Minimum Qualifications of Professionals: : Physician - General and Family Practice The general and family practice physician provides examinations and medical care for the entire family. The physician prescribes and administers medications, performs routine vaccinations, and provides advice regarding personal health and hygiene. The physician will request necessary tests and follow- up visits and refer patients to specialists, as necessary. Degree in medicine from an accredited school of medicine. Physicians will maintain a current, active, valid, and unrestricted medical or osteopathic medical license from a U.S. State. Not Current, Valid DEA number : Physician Psychiatrist (General Adult, Geriatric) The Psychiatrist valuates patients for mental, emotional or behavioral disorders. This mental health specialist diagnoses disorders, including nature and extent, and determines the appropriate course of treatment for the patient. The Psychiatrist may prescribe medications when necessary. Degree in medicine from an accredited school of medicine. Psychiatrist will maintain a current, active, valid, and unrestricted license from a U.S. State. Possess board certification in Psychiatry by the American Board of Psychiatry and Neurology or the American Osteopathic Association. Not 6

Plans, directs and conducts rehabilitative treatment programs. Master s Degree in Speech Pathology. Current, unrestricted license to practice as a speech pathologist in one of the 50 U.S. States. Must possess a Certificate of Clinical Competency in Speech Pathology (CCC- SLP) from the American Speech- Language- Hearing Association (ASHA). Not : Physical Therapy Physical Therapist Assess, plan, organize, and participate in rehabilitative programs that improve mobility, relieve pain, increase strength, and decrease or prevent deformity of patients suffering from disease or injury. Not required : Medical Records Clerk Medical records and health information technicians assemble patients' health information including medical history, symptoms, examination results, diagnostic tests, treatment methods, and all other healthcare provider services. They regularly communicate with physicians and other healthcare professionals to clarify diagnoses or to obtain additional information. High School Diploma and in a Medical Assistant Program. 10 The Ophthalmic Technician s duties include preparing examination and treatment rooms, reviewing the appointment schedule, performing basic and routine vision screening examinations, administering eye medications, cleaning and maintaining ophthalmic instruments and assisting physicians during minor surgical and laser procedures. Minimum high school diploma, or equivalent and completion of an Ophthalmic Technician training or certification program. N/A Must possess at least one (1) of the following: Certified Ophthalmic Assistant (COA); Certified Ophthalmic Technician (COT); or Certified Ophthalmic Medical Technician (COMT). Not : Counselor - Social Work (Graduate Degree) The licensed clinical social worker provides comprehensive services through a broad range of individual, family, programs and services to sustain, restore or enhance the social well- being and functioning of individuals, families, and communities. d clinical social workers are members of the facility s healthcare team, frequently working in social work service, outpatient mental health clinics, family advocacy programs; and substance abuse treatment services; mental health services, combat stress detachments and correctional facilities. Current, unrestricted nurse practitioner license in one of the 50 U.S. States. Must possess a current certification in the specialty practiced as a nurse practitioner by the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP). Not Current, Valid DEA number : LP/VN (d Practical/Vocational Nurse) Participates in the implementation and evaluation of patient care. Familiar with standard concepts, practices, and procedures within a 11

The HCW is responsible for complying with all applicable State licensing regulations. Additionally, HCW s licensed outside of the 50 States must provide proof of having successfully passed the d Practical Nurse National Council Licensure Exam (LPNNCLEX). Not required.
