4 Brief History of Ashadeep To remove a vacuum in the area of Psychiatric Rehabilitation in the State of Assam, Ashadeep was formed by a few Family care-givers in the year At that point of time, access to psychiatric treatment was available in one third of the total districts of the State. The situation has improved marginally, with just over half the total 29 districts having access to Psychiatric treatment facilities. Ashadeep s journey in this area from the year 1996, can be broadly divided into two major phases, the first being from inception to the year 2005, second is from year 2005 to year FIRST PHASE ( ) During the initial years of the first phase, Ashadeep s rehabilitation service facilities, both in Day Care and residential settings was limited to the city of Guwahati only. The Day Rehabilitation centre at Guwahati was developed to address the needs of persons disabled through mental illness and also mental retardation and associated developmental disorders. Vocational training with an emphasis on income generating activities was a major component of the rehabilitation process at this centre. Residential rehabilitation was provided through Roshmi, the Half Way Home, which was created specifically to address the needs of women with mental illness. From the year 2000, venturing out to adjoining districts, where Psychiatric Care was not available, Ashadeep initiated monthly mental health camps in partnership with local groups. These outreach programmes led to the establishment of two Day Rehabilitation Centres at the outreach locations Khetri and Morigaon. At the end of the first phase, in the context of the overall mental health scenario of Assam, it could be said that Ashadeep had managed to introduce specific concepts of Psycho-social rehabilitation in the State. Being the only agency involved in this specific activity, the rehabilitation services could touch only those in and around Guwahati. The needs of the marginalized sections of the population, both economically and geographically could hardly be addressed. SECOND PHASE ( ) Primary Psychiatric treatment is obviously a prerequisite to the secondary level psychological and social rehabilitation interventions. Even in the year 2005, access to Psychiatric care was available in less than half the districts of the Assam. Further, even in the city of Guwahati, the poorer sections found it very difficult to access Psychiatric treatment, and there was no system both in the Governmental or Nongovernmental sectors to help the homeless mentally ill. Guwahati, being the major terminus for most rail and road traffic of the region, had a substantial population of wandering mentally ill people. In this context, it had become necessary for Ashadeep to take a more holistic view and provide a more comprehensive set of interventions starting from the primary Psychiatric treatment to the Psycho social rehabilitation needs of the two marginalized groups of people mentioned above. The Rehabilitation programme for homeless women with mental illnesses of the streets of Guwahati was initiated in 2005 with support from Sir Ratan Tata Trust, Mumbai. The prime objective of this project Navachetana was to intervene with the women in an appropriate 2

5 manner and re-integrate them with their families. Psychiatric and other medical treatment and care; Psychological and other social rehabilitation services were provided at the three Navachetana residential facilities by a team of qualified personnel. In the last seven years 395 homeless women with mental illnesses have been admitted to our facilities, out of which 350 women have been re-integrated with their families. Further, it has been possible to provide follow-up support in terms of treatment, counseling and free medicine supply to 51% of the re-integrated women. A weekly Outdoor Clinic located at Guwahati has been very useful to maintain continuation of treatment of reintegrated residents of Navachetana. Income/livelihood generation is a major outcome of the rehabilitation processes of the different groups of people. A permanent sale outlet Ability the Ashadeep shop was set up at Guwahati in 2011 with support from Give India. All articles produced at our Sheltered Workshop and other rehabilitation settings are sold through this outlet. To create access to Psychiatric interventions in districts where such services were not available, an Outreach Programme was initiated from the year In partnership with a local NGO or Government PHC/CHC, regular monthly mental health camps were held at 9 locations, each for a period of 12 to 18 months. The partner NGOs were provided with the requisite training and support to continue with the work after our departure. So far 130 monthly Mental Health Camps have been held in 9 locations (12-18 camps in each location) covering 1863 patients through 6822 interventions. 5 NGOs and 2 Govt. health centres have been empowered to provide Mental Health interventions in the Outreach locations. Our interventions had been able to touch the user groups and immediate stakeholders like family caregiver groups. However, in order to sensitize the community at large and cater to some of the social issues which affect mental well being awareness programmes and trainings/workshops were conducted at various levels such as police stations, schools, colleges, etc. In addition, a telephonic helpline named Paritranam was initiated through which counseling is being provided for almost every aspect of mental health. The Resource Centre played an active role in terms of advocacy with the Government and gradually Ashadeep has become more and more involved with the Government in terms of planning of policy and action plan for the well being of homeless persons with mental illness. A concrete outcome of this interaction has been the sanction of a grant of Rupees One Crore by the Social Welfare department of the Assam Government for setting up of a Rehabilitation Home for Homeless men with mental illnesses at the P.L. Home campus of the Government at Bamunigaon, Boko. 3

6 From the Secretary s Desk The year was a significant one for Ashadeep with certain developments taking place which will have some major future bearings. Recently a survey was conducted by Ashadeep to ascertain the details of homeless persons with mental illnesses wandering on the streets of Guwahati. This study conducted under directives from the Social Welfare department of the Assam Government revealed that at that point of time there were 101 such individuals wandering on the streets which consisted of 86 men and 15 women. A finding, which underlines the positive outcome of the efforts of Ashadeep in rehabilitating homeless women with mental illnesses of the city, in the last few years. A fact, which has perhaps, not gone un-noticed by the Government Authorities. The Social Welfare department of Assam has taken the initiative in providing the infrastructure and the funds required to replicate our efforts with women through Navachetana, to intervene with homeless men in similar condition. This acknowledgement of the Government for the Ashadeep model of rehabilitation of homeless persons with mental illness also vindicates our belief that advocacy through work instead of mere campaigning can produce far greater results. All our achievements have been made possible by the dedication and hard work put forth by each staff member and encouraging involvement of the members of Ashadeep s executive committee. I also extend thanks and gratitude to all our donors and well wishers who relentlessly supported us in our endeavor. 9 Programme and Activity Information From being a mere service provider to taking up an all encompassing role in the field of mental health Ashadeep as an organization has come a long way and so has its programmes and activities. Today, the following groups of persons with mental health needs are being catered to. A. B. C. D. E. Children and Adults with intellectual disabilities Persons recovering from mental illnesses, living with families. Homeless women with mental illness. Marginalized sections of population, geographically or otherwise who do not have access to Psychiatric interventions. People of all age groups in need of emotional support through Tele Counseling. A. Children and Adults with intellectual disabilities who are provided with special education at our Day Rehabilitation Centre have excelled in various fields during the year. Sports - Students of Ashadeep participated in a National Coaching Camp on Football & Bocce organized by Special Olympics Bharat, Assam at BokaKhat from 24 th to 30 th April Seven Special Athletes took part in a National Coaching Camp on Athletics held at Jamshedpur from 7 13 th Nov, 2011 th Four Special Athletes took part in a National Coaching Camp on Basketball held at Shillong from 9 14 th Dec, One Special Athlete took part in National Games held at Chennai in the discipline of Badminton and won Gold medal in singles and Silver medal in doubles from 17 th 23 rd Feb,2012. th Cultural activities Students of Ashadeep participated in a Dance programme at Rabindra Bhavan organized by Dept. of Cultural Affairs, Govt. of Assam on 20 th May, Bihu Dance performed by the students of Ashadeep at Rabindra Bhawan organized by Karunadhara on 29 th Apr, rd On 3 December 2011, on the occasion of Disability Day Students of Ashadeep took part in An Exhibition at Bishnu Nirmala Bhawan organized by Karunadhara. An Art Competition organized by Social Welfare Dept. at Kalakshetra. An Art Competition organized by Science Museum at Science Museum. Students of Ashadeep participated in an Art Competition on the occasion of Science Day celebrated at Science Museum on 28 th February th Students of Ashadeep participated in the 18 Anniversary of Science Museum on 16 th March The highlights of the year are - Four Special Athletes received State level Sports Scholarship from Government of Assam. One Poem and one painting done by students of Ashadeep were published in a book named Ami Aparajeyo published by Karunadhara. 7

Rehabilitation of Persons recovering from mental illnesses living with their families are intervened with at the Day Rehabilitation Centre and in residential settings at Roshmi the Half Way Home for women. Apart from counseling and other rehabilitation interventions, special emphasis is given on vocational skills development and livelihood generation opportunities in Sheltered Workshop to this target group. The articles produced such as Handloom materials, File covers, Jute items, decorative candles, Pillows and block printed clothing etc. are sold through Ability the Ashadeep Shop located at Sixth Mile, Guwahati. A part of the income from this Shop goes as incentives to those involved in the production. This intervention in addition to generating livelihood plays a major role in preventing relapses in case of the mentally ill, through a structured schedule of occupational activity. C. Rehabilitation of Homeless Women with Mental illness of the streets of Guwahati is being carried out in three Shelter homes, - Navachetana (Transit Care), Navachetana (Rehab) and Roshmi (Half way home). The women rescued from the streets are first admitted in Navachetana (Transit Care) for initial care and psychiatric interventions. When the mental status of the woman has improved and she attains a certain level of functionality, she is shifted to one of the other two homes for intensive rehabilitation inputs and vocational training. The readiness of the shifting and also discharge is gauged by Growth Chart a tool developed in-house to monitor the level of functionality. When the woman attains the final score of the Growth Chart and her home address is verified to some extent, she is discharged and re-integrated with her family. This entire process takes about 3-6 months on an average. Over 80% of the women have been reintegrated with their families. Further, it has been possible to maintain follow-up care for over 50% of the ex-residents of Navachetana. The entire rehabilitation process is depicted in the following Flow Chart. 8

14 The following Charts depict some of the achievements with regard to Homeless women with mental illness during the last year. Total Rescue / Admission = 54 No of women reintegrated in various locations Observation of the above chart clearly indicates the increased involvement of Police and Other Organizations in the rescue process which is 33 (61%) against the direct rescue by Ashadeep, i.e. 8 (15% only). This is perhaps an outcome of the awareness created in the community by Ashadeep s work in this area. It is obvious from the above diagram that a majority of the women who have been intervened with were from Assam. The map below indicates the location of the homes of the re-integrated women. 15 Follow up trend through usage of various medium CASE STUDY Technology helps in locating the Family of homeless woman Malasiya, aged about 45 years, was walking precariously on the divider of one of the busiest roads of Guwahati, totally oblivious to the fast moving traf ic on both sides of her. Though initially she was not responding to the overtures of the Rescue Team she gradually agreed to accompany them to the Navachetana Transit Care home on 2 nd June Symptoms present at that time were auditory hallucination, disorientation, wandering attitude, selfmuttering etc. Immediately her clinical interventions were started. But, perhaps because of the acute symptoms, her recovery from the positive symptoms took nearly nine months compared to the average stay of three months at the transit care facility. For the post cjlinical rehabilitation interventions, she was shifted to Roshmi (Group home) on the 30 th March During the Counselling sessions though she revealed that she was from Sitamarhi in Bihar and also talked of her husband and six children, she was not able to give speci ic details of their location and identity. But like all other residents, she was eager to return to her family, thus creating pressure on the re-integration team. Someday in February 2012, one of Ashadeep s Social Worker while browsing in Google noticed that there is an NGO named Harijaan Mahila Evam Bal Vikash Sangathan from Sitamarhi, working on Women Empowerment. After going through their website, a phone number was found and immediately a phone call was made. It was the Asstistant Director (Raghabendra Rajesh) who received the call. His response was very positive and he requested the Social Worker to Malasiya s photograph with the details. A few more communications through phone and enabled Mr. Raghabendra to locate Malasiya s family at Sitamarhi within two days. On 18/02/2012, Mr. Raghabendra made a phone call to the Social Worker who was on his way to an Outreach Mental Health Camp, and said that Malasiya s family has been found and they are sitting in front of him. After talking to the family members, it was found that they were desperately in search of her for the last two years since she had been missing. They also told that they will visit Ashadeep to take her back at the earliest. Finally on 22/02/2012, her husband and nephew came to Roshmi (Group Home). As per their information abut the onset of her illness - some years back she went to visit Baba Dham near Sultanganj (Jharkhand) with other villagers. But she got lost from the group and after 15 days the local police found her wandering on the streets and she was handed over to the family. As the days passed by, she started showing symptoms like aggressiveness & violent behavior and wandering attitude. Malasiya even assaulted her husband with a dagger. But no intervention had was sought. Again, one day she came out of home and this time they found her in Delhi after about 30 days. But after 40 days of bringing her back home, she again wandered out of the village and was missing since then. Malasiya boarded the train for her journey back to Sitamarhi from Guwahati with her husband on 22/02/

Marginalized sections of population, geographically or otherwise who do not have access to Psychiatric interventions To create access to Psychiatric Care and Rehabilitation Interventions in remote areas of the State where such services are not available, Ashadeep has been conducting monthly Mental Health Outreach Camps in collaboration with local NGOs since October Ashadeep conducts about monthly camps in one location where psychiatric interventions with medication are provided free of cost. During this period, Ashadeep also facilitates capacity building of the partner NGO, so that the psychiatric and other interventions are continued with even after Ashadeep withdraws from that location. In addition to serving the needs of the lower economic groups and mentally ill inmates of different shelter homes, this provision has become the focal point of continuing treatment of Navachetana residents who have been reunited with their families, and also of discontinued Outreach locations. 17 Distribution of patients since the year 2006 E. PARITRANAM Tele Counseling Help Line ( ) Anxiety or stress related problems can affect anyone at any age which arises from emotion. The effects can be mild or severe, creating extreme emotional and psychological problems. If these problems are left unresolved, they can affect a person s daily choices and functioning and ultimately, can manifest into serious psychological disorders with lasting effects and sometimes drastic consequences such as suicide. Just as we need help to heal the physical effects of trauma, we also need help to heal the emotional wounds. PARITRANAM helpline provides free and confidential emotional support for the depressed and suicidal. The help-line address needs of all age groups and all kinds of people with mental health issues and is attended by qualified counselors. This Help Line was inaugurated by Minister of Social Welfare Sri Akon Bora in January 2012 which is being supported by The Hans Foundation, New Delhi for a period of 1 year and the Telephone connection has been donated by Bharti Airtel Ltd., Guwahati. 169 calls have been received during January 2012 to March Trends of callers with problems are shown in the following table : Sl. 19 Dr. Anjana Goswami, Director, Resource Centre presented Ashadeep s work in a seminar titled Building Partnership for Global Mental Health Alliance for Mental Knowledge Transfer which was organized by the United Nations University UNU-IGH in Malaysia on the 21 st, 22 nd & 23 rd of November The presentation made by Dr Anjana, the only NGO representative in this forum was very well received by those present. A one day workshop on Early Identification and Early Intervention for Children with Autism in association with Com-DEALL Trust, Bangalore was held on 19 th December Advocacy with Assam Government had led to the formation of a Consultative Committee through a Government Notification to develop services and policies for meaningful rehabilitation of persons with mental illness in the state of Assam. Headed by the Commissioner Social Welfare, this group includes representatives from all relevant stake-holders both from the Government and Civil society. A direct impact of Ashadeep s work with homeless mentally ill women has been the involvement of the Social Welfare department of the Assam Government in developing similar facilities for men. AWARDS & RECOGNITION Ashadeep s work with homeless women with mental illnesses has been acknowledged by various bodies and the following awards have been presented to the organization in the last year: In February 2012, Ashadeep was jointly awarded the first prize by The EdelGive Foundation, Mumbai under its Social innovation honours under the category Health & Well Being and received a cheque of Rs. 5 lakhs, a trophy and a certificate. The other 3 categories were Education. Economic Security and Livelihoods and Rights and Representation. Winner of all the 4 categories were selected amongst over 240 NGOs. Dr Anjana Goswami was honoured by the National Women s Commission at a function held at New Delhi in March 2012 for her services towards women and children. Future Projects As touched upon in earlier sections, the Social Welfare department of the Government of Assam is providing the land and also the funding support to set up a Rehabilitation Home for homeless men with mental illness. This proposed facility will accommodate 40 men, who will be intervened with using the Navachetana model that has been successfully implemented with women in similar circumstances. The project shall be located at the P.L. Home Campus, Social Welfare Department, Assam Government, Bamunigaon, Boko. The other major initiative of Ashadeep in the coming years would be in setting up of the Ashadeep Institute of Mental Health which would include a Psychiatric Hospital in addition to the existing residential rehabilitation services. This would perhaps materialize because of an unbelievable commitment of an individual who has decided to construct an infrastructure and give it to Ashadeep on a long term lease. We expect this complex to be ready by the end of
