3 Executive Summary This report records the Health and Safety activities of Waterfront Auckland for the months of February and March The key activities for the period include; Incidents There were 9 incidents reported in the 2 month period. A breakdown of the incidents is included in the attachments to this report. Incident data for public spaces has been included in this report. Data is collected from the Kiosk and security reports and reporting from Waterfront Auckland staff and contractors. The contractors are being encouraged to report incidents with a phone text protocol being used to report any serious or potentially serious incidents. Construction Projects Queens Wharf Shed 10 - The replacement contractor (Macrennie) has been inducted and their safety plan reviewed. Site visits have recommenced and a safety audit was completed at the end of March. The audit included an inspection of site activity and documentation. There were some issues with the management of subcontractor safety plans and the contractor has been advised to make improvements. The level of safety being achieved on site was acceptable. An audit closeout will be completed in the next week. Daldy and Halsey Street Works - Two audits have been completed and regular unprogrammed safety inspections are being completed on a weekly basis. Hawkins is maintaining a good level of documentation and the standard of safety on site has been acceptable. There have been some issues around the site tidiness on Halsey Street, which have now been addressed by the contractor. There has also been a focus on the public interface on the Halsey Street works and on the high risk deep excavation works in Daldy Street. Super Yacht Refit Facility - Site visits have been scheduled for April. Safety Planning for Public Events There continues to be liaison with the event staff to provide guidance with hazards associated with the current events. Events reviewed included: Silo Markets and Theatre, Beer Festival and Triathlon. Drug and Alcohol Testing Both Macrennie and Hawkins have completed random drug and alcohol testing on their sites. There were no non-negative tests recorded across the 26 workers tested. The testing is part of Waterfront Auckland s desire to see a proactive approach and ensure the sites are kept alcohol and drug free. 3

The policy was updated to reflect the changes to the Waterfront Auckland and Auckland Council structure. The policy will need to be reviewed in terms of the Auckland Council overarching policy and its compatibility/compliance with this. Waterfront Auckland is now participating in a Council initiative to review the policies that are to be implemented across the Council organisation and to ensure that Waterfront Auckland policy is consistent with the Auckland Council requirements. The PSP will be updated and reissued in April The PSP will need to be updated to reflect the requirements of the Auckland Council ACC programme and changes to best practice. Stakeholder Induction Checklist /Agreement Current Revision Status Comments Activity Form 5.1/Rev3/January2011 Issued for implementation The Stakeholder Induction Checklist/Agreement, Form 5.1, is included in the PSP. There are some changes to the format and content of the induction and this has been included in PSP Rev4, and is currently being used for the project inductions. An induction was completed for the new contractor on the Shed 10 project. Communications Activity Contract Meeting #38 completed 2nd April Liaison with Waterfront Auckland Events staff on process for monitoring risks associated with events. Meetings (fortnightly) to review risks associated with events for Wynyard Quarter, Queen s Wharf, and Westhaven Marina. Liaison with event staff to review safety plans. Liaison with Auckland Council with respect to event planning Liaison with Marina staff. Shed 10 site visits to view health and safety, induct contractor, review incidents and audit site. WA staff safety meeting to communicate Council and Waterfront safety processes. Daldy+Halsey project visits to review incidents, general safety inspections and safety audits. Review safety 5

6 aspects of methodologies. Participation in site meetings on the construction projects. Review security and kiosk reports. The audit tool to be used on the new construction projects (Shed 10 initially) has been updated to reflect changes to construction best practice. This includes changes to the Best Practice Guidelines for working at height. A section on wharf safety is also being included. A template plan has been developed for small events and activations (this has now been adopted by Auckland Council for all their small events and activations). This records basic information concerning health and safety aspects of a proposed event and provides event organisers hazard and emergency evacuation information associated with the Wynyard Quarter and other Waterfront Auckland Sites. 6

Training Overview & Management Activity NA Incident Management & Reporting Activity Incident summary included in attachments to this report Additional Services Auckland Waterfront - Marinas The H&S consultant continues to liaise with the Waterfront Auckland marina organisations (Westhaven, the Landing in Okahu Bay and the Viaduct Basin) in terms of health and safety. Safety Planning for Public Events There continues to be liaison with the event staff to provide guidance with hazards associated with the current events. A process for the review of risks associated with events is in place. This includes a regular meeting between event staff, operational staff, and the Health and Safety Consultant to complete a high level review of events to identify higher risk activities that may require a more detailed review. An event register has been established which records upcoming events and planned safety activities associated with that event, including safety plan reviews and audits of event setup. A template plan has been developed for small events and activations. This records basic information concerning health and safety aspects of a proposed event and provides event organisers hazard and emergency evacuation information associated with the Wynyard Quarter. We have established a liaison process with the Auckland Council Safety team to ensure that the review of event safety plans and site setup is efficient as possible. There are some issues around the monitoring of the events to ensure compliance with the approved safety plan. This is the responsibility of the permit issuing organisation (Auckland Council). This has been discussed with Auckland Council in the most recent liaison meeting. Agreed that Waterfront Auckland will raise any issues as they arise. 7

8 Variations Variation #1 completed. Variation #2 Te Wero Bridge and Halsey Street Wharf Health and Safety completed. Variation #3- IBP Report Implementation The recommendations from the IBP report are now being implemented as described below. The PSP will be updated to reflect the recommendations where appropriate. 1. Continue with current initiatives continue as per current PSP. The recommendations from the IBP report are included in the latest revision of the PSP (Rev4/March2012). 2. Cooperative Research Centre (CRC) Alignment no further action 3. Site Safe Accreditation In place. This is being extended as new stakeholders commence work. 4. Construction Client Group Waterfront Auckland are no longer participating as per the recommendation. 5. Auckland Council Liaison PA has been attending Auckland Council meetings from time to time, as time allows. Liaison, reporting and meeting will be minimised. A Waterfront Safety Liaison Group has been established with the Harbour Masters office and Auckland Council. The group meets on a bimonthly basis and reviews safety activities in and around the harbour. 6. Government Interaction Informal and irregular liaison & meetings with the DoL is the norm now. 7. Software/Computer Based Solutions a final report has been produced recommending that a software/computer based solution is not appropriate for the project at this stage. Waterfront Auckland is able to use the Auckland Council Vault software and this is currently being used. Variation #4- Investigation into using the Red Shed as a venue complete Variation #5- Increase to PI Insurance ongoing, additional cost of 2011/12 insurance has been processed. Optimisation and Value Activity Review of PSP - A review of the PSP is currently in progress and will be completed in April with the view to reissuing in April
