ehealth ELSA: The perspective of the VPH research community Bernard de Bono, Peter Coveney, Alejandro Frangi, Martin Hofmann-Apitius, Rod Hose, Marco Viceconti VPH Network of Excellence ELSA Meeting Brussels, September 29th, 2009
EU ehealth R&D scene The ehealth R&D actors Virtual Physiological Human - VPH Personal Health Systems - PHS Biomedical Informatics - BMI The goals: PPI Personalised Predictive Integrative 2
We share the same goals Personalised Predictive Integrative PHS Wearable/ implantable health devices Predictive instrumentation Health Technology Cloud BMI Personal data Semantic exploratory analysis Global EHR BMI / VPH EHR VPH Subject-specific modelling Predictive models (mechanistic, probabilistic) Integrative models (multiscale, multisystem) 3
Domain There is a set of challenges that is common to many different sub-domains of biomedical research and practice These challenges can only be faced with adequate Information and Communication Technology (ICT) This domain is described by the generic term ehealth R&D. 4
R&D targets (proposed) Maximise yield of biomedical research expenditure Personalise healthcare for individuals and groups (women, children, etc.) Improve the reliability, repeatability, and the timeliness of medical decisions Develop clinically deployable integrative approaches to fight age and life style related (chronic) diseases Integrate digital health information at a global scale Resolve the individual-society conflict around the privacy of health data 5
Sources Some sources used to identify the target needs VPH Research roadmap 2006 The benefits from translating biomedical research into the health care system Report to Bio21 Australia 2007 Report of the Research Simulations, UKCRC R&D Advisory Group to Connecting for Health, June 2007 Personalized health care: opportunities, pathways, resources US DHHS 2007 Pharma 2020: Virtual R&D, PWC 2008 Innovative Medicine Initiative call 2009: Predictivity of Efficacy Evaluation, and Knowledge Management VPH NoE Strategic roadmap (draft)
Is the need quantifiable? Information is fragmented over dozens of reports produced by different communities of practice A specific, coordinated effort is required to elaborate into a single coherent framework a detailed and quantifiable description of societal and clinical needs which can be approached by integrative ehealth R&D Recommendation: CSA (Coordination & Support Action) on Integrative ehealth 2020 toward a unified scenario 7
Research objectives (bio) Complementing ELSA, VPH technology can bridge several key gaps in research to boost biomedical advances VPH for GWAS Genotype-phenotype association models in genome-wide association studies (GWAS) Virtual Physiological Mouse Comparative integrative models of all animal models (mouse, rat, rabbit, sheep, etc.) pathophysiology to predict inter-species differences, strengthening human relevance of animal models V-Tissues (Virtual Tissues) Modelling cells-tissue interaction connects: fundamental research and clinical research human pathogenesis and cell reaction to contaminants phenomenological and mechanist modelling 8
Specific RTD Objectives: PPI Personalised medicine New generation of medical technologies that integrate the data available on the patient to support a more personalised diagnosis, prognosis, treatment planning, and monitoring. Fight drug resistance in infectious diseases, neurodegenerative diseases and cancer Develop new drugs, medical devices, assistive and diagnostic technologies that are optimised for specific groups of patients (age, gender, co-morbidity, etc.) Biomarker profiling to optimize treatment in chronic diseases Cardio-vascular implantable devices for newborns (rare) 9
Specific RTD Objectives: PPI Predictive medicine Drastically reduce the costs and the risks of new drugs development using predictive models to integrate between biology and physiology Predicting effect of new compounds on the growth of gliomas New generation of medical technologies based on predictive models that support diagnosis, prognosis, treatment planning, and monitoring. Planning selective ablation in cardiac arrhythmias Define the diagnostic workflow not on pre-defined general protocols but on the prediction of risk obtained by models that combine both population and patient-specific information Progressive protocol for the prognosis of OP fractures 10
Specific RTD Objectives: PPI Integrative medicine patient data stored in different hospitals in different member states or in clinical research databases the information related to various elements and processes of pathophysiology into a systemic understanding the knowledge digitally captured into metadata, ontologies, and models in order to fight the combinatory explosion of cognitive complexity the wisdom produced in research laboratories and in clinical practice, so that it can be formalised in guidelines, standards, and protocols. 11
An exemplary goal for integrative ehealth R&D The DIGITAL ME a tool created for PPI and used for health research, patient care, patient education and coaching, etc. a coherent digital representation of the individual used as an integration framework for: unifying of fundamental and translational Integrative Biomedical Research; provision to European Citizens of affordable Personalised, Predictive, and Integrative (PPI) Medicine. 12
Core ICT services for integrative ehealth R&D Interoperability of on-line communities ID management Global indexing, search, of shared resources Biomedical data cloud Application of the concept of data cloud to sensitive data Data translation services for format interoperability Semantic data mediation to integrate incompatible IT systems QA: European institute for quality in biomedical data sharing? Integrative modelling services Mark-up languages for models description, sharing, and reuse Integrate heterogeneous predictive models running everywhere Web 4.0: Web of models 13
Next steps : Establish a collective identity VPH Institute : definition and quantification of needs Road-mapping CSA on VPH FET (call 5) Road-mapping CSA in integrative health research Road-mapping CSA in health e-infrastructures : disseminate and structure Road-mapping CSA on integrative ehealth aimed to sustain the dissemination toward all stakeholders and support the EC in the preparation of the ELSA Integrative ehealth
