Goodman President and CEO Saint Luke s Foundation (216) agoodman@saintlukesfoundation.org SAINT LUKE S FOUNDATION ANNOUNCES THIRD CYCLE GRANTS FOR 2015 Grant Payments Intended to Promote Health and Well-Being in Greater Cleveland CLEVELAND The board of trustees of Saint Luke s Foundation has approved its third cycle grants for 2015, awarding a total of $4,242,429 to 27 organizations that advance the Foundation s strategic priorities of Healthy People, A Strong Neighborhood and Resilient Families. These third cycle grants demonstrate how our mission of improving and transforming the health and well-being of individuals, families and communities is being pursued each day through the work of our grantee partners, said Anne C. Goodman, Saint Luke s Foundation President and CEO. Our grantees bring this mission to life; their spirit of innovation and their dedication to our community really is creating lasting change. This grant cycle is the third of the Foundation s three annual grant cycles. The full list of third cycle grants is noted below. HEALTHY PEOPLE Asian Services in Action (ASIA): $100,000 over two years to strengthen the organization s health home model of care provided through its International Community Health Center. Founded in 1995, ASIA empowers and advocates for Asian Americans and Pacific Islanders and provides linguistically and culturally relevant wrap-around

2 PAGE TWO programs and services. It is the only pan-asian social services agency of its kind in Northeast Ohio, and the largest pan-asian serving organization in the state. Cuyahoga Health Access Partnership (CHAP): $30,000 for implementation work related to governance transition. CHAP is a joint public-private partnership focused on access to care for the uninsured population of Cuyahoga County and founded on the principle of shared responsibility. The collaboration coalesced in 2008 and began enrolling members in FrontLine Service: $244,289 for primary and behavioral health integration. FrontLine Service provides mental health and supportive services for nearly 30,000 adults and children annually as the provider of choice for many of the most vulnerable members of our community the homeless, those severally mentally disabled, those in psychiatric crisis, and adults and children in trauma. Health Policy Institute of Ohio (HPIO): $300,000 over two years to support the Institute s ongoing efforts to advance the health of all Ohioans through informed policy decisions. HPIO serves as Ohio s nonpartisan, independent source for forecasting health trends, analyzing key health issues, and communicating current research to policymakers, state agencies and other decision-makers. It provides independent information to create informed health policy. Neighborhood Family Practice: $350,000 over two years for operating support, including support to develop a new strategic plan. Neighborhood Family Practice is a federally qualified community health center dedicated to partnering with the community to serve the family system and address public health issues. Planned Parenthood of Greater Ohio (PPGOH): $45,000 for a prenatal linkage pilot program. PPGOH provides Ohioans with safe, accessible, and trusted reproductive health services, regardless of ability to pay.

3 PAGE THREE St. Vincent Charity Medical Center: $57,000 to fund a psychiatric emergency department intervention specialist. For 150 years, St. Vincent Charity Medical Center has addressed the complex health challenges facing Cleveland s urban, largely lowincome, population. Cuyahoga County Board of Health: $200,000 over two years to support the Safe Routes to School coalition, which is charged with implementing the Safe Routes to School program. The program will enable kindergarten through eighth-grade CMSD students to engage in active transportation such as walking or bicycling as they travel to and from school. It will help improve walking and bicycling conditions for students while promoting and encouraging active transportation to school. Additionally, funds will support the infrastructure of the Health Improvement Partnership Cuyahoga (HIP-C), a collaborative effort with the City of Cleveland and the City of Shaker Heights Public Health Departments that produced the County s first ever community-wide health improvement plan. Lutheran Metropolitan Ministry: $68,292 over two years to support the My Life, My Future obesity prevention program. LMM serves individuals who are homeless, unemployed, and involved in the criminal justice system, and assists these individuals with behavioral health, guardianship, employment, and life-skill needs. A STRONG NEIGHBORHOOD Cleveland Botanical Garden: $165,000 over two years to support the organization s Green Corps and Vacant to Vibrant programs. The mission of the Cleveland Botanical Garden is to spark a passion for plants and cultivate an understanding of their vital relationship to people and the environment. Its two community based programs, Green Corps and Vacant to Vibrant, directly serve Cleveland s neighborhoods through urban agriculture and land revitalization efforts.

4 PAGE FOUR Greater Cleveland Habitat for Humanity: $50,000 for 100 Homes for Clevelanders, a project that includes fully rehabilitating ten vacant homes; a neighborhood-wide spruce up for 30 homes; and repairs to five homes owned by seniors in the Buckeye neighborhood. Greater Cleveland Habitat for Humanity works with the faith community to eliminate substandard housing. During the past 27 years, the organization and its partners have helped more than 200 families find a safe and decent place to live. It also provides homeowners with training in home construction/rehabilitation, basic home maintenance and finances, and budgeting. Kent State University Foundation: $95,000 to support Making Our Own Space (MOOS) Youth Build Workshop, an initiative of the Cleveland Urban Design Collaborative (CUDC), which is the outreach division of the College of Architecture and Environmental Design at Kent State University. This initiative will engage young residents of target neighborhoods in community design planning processes in an effort to empower them to positively change the places in which they live, work and play. Mt. Pleasant NOW Development Corporation: $150,000 to support an effort to renovate vacant homes by employing and training residents to perform light construction. Western Reserve Land Conservancy: $699,148 over two years to support the Thriving Communities Institute Saint Luke s Neighborhood Revitalization project, phase II. Specifically, funds will be used to help TCI work within the Foundation s target neighborhoods to create demolition, rehabilitation and greening target plans in partnership with residents, local government agencies, community development corporations and other key stakeholders. East End Neighborhood House (EENH): $300,000 over two years to support the East End Anchoring Collective Impact for Strong Communities project. The project is a capacity building effort focused on diversifying funding, optimizing marketing, and

5 PAGE FIVE improving operations. It also includes planning work for Ubuntu Park, a green space on East End s campus. EENH is a 108-year old settlement house serving primarily the Buckeye-Woodland Hills, Fairfax and Kinsman neighborhoods with an array of services designed to meet the needs of residents cradle to grave. EDWINS Leadership & Restaurant Institute: $125,000 for operating support, as well as capital/operating support for the EDWINS Life Skills Campus, which will provide students with safe housing, a life skills center, a fitness center, and a test kitchen. EDWINS, an acronym for education wins, is a social enterprise with the bold mission to develop a new generation of leaders in the culinary industry by training returning exoffenders for careers in the fine dining restaurant industry. Fairhill Partners: $150,000 over two years to support existing programs and services, as well as help to convene partners around crime and safety issues, and conduct a strategic planning process.. Fairhill Partners provides lifelong learning, intergenerational relations initiatives and aging programs with an emphasis on volunteerism and health. Direct service programs include kinship care, housing for older adults, disease selfmanagement, benefits access, and healthy eating and active living programming. Hebrew Free Loan Association of Northeast Ohio (HFLA): $110,000 over two years to establish the Mount Pleasant Strong Neighborhoods Pilot Loan Fund. Funds will be used for staff cost, a loan pool fund, and a reserve fund to underwrite or bolster a weak cosigner. Over a 30-year period, HFLA predicts that this investment will result in $100,000 in GED loans, and $900,000 in home repair loans. Founded in 1904, HFLA was established to help newly arrived immigrants who could not secure bank loans to attend trade schools and or to help with emergency expenses. Non-sectarian since its inception, HFLA has supported Cleveland through demographic changes and shifting societal standards. PolicyBridge: $50,000 for operating support to cover staff costs and program expenses. PolicyBridge is a nonprofit organization that has provided public policy research, analysis and evaluation as a public policy "think tank" since its creation in 2005.

6 PAGE SIX RESILIENT FAMILIES FrontLine Service: $150,000 to support the Police Assisted Referral (PAR) project, which engages Cleveland Metropolitan Housing Authority (CMHA) police officers as first social responders, linking residents to needed services and resources. FrontLine Service is the lead agency on this project, working collaboratively with Beech Brook, the Begun Center for Violence Prevention at Case Western Reserve University, the CMHA police force, and the Partnership for a Safer Cleveland. University Settlement: $81,200 to support the Strengthening Families program. University Settlement is a nearly 90-year-old settlement house in the Slavic Village neighborhood that provides a range of services to individuals cradle to grave. Under the umbrella of the Strengthening Families Program, University Settlement offers the Fatherhood Connection program, parent empowerment workshops tailored to meet the needs and interests of families, as well as case management to address the many challenges and barriers that can get in the way of parents achieving their goals for their families. Esperanza, Incorporated: $337,500 over two years to support the Family Engagement in Education program, which offers a variety of supports for the education and development of all family members. The mission of Esperanza is to improve academic achievement of Hispanics in Greater Cleveland by supporting students to graduate high school and promoting post-secondary educational attainment. Esperanza has recognized that engaging the parents of the students they support is critical to their ability to achieve their mission. Ohio IAF (Greater Cleveland Congregations): $50,000 to support the Listen. Act. Win. program. GCC is a coalition of 40 religious congregations and community organizations working together to make the Greater Cleveland region a more just and prosperous place. Listen. Act. Win. refers to the steps in GCC s parenting organizing efforts in which officials listen to the concerns and interests of parents, work with them to develop and then pursue a policy agenda, and then celebrate policy wins.

7 PAGE SEVEN Passages Connecting Fathers and Sons, Incorporated: $109,000 to support the Jobs for Dads and Moms program. The organization is dedicated to engaging fathers in the lives of their children, both with respect to their roles as financial providers and also their roles as nurturers and role models. The Jobs for Dads and Moms program combines employment and life skills training with parent education and case management to ensure linkage to important external services in order to help parents and families meet their goals. West Side Community House (WSCH): $76,000 to support its Wrap 4 Success program, which builds upon the eight-week, evidence-based Adults and Children Together (ACT) and Families and Schools Together (FAST) parenting programs by offering additional opportunities to engage and build relationships with other parents and staff, as well as individualized supports and small group sessions that address interests or circumstances which participants have in common. WSCH provides an array of services for children, families and seniors. It is particularly engaged with families through its role as the lead for the Family-to-Family Collaborative and also the Tapestry program. In addition to grants targeting the Foundation s specific priorities, the board awarded the following grant under its Strengthening the Nonprofit/Philanthropic Sector grantmaking category: Business Volunteers Unlimited: $100,000 for capacity building services for nonprofits. BVU promotes business volunteerism and foster excellence in nonprofit organizations through board matching, nonprofit consulting and seminars, connecting individuals to volunteer opportunities and a resource center. Philanthropy Ohio: $50,000 for operating support. Philanthropy Ohio is a network of 221 foundations, giving programs and individual philanthropists across the state of Ohio.
