Administration - NCACC Annual Conference Voting Delegate Request that the Board of Commissioners ratify the appointment for Commissioner Pat Sykes as voting delegate to represent Brunswick County at the North Carolina Association of County Commissioners Annual Conference. 2. Administration - Bolivia Volunteer Fire Department Fundraiser Request that the Board of Commissioners consider a request from the Bolivia Volunteer Fire Department for permission to use the Administration parking lot at the Government Center on April 30, 2016 for a fundraising event. 3. Administration - Meeting Cancellations - Agenda Meeting& Regular Meeting Cancellations Request that the Board of Commissioners consider approving cancellations for the following meetings: Agenda Meeting- September 29, 2015 Regular Meeting- October 5, 2015 Agenda Meeting- December 15, 2015 Regular Meeting- December 21, Administration - Select Strategic Plan Meeting Facilitator Request that the Board of Commissioners consider awarding a contract in the amount of$ 2, to the Cape Fear Council of Governments to facilitate the public meetings for our upcoming strategic planning process. Also request that the Board approve a tentative calendar to schedule the public meetings between late September and mid- November of this year. 5. Administration - Holden Beach Bridge Beautification Project Request that the Board of Commissioners approve two agreements, one between the North Carolina Department of Transportation and the County, the other between the County and the Town of Holden Beach, to implement the beautification project for the approach to the Holden Beach Bridge. 6. Attorney- Southwood Developers Utility Deed of Dedication Consider accepting the Deed of Dedication for water infrastructure for Windsor Park Phase 3 Section Attorney- Naming of Long Beach Road and Old Long Beach Road To approve first reading and set public hearing on September 21, 2015 for the naming of Long Beach Rd and Old Long Beach Road. 8. Clerk to the Board - Meeting Minutes Request that the Board of Commissioners consider approving the August 17, 2015 Regular Meeting Minutes and the April 9, 2015 Budget Workshop Minutes. 9. Emergency Services - Grissettown Facility Use Agreement Request that the Board of Commissioners approve a Facility Use Agreement with Grissettown/ Longwood Volunteer Fire Department for the use of their facility as a fire/ rescue training site. 10. Emergency Services- Grissettown Search Management Agreement

2 Request that Board of Commissioners approve agreement with Grissettown/ Longwood Volunteer Fire Department for the operational management of the Brunswick County Land Search Team. 11. Emergency Services - Shallotte Interlocal Agreement for Fire Training Facility Request that the Board of Commissioners approve an Intergovernmental Agreement with the Town of Shallotte for the use of the Shallotte Regional Fire Training Facility. 12. Emergency Services - Oak Island Water Rescue Agreement Request that the Board of Commissioners approve the annual contract with Oak Island Water Rescue. 13. Emergency Services - Brunswick Search and Rescue Agreement Request that the Board of Commissioners approve the annual agreement with Brunswick Search and Rescue. 14. Emergency Services - Mutual Aid Agreement with New Hanover Regional EMS Request that the Board of County Commissioners approve an updated mutual aid agreement between Emergency Services and New Hanover Regional Medical Center Emergency Medical Services for county to county Emergency Medical Services mutual aid. 15. Engineering- Leland BSRI Senior Center- Site Demolition Request that the Board of Commissioners approve site demolition work by Operation Services for the Leland BSRI Senior Center project in the amount of$ 24, Finance - Fiscal Items Request that the Board of Commissioners approve Budget Amendments, Capital Project Ordinances and Fiscal Items of a routine nature presented on the consent agenda. Request that the Board of Commissioners ratify the decision to approve the Sheriffs Office Application for Live Scan Grant from NC State Bureau of Investigation. The grant fully covers the cost of the live- scan equipment and software maintenance for the the first 3 years in the amount of$ 26, 875 and requires the county to provide the funding in the amount of$ 3, 090 for year four and $ 3, 183 for year five of the maintenance contract as a match to the grant. JCPC FY16 Administrative Budget Amendment Appropriate State Revenues Restricted of$ 7, 000 for JCPC Administration as awarded by the Juvenile Crime Prevention Council from the NC Department of Public Safety. JCPC FY16 Strengthening Families Budget Amendment Appropriate State Revenues Restricted of$ 22, 000 for JCPC Strengthening Families Program as awarded by the Juvenile Crime Prevention Council from the NC Department of Public Safety. Northeast Regional Capital and Replacement Fund Transfer $ 121, 536 to the NE Regional Capital and Replacement Fund from the Wastewater Fund for the FY15 operating and maintenance surplus in the NE Regional Facility as required by the participants agreement and appropriate interest earned in the wastewater reserve fund for NE Regional Capital and Replacement Fund in that amount of$ 98. West Regional Capital and Replacement Fund Transfer $ 800, 311 to the West Regional Capital and Replacement Fund from the Wastewater Fund for the FY15 operating and maintenance surplus in the West Regional Facility as required by the participants agreement and appropriate interest earned in the wastewater reserve fund for West Regional Capital and Replacement Fund in that amount of$ 99. Sheriff' s Office Insurance Proceeds Budget Amendment Appropriate Insurance proceeds received in FY15 for four Sheriffs Office total loss vehicles ending Vin #' s 6217, 1856, 8125, and 8972 in the amount of$ 72, 534 for replacement of these vehicles. Insurance proceeds were received too late in fiscal year for vehicle replacement purchases.

3 Parks and Recreation Maintenance FY15 commitments continued in FY16 Appropriate fund balance of$ 11, 076 of parks and recreation maintenance overtime associated with a tournament budgeted in FY15, but paid out of FY16 due to the timing of year end payroll. Rural Operating Assistance Program Request the Board of Commissioners approve the County Manager to move forward with the application and appropriate Rural Operating Assistance Program ( ROAP) funding of$ 208, 875 from NC Department of Transportation. The entire allocation of funds will be pass through to Brunswick Transit System, Inc. Lockwood Folly Navigation Dredging Project Appropriate$ 101, 828 of funding originally obligated for the FY14 Lockwood Folly project for use in the forthcoming Lockwood Folly Navigation Maintenance project. The original crossing project achieved significant cost savings and the Town of Holden Beach is requesting to utilize the County' s portion of those cost savings in the amount of$ 101, 828 for the Lockwood Folly Navigation Maintenance project starting in September of 2015, The County' s cost share of the Navigation Maintenance project is less than the FY 14 project. 17. Health and Human Services- Request Approval of Document Scanning Contracts Request that the Board of Commissioners approve the scanning contracts with Confidential Records Management in the amount of$ 223, for the back scanning of documents in the Environmental Health and Social Services programs. 18. Health and Human Services Services- Request Approval for Debt Write Off Request that the Board of Commissioners approve the debt write off, incurred for clinical services for the time period of July 1, 2008 through December 31, 2013, in the amount of$ 249, , and approve the NC Local Government Debt Set- off Clearinghouse to be used to collect debts. 19. Health and Human Services - Request Approval of the Brunswick Senior Resources Inc. Request Budget Neutral Revision# 5 to Home and Community Care Block Grant Funding FY2015 for Brunswick County Request that the Board of Commissioners approve and execute the attached FY 14/ 15 Home and Community Care Block Grant for Older Adults Budget Document, revision number 5, for year ending Human Resources - Policy Revisions Request that the Board of Commissioners adopt revisions to County Personnel Policies: 110 Use, Revision and Exclusions of Policy Manual; 280 Political and Civic Activity; 472 Wellness Program. 21. Operation Services- Ground Water Monitoring Request that the Board of Commissioners approve a service agreement with AMEC Foster Wheeler Environment Infrastructure, Inc. ( AMEC) for semi- annual sampling of ground water wells at the Brunswick County Landfill as required by the North Carolina Department of Environment and Natural Resources, Division of Waste Management ( DENR). 22. Parks& Recreation - Request for Qualifications for Ocean Isle Beach Park Project Phase 2 Site Master Plan Request that Board of Commissioners consider approving the RFQ for the Phase 2 Site Master Plan at Ocean Isle Beach Park. 23. Sheriffs Office- Governors Highway Safety Program Traffic Safety Project 3rd Year Contract Request that the Board of Commissioners approve the Resolution for the Traffic Safety Funding Application with the NC Governor' s Highway Safety Program. 24. Sheriffs Office - Governors Highway Safety Program Traffic Safety Project 5th Year Contract Request that the Board of Commissioners approve the resolution for Traffic Safety Funding Application with the NC Governors Highway Safety Program. 25. Sheriffs Office - Additional Vice/ Narcotics Agent Request that the Board of Commissioners appropriate an additional FTE and funding for a vice/ narcotics agent. 26. Tax- September 2015 Releases Request that the Board of Commissioners approve the September 2015 tax releases.

Tax- August 2015 MV Discovery Valuation& Levy for July 2015 Request that the Board of Commissioners approve the August 2015 motor vehicle discovery valuation and levy for motor vehicles discoveries created in July Utilities - Request for Approval of NC DOT Antenna Agreement Amendment Request that the Board of Commissioners approve an amendment to an existing agreement with the North Carolina Department of Transportation ( NC DOT) for the location of an antenna on the Shallotte Elevated Water Storage Tank adjacent to the NC DOT Maintenance Facility. 29. Utilities - Request for Approval of Ocean Isle Beach Wastewater Treatment Plant, Pump Station, and Force Main Improvements Contract Final Adjusting Change Order DeVere Request that the Board of Commissioners approve the final adjusting change order on the Ocean Isle Beach Wastewater Treatment Plant, Pump Station, and Force Main Improvements Contract which adds $ 3, to the contract. 30. Utilities - Request for Appointment of Board Member( s) to NC 211 Water Treatment Plant Consultant Selection Committee Request that the Board of Commissioners appoint a member or members to the NC 211 Water Treatment Plant Improvements Consultant Selection Committee. VII. Presentation I. Administration - Proclamation in Honor of Parks & Recreation' s Special Populations Program Being Recognized - ( Amanda Hutcheson, Public Information Officer) Request that the Board of Commissioners adopt a proclamation honoring Parks and Recreation, which has received the Distinguished TR Program Award from the North Carolina Recreation and Park Association. 2. Parks & Recreation - Recognition of 2015 West Brunswick Dixie Debs Softball Team- ( Aaron Perkins, Director of Parks & Recreation) Request that the Board of Commissioners recognize 2015 West Brunswick Dixie Debs Softball Team as the 2015 World Series World Series Champions. 3. Recovery Month Proclamation - ( Mandy Anderson, Wilmington Treatment Center) Request that the Board of Commissioners consider approving the Proclamation proclaiming September as National Recovery Month. VIII. Public Hearing 1. Administration - Public Hearing to Adopt County Utility Rates in the Caswell Beach Service Area( Ann Hardy, County Manager) Request that the Board of Commissioners conduct a Public Hearing regarding adopting county utility rates and fees in the Caswell Beach service area and after the hearing consider adoption. IX. Administrative Report 1. Utilities - Request for Approval of Submittal of Proposal to the City of Southport for Operation and Maintenance of Water and Sewer Systems- ( Jerry Pierce, Director of Public Utilities) Request that the Board of Commissioners approve submitting a proposal to the City of Southport for the operation and maintenance of their water and sewer systems. 2. Utilities - Request for Approval of ASR Systems LLC Contract for Phase II Aquifer Storage Recovery Project- ( Jerry Pierce, Director of Public Utilities) Request that the Board of Commissioners approve a contract with ASR Systems LLC for the work associated with Phase II of the Aquifer Storage and Recovery ( ASR) Project. The contract amount is $ 215, 493 for engineering and $ 152, 000 for construction of the test wells and monitoring wells. 3. Utilities - Request for Approval of Initial Priority List for Enterprise Funded Sewer Main Extensions

5 Program - ( Jerry Pierce, Director of Public Utilities) Request that the Board of Commissioners approve the initial Priority List for the Enterprise Funded Sewer Main Extensions Program. X. Board Appointments 1.
