Exploratory Brief on Nanomedicine or the Application of Nanotechnology in Human Healthcare Paula Corabian, Dagmara Chojecki March 6, 2012
Objectives Provincial Health Technology Assessment (HTA) Program Introduction Project by the Institute of Health Economics (IHE) Methodological approach Findings
Provincial HTA Program HTA program at the Institute of Health Economics (IHE) Services funded by Alberta s Ministry of Health and Wellness Support decision making in health policy and practice
Nanotechnology Introduction the application of science, engineering, and technology at nanoscale involves manufacturing, manipulation, and control of matter an enabling technology with many medical application areas still in early stages of development Nanomedicine the application of nanotechnology in medical science and human healthcare exploits new properties of nanomaterials and nanostructures has potential to enable early detection and prevention of diseases, and improve diagnostics, treatment, and follow up still in a formative phase Both fields are vast and rapidly growing A vast literature exists in both fields
Great interest in Alberta IHE project Nanomedicine and its anticipated future for healthcare Alberta Health Services commissioned IHE project Exploratory brief of nanomedicine What opportunities does nanotechnology offer in the early detection and intervention of diseases? What opportunities does nanotechnology offer in the areas of diagnostic and therapeutic services over the next five to ten years? Two parts Part I: an inventory of published horizon scanning, foresight, forecast, and/or roadmapping studies Part II: an inventory of resources
Part I: Methodological approach Limited literature search of horizon scanning databases Euroscan, CADTH, AHRQ, ANZHSN, National Horizon Scanning Centre National Health Service (NHS), and Sigma Scan. Internet searches, searches of blibliographies of relevant studies Included horizon scanning, foresight, forecast, and/or roadmapping studies published in English between January 2006 and November 2011 reported on nanomedicine opportunities and projected timelines to clinical use described study design and methods used Excluded studies not meeting above mentioned inclusion criteria primary research studies, reviews, overview articles animal and laboratory studies commentaries, position papers, editorials, and letters Overview of literature search findings Limitations
Part I Findings: available studies Literature search results Thirty documents published since 2000 (North America, Europe, Australia) Seven documents selected Horizon scanning, foresight, forecast, roadmapping studies Most published in 2006 and 2007 Various objectives and methods Four reports summarize workshop proceedings and materials Three reports summarize research published up to 2006 Ontario Health Technology Advisory Committee (2006) RAND Corporation (2006) Adelaide Health Technology Assessment Program (2007)
Part I Findings: Nanomedicine applications Opportunities to improve healthcare in all phases Main domains Diagnostics In vitro diagnostics and in vivo imaging Therapeutics Drug and gene therapy delivery and drug discovery and development Regenerative medicine Tissue engineering and repair Conditions High prevalence Reduce patient s quality of life Impose high socio economic burden on society Cancer, cardiovascular, neurodegenerative, inflammatory, infectious diseases
Part I Findings (cont d): Current status of research in nanomedicine Substantial developments since 2000 first and second generation nanotechnologies liposomes, albumin bound nanoparticles, iron oxide particles for MRI, silver nanoparticles quantum dots, nanoshells, and multifunctional nanodevices (theranostics) nano therapeutics and nano diagnostics used for patient care nanotechnology enabled structures and nanocomposite materials manipulation and study of cellular systems in vitro diagnostics in vivo imaging agents surgical methods, implants, prosthetics, tissue engineering, tissue repair over 50 cancer targeting drugs in clinical trial
Part I Findings: A look forward By 2020 nanomedicine is expected to revolutionize healthcare practice create new diagnostic and therapeutic applications lead to advances in molecular and cell biology make biomedicine more predictive, preventive, personalized, and regenerative Within the next 5 to 10 years cancer early detection, diagnosis, treatment, and follow up drug development, drug /protein/gene delivery, molecular diagnostics and imaging, theranostics, nanobiosystems, biomarker patterning and discovery early detection, diagnosis, treatment, and follow up of other chronic conditions regenerative medicine, surgical procedures improvement of current and future in vitro diagnostics and in vivo imaging Most promising applications multiplexed diagnostics, drug delivery and development, and targeted therapy for cancer, cardiovascular and infectious diseases
