1 Registration for the Healthy Foods Reward Card To register for the Healthy Foods Reward card you must first complete your health assessment on the Vitality site and be at Bronze status. After the health assessment is complete, click on the Get Healthy Tab at the top of the page and then select the Vitality Healthy Foods from the drop down menu. The next screen will have a link on the right side of the page to apply for your Healthy Foods Reward Card. The card will be mailed to you in 7-10 business days.

2 If you are at Bronze status you will receive a 5% discount on healthy food purchases and when you reach Silver status you will receive a 10% savings.

In the Get Healthy section, accept the Vitality HealthyFood terms and conditions to activate your Vitality HealthyFood Shopping Card. 4. Receive your Vitality HealthyFood Shopping Card in 7-10 business days. 5. Increase your 5% savings to 10% when you or an eligible HumanaVitality member in your family completes a Vitality Check in your current program year. 4 Once your Vitality Check results are received and verified by HumanaVitality, your savings will increase to 10%! Sign in to HumanaVitality.com to take the Health Assessment, if you haven t already. Then, request your Vitality HealthyFood Shopping Card right online. 1 The 5% or 10% savings will be applied to that member s Vitality HealthyFood Shopping Card within 5-7 business days. Once applied, the savings may be used on any future purchase made at Walmart where gift cards are accepted. 2 The Vitality HealthyFood program is only available at Walmart Neighborhood Markets and Walmart retail stores. 5 Shopping Card Shopping Card Important things to know about using your Vitality HealthyFood Shopping Card. Q. How do I know if my Vitality HealthyFood Shopping Card was accepted at checkout? A. Check your receipt. Below the total you should see the words Shopping Card Tend. This shows that your Vitality HealthyFood Shopping Card was accepted at the register. Q. How soon after I use my Vitality HealthyFood Shopping Card will I receive my savings, and when can I redeem it? A. The savings you have earned on your Great For You healthier food purchases will be applied to your Vitality HealthyFood Shopping Card in 5 7 business days. (These savings will not appear on your receipt the day of your transaction.) Once your savings are applied, you can redeem them on a future purchase at Walmart where gift cards are accepted. (Within 5 7 business days, you can also view the savings from your most recent purchase on your Vitality HealthyFood Savings Statement at HumanaVitality.com.) Q: How do I increase my savings from 5% to 10%? A: You (or an eligible HumanaVitality member in your family) must complete a Vitality Check in the current program year 1 to increase your savings. After the cashier has told you the amount you owe, tell the cashier you have a Vitality HealthyFood Shopping Card. Have your Vitality HealthyFood Shopping Card scanned before you pay.
