On-Call /Call Back Terms: On-Call - $5.00 per hour when the health care provider is assigned to carry a pager or cell phone and provide phone consultation. The provider must also be within 30 minute radius of the facility. The overtime rate will apply once the provider is physically on site and the normal hourly rate applied when employee is physically on site at facility with a 2 hour minimum. The Call Back rate will be at the regular hourly rate up to 40 hours a week and when hours exceed 40 then the overtime rate will apply. 15. Holiday Terms: 1.4 x normal hourly rate for all federal holidays actually worked. 16. Overtime Terms: 1.3 x billable rate 17.
