2 OUR NON-PROFIT mission is simple It s all about quality of life and we make it our mission to respond to community needs and develop innovative services that help individuals live their best life. From the grandfather receiving a nutritious meal right to his door, to the mother struggling to maintain sobriety, to the active older friend recovering after a surgery our continuum of services is uniquely positioned to meet the holistic needs of each individual we serve. The goal of Community First Solutions is to be THE non-profit resource for meeting the needs of our neighbors throughout Butler County. We seek to build collaborative relationships with partners who share our drive; working together to sustain programs that keep our community well. THE NON-PROFIT DIFFERENCE Community-based volunteers, not corporate investors, govern Community First Solutions. Our Board of Trustees offers valuable leadership that ensures we remain true to our mission, respond to local needs and serve as an effective steward of services and resources. Being a non-profit means that any revenue generated is reinvested in the mission of the organization. We reinvest our funds in improving quality, expanding the scope of our services and developing innovative programs to meet community needs.
3 FOR NEARLY A CENTURY, the non-profit businesses that make up Community First Solutions have been committed to the greater Hamilton community. In 1918, the single gift of a Dayton Street home established Colonial, a leading provider of older adult and multigenerational services. In 1954, Partners in Prime developed the first Senior Center in the state of Ohio. In 1974, Horizon Services, now Community Behavioral Health, became one of the first behavioral health providers in Butler County. In 2008, Community First Pharmacy opened, establishing the 2nd full service retail not-for-profit pharmacy in the nation. In 2010, these organizations united under their non-profit parent company, Community First Solutions, and committed to developing dynamic services that help more people, more often. I am very proud of the growth of our organization but the most rewarding part of my job is seeing our employees develop. 5 EVERY DAY, OUR EMPLOYEES bring our mission to life. SAVING ON PRESCRIPTIONS The best part of Ashley Hoehn s job is interacting with clients. Whether they have insurance or not, all clients are hoping to save as much as possible on medication. Getting to know them on a personal level, Ashley says, makes her a better pharmacist. Our mission every day is to save patients money but also provide them with the best medication. If I can find a medication that is substantially below the price or even copay, especially for clients who don t have insurance, it makes a huge impact on their lives. FINDING HOME Shelly Henderson tells people the best part of her job is guiding families through the process of moving to a retirement living community whether they are seeking to downsize and live maintenance-free or need more healthcare assistance. Initially, most people are worried about losing independence, but once they move in, residents find that they re more active and more independent than they ever expected. From trips to the Reds games to happy hours, they are doing all kinds of fun things. They ve joined a big family. A CONTINUUM OF CARE As the Admissions Coordinator for Berkeley Square and Westover, Dinah Kellum works with area hospitals to ensure that after an illness or surgery, the transition to skilled rehab care goes as smoothly as possible. She s seen a lot of change very positive change for clients. You re not just coming in for rehab anymore. We have a full continuum of services to help you regain independence quickly from inpatient rehab, to rehab at home and outpatient therapy. After a stay, we can assist with managing medication or preparing meals, provide transportation to your appointments, and even maintain your strength at Elements Wellness Centers. We work very closely from admission through follow-up care to ensure the highest quality outcomes.

7 OUR SERVICES COME TO LIFE with our employees MORE THAN A MEAL Of the 168,727 meals delivered by Partners in Prime s Meals on Wheels Program last year, Don Peters delivered close to 7,000 of them. But what s even more impressive, is the way in which he did it. Having been a Meals on Wheels driver for 12 years, Don knows each recipient by name, knows what kind of pets they have, asks about their kids and grandkids by name, and stops at the end of their driveways to bring in the newspaper or garbage cans. No surprise that the best part of his job is the clients. Can t you imagine how you just fall in love with the people? FOUNDATION FOR THE FUTURE At Colonial Schools students play with a purpose, and that purpose is to learn. Veteran preschool teacher Marianne Honchell treats her classroom like a stage singing, dancing, entertaining and most importantly, teaching. We try to go above and beyond to get to know the families and what they value so that we can make a connection with the student and make them feel safe. I love working one-onone with my students, getting to know them and drawing out their personalities. TREATING HOLISTICALLY Ohio has a critical shortage of psychiatric hospital beds, making Great Miami Services vital for Butler County. Beyond a step-down from psychiatric hospitalization, the facility serves anyone with a mental illness who may need a medication adjustment, individuals who are not eligible for hospital admission but not safe at home, and families who may need a temporary respite location for an adult child with a severe mental illness. Watching clients succeed at GMS is Tara Cunningham s favorite part. We had a client who came in after losing everything. We helped her become stabilized with medication, connected her to the outpatient services she needed, and worked with Children s Services and Ohio Means Jobs. Now she has fully regained custody of her children, she has a job and a home.
