛 的 相 应 的 神 经 传 入 通 路, 致 使 传 导 疼 痛 的 神 经 反 射 弧 中 断, 从 而 使 痛 觉 消 失 王 亚 男 等 [9] 通 过 在 腹 腔 镜 下 切 断 骶 前 神 经 治 疗 难 治 性 慢 性 盆 腔 痛 取 得 了 一 定 的 疗 效 国 外 诸 多 相 关 文 献 显 示, 通 过 腹 腔 镜 下 子 宫 神 经 切 断 术 可 以 明 显 缓 解 患 者 疼 痛 的 症 状 贾 和 平 等 [10] 通 改 良 上 腹 下 丛 毁 损 术 治 疗 难 治 性 慢 性 盆 腔 痛, 提 示 此 法 安 全 有 效 汪 芳 等 [11] 同 年 对 30 例 妇 科 难 治 性 慢 性 盆 腔 痛 患 者 行 骶 前 神 经 热 辐 照 治 疗, 子 宫 腺 肌 症 还 行 局 部 病 灶 热 辐 照 治 疗 ; 随 访 观 察 l~18 个 月 不 等, 结 果 显 示,HIFU 治 疗 难 治 性 慢 性 盆 腔 痛 具 有 无 创 伤 无 副 作 用 等 优 点, 即 安 全 又 有 效 Page 42

4 2.1.3 psychotherapy The degree of pain due to refractory pelvic pain usually doesn t conform to the grade of lesion, because of prolonged torment by disease, psychological factor plays a very important role in the development of disease, some neuropsychological symptoms, like depression, suspiciousness and anxiety, are commonly seen in those patients. Some literatures acclaimed that refractory pelvic pain due to social psychological factors takes 5%- 25%. Therefore, social psychological factors in refractory pelvic pain should not be ignored. Psychological therapy should be applied with medicine to avoid or eliminate harmful influence due to social psychological factors, it could be taken up by the following aspects, 1. Educate patients that social psychological factors influence a lot on pain, 2. Overall evaluate patient s psychology, social communication and career and confirm the psychological cause, 3. Encourage patient s family to take part in the treatment for patient, 4. Dispel patient s doubts by psychological treatment, 5. Psychological treatment by professional psychologist should be applied if it is necessary. Incidence rate of psychological disorders is very high in refractory chronic pain, and it is usually accompanied with sex dysfunction, so the treatment should be based on psychosomatic medicine [12] Application of physical therapy in the treatment for refractory pelvic pain Because of not invasive treatment, the physical therapy is easily acceptable, and it is safe, economic and effective, many researchers focused on the application of physical therapy on refractory pelvic pain. Mo shengdi [13] treated patients 3 days after menstruation with heat therapy on pelvic by external highfrequency diathermy machine and got a good result. Electric stimulation may promote general blood circulation by accelerating muscular contraction and nerve transmission in order to relieve blood circulation and blood stagnation in the pelvic. In addition, local low-frequency electric stimulation may relieve pain by means of inhibiting prostaglandin secretion, counteracting prostaglandin and increasing pain threshold. It was proved by research that lowfrequency electric stimulation given by neuromuscular therapeutic apparatus in non-menstrual period may treat pelvic inflammation and pelvic congestion syndrome with a good result [14] Chinese Medicine Therapy Chinese medicine is a traditional therapy in China and plays an important role in the treatment for obstinate diseases, including various treatment techniques such as oral taking herbal decoction, external application, acupuncture and moxibustion, Chinese tuina (massage) and enema 心 理 治 疗 难 治 性 慢 性 盆 腔 痛 疼 痛 程 度 与 病 变 程 度 不 一 定 相 符, 由 于 患 者 长 期 受 病 痛 折 磨, 心 理 因 素 在 疾 病 的 过 程 中 起 着 重 要 的 作 用, 患 者 多 伴 有 抑 郁 多 疑 焦 虑 等 神 经 精 神 系 统 症 状 且 有 文 献 称 由 社 会 心 理 因 素 导 致 的 难 治 性 慢 性 盆 腔 痛 约 占 5%~ 25% 因 此, 心 理 和 社 会 因 素 对 难 治 性 慢 性 盆 腔 痛 的 影 响 不 容 忽 视 对 难 治 性 慢 性 盆 腔 痛 患 者 在 药 物 治 疗 的 同 时 辅 助 心 理 治 疗, 避 免 或 消 除 不 利 于 身 心 健 康 的 心 理 和 社 会 因 素 的 影 响, 从 而 达 到 治 疗 的 目 的, 可 以 从 以 下 几 个 方 面 着 手 :1 使 患 者 了 解 心 理 因 素 会 对 疼 痛 产 生 重 要 的 影 响 ;2 全 面 评 估 患 者 的 心 理 人 际 职 业 等 因 素, 明 确 心 理 因 素 ;3 督 促 患 者 家 属, 共 同 参 与 患 者 的 治 疗 ;4 采 用 心 理 治 疗 来 消 除 患 者 的 疑 虑 ;5 必 要 时 接 受 专 业 心 理 医 师 的 治 疗 难 治 性 慢 性 盆 腔 痛 患 者 精 神 方 面 改 变 的 发 生 率 很 高, 且 常 伴 有 性 功 能 障 碍, 应 从 心 身 医 学 的 角 度 出 发 [12] 物 理 疗 法 在 难 治 性 慢 性 盆 腔 痛 治 疗 中 的 应 用 由 于 物 理 疗 法 对 组 织 无 创 伤 或 创 伤 小, 患 者 容 易 接 受, 且 安 全 经 济 有 效, 现 有 许 多 学 者 着 重 于 研 究 物 理 疗 法 在 难 治 性 慢 性 盆 腔 痛 治 疗 中 的 应 用 莫 生 娣 等 [13] 在 特 发 性 盆 腔 痛 患 者 在 月 经 干 净 3 d 后 进 行 体 外 高 频 热 疗 机 对 盆 腔 的 局 部 热 疗, 治 疗 效 果 较 好 电 刺 激 可 加 速 肌 肉 收 缩 和 神 经 传 导 运 动, 使 机 体 血 液 循 环 加 速, 从 而 直 接 改 善 盆 腔 的 血 液 循 环 减 轻 盆 腔 血 管 淤 血 程 度 另 外, 局 部 低 频 电 刺 激 有 抑 制 前 列 腺 素 分 泌 对 抗 前 列 腺 素 的 作 用, 提 高 痛 阈, 从 而 缓 解 疼 痛 有 研 究 表 明 可 采 用 神 经 肌 肉 治 疗 仪 在 非 月 经 期 给 予 低 频 电 刺 激 治 疗 盆 腔 炎 和 盆 腔 淤 血 综 合 征, 效 果 显 著 [14] 2.2. 中 医 治 疗 中 医 药 是 我 国 的 传 统 医 学, 在 治 疗 疑 难 杂 病 中 有 着 重 要 的 作 用, 其 治 疗 方 法 多 样 化, 包 括 中 药 口 服 外 敷 针 灸 推 拿 灌 肠 等 多 个 方 面 Page 43

5 Internal therapy Experience Formula of Chinese Medicine Professor Deng Gaopi [5] believes that refractory chronic pelvic pain is root deficiency and superficial excess in Chinese medicine because of long course and chronic onset, so the treatment should strengthen the healthy Qi and eliminate pathogenic factors, treat the superficial and root simultaneously, his basic formula contains the following herbs, qi ye yi zhi hua, huang qi, dang gui, tu fu ling, ku shen, xue jie, modified according to symptoms. Professor Han Yanhua [15] treats refractory chronic pelvic pain with a good result by combined therapies of oral taking herbal decoction, enema and micro-wave therapy, his experience herbal formula mainly consists of herbs with action of activating blood and removing blood stasis and is modified and applied for different patterns, such as regulating qi, clearing away heat and removing dampness, resolving blood stasis and stopping pain, tonifying qi and nourishing blood, tonifying kidney, soothing liver and stopping pain, which may relieve pain and improve life quality. Professor Wang Chengrong [16] classifies refractory chronic pain into six patterns, among which his experience formula for qi stagnation and blood stasis is effective (huangqi, dangshen, baishao, roucongrong, xianmao, yinyanghuo, danggui, yanhusuo, chuanxiong, shechuangzi, taoren, xixin). Professor Wei Shaobin [17] emphasizes treatment priciples regulating qi and dredging meridians, resolving phlegm and accumulation, removing dampness and stopping pain, and treats with oral taking basic formula si ni san and shi xiao jin ling zi san which may stop pain by smoothing qi activities, regulating qi and blood, accompanied with other therapies such as herbal enema, suppository, herbal packet and steaming Single Herbal Formula Traditional herbal formula, dang gui si ni tang may warm meridians and dredge collaterals, tonify and nourish blood, regulate liver qi and stop pain. Lu Lifang [18] treated 48 cases refractory chronic pain by dang gui si ni tang with result of overall response rate 89.6%. Lei Jieying [19] treated 30 cases by oral taking ge xia zhu yu tang for 2 course with result of over response rate 86.7%, and indicated ge xia zhu yu tang is able to improve pain and psychological symptoms. Ran Qingzhen [28] reported she got an ideal result for refractory pelvic pain due to abdominal surgery by bian needle comprehensive therapy Integrated Chinese and Western Medicine Therapy In the past years, treatment for refractory pelvic pain tended to integrated Chinese medicine and western medicine therapy and got a better result than single western medicine or Chinese medicine treatment. Refractory chronic pelvic pain has a high incident rate and various etiological factors, it is a tough disease and reoccurs frequently, effect of single treatment with western medicine or Chinese medicine is limited, so the treatment by integrated Chinese medicine and Western medicine is a hot area for research. Lin Song [21] randomly divided post-surgery refractory chronic pelvic pain patients into two groups and applied dan e fu kang decoction for 1 months in treatment group, statistical significance was found by comparing treatment group and controlled group (P<0.005). Lei Chengshan [29] got a good result by 3 months treatment of shao fu zhu yu tang for the serious pelvic congestion syndrome patients more than 40 years old who had been done hysterectomy and ovary transposition by laparoscope. Liu Yiqin [30] treated refractory chronic pelvic pain by cephalosporin or macrolide antibiotics infusion accompanied with his experience formula hong teng tang and intra-vaginal microwave irradiation, he got a good result by 3-5 courses treatment. Some researchers compared combination of jin gang teng capsule and indomethacin suppository with single metronidazole vein infusion, the curative rate and remission rate of the combination therapy were significantly higher than the infusion. Some literatures said pelvic exercise could improve clinical effect with the foundation of integrated Chinese medicine and Western medicine therapy. 3 Summary and Outlook Generally speaking, refractory chronic pelvic pain is a syndrome due to various etiological factors, which damages body and metal health in women. Fenton [32] mentioned in his research that recurrence and repeated surgery were very common in refractory chronic pelvic pain. This article analyses the etiology and treatment strategy of Chinese medicine and Western medicine in details and illustrates the complicated and obstinate characteristics of refractory chronic pelvic pain by various etiological factors and means of treatment. Comprehensive treatment should be applied for refractory chronic pelvic pain due to the complicated pathogenesis, including drugs, surgery, physical therapy, internal therapy and external therapy of Chinese medicine and psychological therapy. We have got a certain effect on control and treatment for refractory chronic pelvic pain by hard work, especially in the research on the Chinese medicine treatment for refractory chronic pelvic pain, a group of effective herbal natural complex and compounds has been found.
