2 Introduction MiCD Symposium 2011 Welcome to the second regional MiCD symposium featuring a comprehensive scientific program that encapsulates the broader scope of Minimally Invasive Cosmetic Dentistry (MiCD) and its relevance in clinical dentistry today. A growing number of clinicians have embraced MiCD movement by successfully integrating minimally invasive cosmetic dentistry in their practice. The adoption of minimally invasive techniques and principles has added a new dimension to cosmetic dentistry as it promotes the conservation of sound tooth structure while achieving predictable smile aesthetics and a positive psychological impact on the patient. The second regional MiCD symposium is being held in collaboration with the Chulalongkorn University, following the success of the first regional MiCD symposium in 2010 in Asia. 3 By Dr. Sushil Koirala MiCD Concept and Treatment Protocol Brief CV: Dr. Sushil Koirala is the founding president of Vedic Institute of Smile Aesthetics (VISA) and the chief instructor of VISA- Comprehensive Aesthetic Dentistry (V-CAD), a two- year training program based on Vedic philosophy of beauty & aesthetics. He maintains a private practice focusing primarily on minimally invasive cosmetic dentistry (MICD) and has more than 17 years of clinical experience in aesthetic dentistry. Dr. Koirala has developed the Vedic Smile Concept, the Smile Design Wheel, MICD Treatment Protocol and various clinical techniques for direct aesthetic restorations. He is the Vice-President of Asian Academy of Aesthetic Dentistry (AAAD) and International College of Continuing Dental Education (ICCDE). Dr. Koirala is also the founding president of Nepalese Academy of Cosmetic & Aesthetic Dentistry (NACAD) and South Asian Academy of Aesthetic Dentistry (SAAAD). He has published numerous clinical articles in aesthetic dentistry and authored A clinical guide to Direct Cosmetic Restorations with Giomer, published by Dental Tribune International GmbH, Germany. Dr. Koirala also serves as the executive council member of International Federation of Esthetic Dentistry (IFED) and editor-in-chief of Cosmetic Dentistry beauty & science of DTI. He frequently conducts handson programs and delivers lectures globally on smile aesthetics. Synopsis: The treatment modalities and protocol of any health-care service should be aimed at the conservation of the healthy tissues and establishment of its natural function and aesthetics. The comprehensive concept of minimally invasive cosmetic dentistry (MICD) and its treatment protocol were introduced by the presenter in 2009 with the basic aim of a clinician effecting optimum clinical therapeutic improvements in smile enhancement, while performing corrective procedures that require as little clinical intervention as possible. The intervention level of the treatment in MICD however, depends on the type of smile defect and the aesthetic needs of the patient. The presenter will demonstrate simple to complex clinical cases of smile enhancement which were treated satisfactorily adopting the MICD-TP (MiCD-treatment protocol). By Dr. Dinos Kountouras Minimally Invasive Cosmetic Dentistry: Customized Rehabilitation of the Smile using Minimal and non Invasive Restorations Brief CV: Dr. Dinos Kountouras qualified from the University of Liverpool. He received his Master of Science degree in Prosthetic Dentistry from the Eastman Dental Institute, University of London. He was awarded the Fellowship in Implant Dentistry from the department of Implant Dentistry of New York University. He received his PhD in Biomaterials from the University of London with his thesis on fatigue of resin bonded systems. He is a Lecturer in the Department of Restorative Dentistry at the Aristotle University of Thessaloniki, Greece. He is the founding and current President of the Hellenic Academy of Aesthetic Dentistry. He maintains a private practice in Thessaloniki, Greece with a special emphasis in aesthetic reconstructions and implant dentistry. Dr. Kountouras lectures nationally and internationally in the fields of Aesthetic and Implant Dentistry. Synopsis: Modern restorative dental treatment should be characterized by the adoption of a minimally invasive methodology. Current aesthetic restorative materials have optical and mechanical properties that closely mimic the respective properties of dental tissues and can be used to create minimal or non-invasive biomimetic restorations that best replace or augment missing tooth structure. Bonded restorations can restore the aesthetics and form of individual natural teeth and appropriate treatment planning will also allow for the patients functional and aesthetic demands to be met. When this biomimetic restorative methodology is combined with a predictable treatment planning protocol, it will allow for the smile to be designed in harmony with the functional needs of every patient and at the same time create an aesthetic result that will best fit each patient s character and overall expressions; it can then be ensured that the highest aesthetic level can be achieved during our restorative rehabilitations

By Dr. José Roberto Santana de Moura Junior Precision in Minimally Invasive Direct Composite Restorations: a Microscope Perspective Brief CV: Dr. Adrian Yap is a Consultant and Prosthodontist at Raffles Hospital. He is also the Dental Director of Raffles Dental / Raffles Hospital and an Adjunct Associate Professor at the Faculty of Dentistry, National University of Singapore (NUS) and School of Science and Technology, SIM University (UniSIM). He obtained his PhD, BDS and Graduate Diploma in Psychotherapy from NUS and MSc (Conservative Dentistry) from the University of London. Dr. Yap is known for his expertise in Temporomandibular Disorders, Occlusion and Restorative Dentistry and has lectured worldwide in these areas. He has authored more than 160 refereed journal articles, 400 conference / seminar papers as well as several book chapters. Dr. Yap has received numerous clinical, research and teaching awards and serves on the editorial boards of several international journals. Synopsis: MiCD aims to correct aesthetic dis-ease and fulfill aesthetic demands of patients using conservative and minimally invasive treatment options. This two-part lecture focuses on the Occlusion and materials aspect of MiCD. Occlusion is not about static structural relationships but the functional relationships between the various components of the masticatory system. If Occlusal Disease is not recognized and occlusion is not properly managed, restorations will inadvertently fail. Discussion of quality of life issues including patients wants, needs and expectations are important during patient selection for MiCD especially when occlusal problems are present. A wide spectrum of direct restorative materials, ranging from glass ionomers to composite resins, can be used for MiCD. The relative merits of the continuum of direct restorative materials will be reviewed. Giomers are the most recent category of glass ionomer-composite hybrid restorative materials. They are particularly useful for MiCD procedures in view of their good aesthetics, handling and anticaries properties. Possible risks and benefits associated with MiCD should be communicated to patients as well as the need for teeth / restoration protection. Brief CV: Dr. José Roberto Santana de Moura Junior (DDS, MS) graduated from the University of Taubaté (Brazil) in 1983 with a specialty degree in Restorative Dentistry and later completed his master s degree in Prosthodontics at the same university. Dr. Moura served as President of the Brazilian Society of Aesthetic Dentistry for the years 1999 ~ 2000 and 2002 ~ He is in the Advisory board of the Dental Press Esthetic Journal, the official publication of the Brazilian Society of Aesthetic Dentistry and in the Board of the Academy of Microscope Enhanced Dentistry (AMED), USA. Dr. Moura is currently President of the International Federation of Esthetic Dentistry (IFED). He lectures all over Brazil and internationally (USA, Europe and Asia) on Esthetic Dentistry and Clinical Microscopy. He maintains a private practice in Taubate, Brazil. Synopsis: Direct composites are one of the most popular and yet conservative techniques worldwide. However due to the critical nature of the adhesive materials and composites there are many aspects that must be carefully considered in order to achieve superior esthetic results and longevity on an everyday basis. We intend to discuss these aspects and also the use of the operative microscope during the whole restorative procedure in a way that is ergonomically comfortable for the dentist, less time consuming than with no magnification and achieving the most satisfying, long lasting results for the patient

5 By Dr. Noboru Takahashi Challenges in achieving the Ultimate Restoration: A Minimally Invasive Approach By Dr. Ko Hinoura Downsizing caries treatment to prolong tooth life Brief CV: Dr. Noboru Takahashi obtained his DDS degree at Tokyo Medical and Dental University, Tokyo Japan in He is a clinical lecturer at the Tokyo Medical and Dental University and has published numerous clinical papers on composite and ceramic restorations. He also lectures internationally on direct and indirect aesthetic restorations. Dr. Takahashi is a member of the American Academy of Cosmetic Dentistry, American Prosthodontic society and Vice-President of Design Technique International. He maintains a private practice in Tokyo, dedicated to Restorative and Prosthodontic dentistry. Synopsis: The growing trend towards more conservative dental procedures in cosmetic dentistry is becoming popular among dental professionals. Patients are also increasingly demanding superior aesthetic results for their dental restorations. Thus direct aesthetic restorations, which are less invasive, have become an essential treatment option in restorative dentistry. This lecture will highlight crucial elements for attaining optimal functional and aesthetic outcome in direct aesthetic restoration while adopting a minimally invasive approach. In particular the speaker will address the predictability and productivity of the direct aesthetic restorations, achieved through accurate diagnosis, meticulous treatment planning and step by step clinical procedures. The concepts of 3D Gradation Layering Method and Anatomic Navigation System formulated by the presenter will be discussed with an insight to the latest direct aesthetic restorative resin systems and a special focus on the unique functional direct bioaesthetic restorative material Giomer and its usage. Brief CV: Dr. Ko Hinoura (D.D.S, Ph.D) is the director of Hinoura Dental office in Tokyo and obtained his Ph.D and D.D.S. from Nihon University, Tokyo, Japan. He is a visiting Lecturer, Nihon University School of Dentistry, Niigata University, Kyushu University and Indiana University School of Dentistry. Dr. Hinoura is an active member of The Japanese Society of Conservative Dentistry, Japan Society for Adhesive Dentistry, Japan Academy of Esthetic Dentistry, The Japanese Society for Dental Materials and Devices and International Association for Dental Research. Synopsis: The notion of caries as an oral disease and the technique to treat caries, including the adhesive restoration and conservative cavity preparation design, have been dramatically changed since the introduction of the guidelines by G. V. Black. Particularly, there is a growing move trying to extend tooth longevity by minimizing the repeated restoration cycle. In order to restore the carious tooth, first we need to control the micoflora, then eliminate the caries and restore affected tooth back to its original form. Downsizing, in this context, means not only downsizing (minimizing) the amount of cutting sound and healthy tooth structure, but also downsizing (reducing) the oral bacteria to maintain oral hygiene. With this downsizing treatment, natural dentition can be maintained for a longer period. The philosophy behind this treatment will be discussed in this lecture with an attempt to comprehend and put all the current knowledge together, such as prevention, remineralization, ion exchange and adhesion, with an aim to restore the carious tooth in the simplest and least invasive manner

Adhikari is the founding member and President of Nepalese Academy of Cosmetic and Aesthetic Dentistry (NACAD), Vice-President of South Asian Academy of Aesthetic dentistry (SAAAD), country representative of Asian Academy of Aesthetic Dentistry (AAAD) and founding member of the National Dental Hospital, Nepal. He has published various articles related to digital dentistry and dental photography. Dr. Adhikari serves as a member of editorial board of Cosmetic dentistry (DTI) and Dentistry South Asia, an online dental magazine. He maintains a private practice focusing on cosmetics and orthodontics. Dr. Adhikari conducts hands-on program in dental photography and is the Trainer in Digital Dentistry at the Vedic Institute of Smile Aesthetics (VISA). Synopsis: Case documentation is an essential component of any treatment protocol for treatment planning, rendering the planned treatment and follow up. Digital technology is being widely adopted in dentistry today and considered critical for case documentation, practice management and continual self-assessment. MiCD treatment protocol demands detailed and precise case documentation in all three phases of its protocol. In the Understand phase, case documentation enables recording and evaluating diseases / defects, designing possible treatment options, presenting these options to the patient and preparing the informed consent. In the Achieve Phase, details of the clinical procedure are documented in an orderly manner while documentation in the keep in touch phase plays a pivotal role in patient education and motivation. Digital case documentation requires less time and space which is the need of the hour.
