On-Call Rate(s): Physicians ( through : 1.0 x base hourly rate for a 24 hour period. Allied Healthcare staffing ( through ) $6.00 per hour. On-call is defined as the provider on- call is to carry a cell phone and be able to provide phone consultation. The provider must be within 30 minute/30 mile radius of the assigned. Call Back Rate(s): If the provider is called back to the, the regular hourly rate applies. This rate begins once provider is physically at the and begins work. Minimum of one hour will be paid regardless if worked less. Physician, normal hourly rate, 1 hour minimum. For Allied Healthcare staffing only ( through ): If the provider falls into overtime status, then overtime will be paid. Workweek: Will accept Government s work week of Sunday through Saturday. Background Investigations: Prices for more extensive FBI background investigations can be negotiated at the task order level. 5
5 Minimum Qualifications of Professionals: Physicians - 001, 003, 005, 006, 017, 020, 021 Board Eligible/Certified in their individual specialty. Completed an Undergraduate Program, Medical School (MD or DO) Completed Internship/Residency Program At least one year of experience in specialty Maintained appropriate unrestricted licensure Psychologist PhD Clinic Psychologist Current unrestricted State License National Certification in Specialty Applicable certificates/training according to requirement for specialties (i.e. BLS. ACLS. PALS, etc) Nurse - 025, 038 Graduated from an accredited RN/LPN Nursing School, respectively Current unrestricted RN/LPN license At least one year of experience in specialty Pharmacist A standard RPh or PharmD degree from an accredited school of pharmacy Completion of a Pharmacy Practice Residency or a Pharmacy Practice Fellowship Current unrestricted State License Physician Assistants Bachelor of Science Degree Completion of an accredited Physician Assistant Program National Certification Current unrestricted PA license Audiologist/Speech Pathologist
