2 A Compendium of Innovative Practices and Partnerships: Epanding Educational Capacity in California Schools of Nursing October 00 - rd Edition Introduction Epanding educational capacity in California schools of nursing was perhaps the state s most urgent need for addressing the nurse shortage and is Goal of the Master Plan for the California Nursing Workforce. The California Institute for Nursing &Health Care (CINHC) developed the statewide strategic plan building upon successful programs already underway. With that in mind, in 00, the Compendium of Innovative Practices and Partnerships, First Edition was published and identified successful practices that merit replication throughout the state. Many ecellent innovations and programs have been created through public/private partnerships to respond to the challenges facing nursing education today. These efforts include partnerships between schools of nursing, hospitals, health systems, and foundations. The information contained in the First Edition was used to demonstrate the collaborative, statewide approach to epansion of educational capacity in California at the 008 National Nursing Education Summit. This Summit was sponsored by the Center to Champion Nursing in America, under the auspices of AARP. The Compendium, Second Edition, updated the successful practices and the partnerships that support them published in the First Edition and added new programs and partnerships initiated in 008. This Third Edition now serves as a resource in providing information on current programs and successful practices in California. To create the Compendium, deans and directors of nursing programs, and healthcare nursing service administrators shared their successful innovative approaches with us. They completed an information form describing the program, the partnerships supporting the practice, and the outcomes. This Compendium reflects the program information as submitted by the participants. It does not include all the innovative practices and partnerships in the state, but is reflective of the ecellent work that occurs through the numerous public/private partnerships underway in California that support nursing education. CINHC wishes to thank the deans, directors, nursing educators, administrators and other nurse leaders who provided this information and contributed to this work. Identify challenges or issues d tuition fees, budget constraints on medical facilities which have less funds for nursing education Identify any breakthrough concepts or key learning While students initially like the onsite instruction offered at the agency, they also appreciate the convenience of online courses once they advance in the program. Advise adaptability of practice to other settings Really need to have a strong online option so that students who stop out or can t attend the evening classes have the opportunity to continue in the program to graduate. MSN program is much more varied as the total number of credits per role option varies. Funding is provided by the agency on a semester basis. 20 Lead Institution or Organization (provide full name and place an in the appropriate California State University, Fullerton Innovative Practice (provide the name of the program or practice) Education Service Facilitating Transition to Practice --- A New Graduate Eternship/Residency Program Innovative Practice Description (provide a brief description) The current economic recession has negatively impacted hiring trends for new graduate nurses with 0% of graduate nurses remaining unemployed and unable to find a job as a Registered Nurse (RN). A statewide concern for legislators, employers, and educators is that new graduates who do not find employment, will lose the skills they acquired while studying to become an RN, thus making them unemployable in nursing. A similar concern evolved at CSUF with new RNs in the EL-MSN program who were not able to find employment after successfully completing the RN licensure eam in year of the program. In response to this concern, the CSUF faculty transitioned a paid Nursing Eternship course into an unpaid new graduate residency eternship, where students were able to maintain clinical recency in a 6-week preceptored course while continuing enrollment in graduate level coursework. All of the students completed the eternship course ecept for a student who had a baby and students (n=) who became employed during the eperience. Student and faculty satisfaction with the preceptor program. The majority of students and preceptors (96%) epressed satisfaction with the eperience. Student employability upon completion of eternship. Only students were offered employment at the hospital site where they did their preceptorship. A contributing factor to this outcome was that several hospitals planned to delay hiring until the student(s) completed all their graduate coursework (- semesters). Students who were employed after the residency course epressed increased self-confidence and an easier transition to the RN role at their place of employment
21 Cost (indicate the cost of the program or practice) The University costs of the program were faculty salary costs for a -unit course and the student costs were enrollment in a unit Nursing Eternship elective. There were no costs to the practice setting since this was an unpaid eternship. Students epressed a high level of satisfaction with the course and many reported that this course was the highlight of the semester for them. Preceptors also appreciated the opportunity to work with students over the duration of - semesters and to be a part of the new graduate transition to practice. Identify challenges or issues Challenges included identifying a sufficient number of preceptors, especially in fall and spring semesters, when hospitals were impacted by a larger number of clinical placements requests from other schools. Another challenge was that hospitals and preceptors did not understand the nature of the student since they were not used to preceptoring new graduate RNs, ecept in conjunction with hospital-based new graduate programs. Identify any breakthrough concepts or key learning The key learning from this project is that it is essential for education and service to work together creatively in meeting current and future workforce needs. The hospital partners in this project are to be commended for their willingness to think outside the bo in supporting placements for this course. Hospitals could benefit from the program if residency students were hired by the facility, since these students would already be acclimated to the facility. Advise adaptability of practice to other settings Other universities and colleges that have an eternship course could modify the course in a similar manner. The course could also be offered through Etended Education to generate revenue for the program. A review of attrition rates in the previous 6 years identified a pattern of higher than normal attrition rates for the men. A review of the literature revealed that higher attrition among men was common in health care educational programs. We developed a plan to better understand and address this phenomenon which included collaboration with a behavioral health specialist colleague, Wayne Lavengood, LCSW, to consult in developing a realistic approach to address the problem. All male students and graduates were invited to a series of meetings to obtain feedback about potential issues and barriers to success in the program for male students. We surveyed male students who had terminated the program prior to graduation. After reviewing the survey results, incorporating suggestions from current and former male students, and reviewing literature related to men in nursing success in educational programs, it was decided to schedule a monthly meeting for male students led by the social worker consultant that provided a format for both general discussion of issues but also a learning opportunity on topics of special interest i.e. issues of touch, communication differences, caring styles. Male RN s actively participated by sharing their own eperiences and suggestions. In addition, the consultant became available to meet with any male nursing student who was eperiencing difficulties that would impact his success. We also recruited a male clinical instructor to serve as a role model for male students. Students in the pre-licensure portion of the curriculum attend a clinical skills lab equipped with medium and high fidelity mannequins. 91 Kaiser Permanente Southern California Patient Care Services California Employment Development Department (EDD) Workforce Investment Act (WIA) Kaiser Permanente Fund for Health Education at the East Bay Community Foundation in Northern California Were epected outcomes met? Yes No If not met, why? Candidates for the program were pre screened to assure minimum BRN qualifications were met (Assistant Instructor level). A Memorandum of Understanding was implemented to obtain agreement of teaching commitment over a year period following completion. Three courses conducted from October 009 to May 00 for 79 nurses in the Los Angeles region. Several nurses completing the program have obtained adjunct teaching positions with some also considering full time faculty positions. As of July, nurses have taught clinical courses and provided over, hours of clinical instruction working for schools of nursing. Identify challenges or issues Course participants were eperienced nurses generally working full time so scheduling challenges needed to be considered. Communication of schedules in advance of the program, providing full day classes to support fewer scheduled days, and fleible options to self-schedule the 8 hour clinical practicum helped minimize these issues. Lead time to establish MOU agreements with each school providing the clinical practicum need to be considered. Guidance for participants to meet required prerequisites unique to each school and clinical affiliate prior to the start of their clinical practicum requires coordination and monitoring. Identify any breakthrough concepts or key learning There are several contributing factors related to the nursing faculty shortage in California. Eperience with this program found there is not a shortage of eperienced nurses with an interest and passion to teach. This program provided the resources and support needed for nurses to move forward on a career path that included teaching. The course provided nurses with an important foundation of knowledge and skills to succeed in transitioning to clinical faculty roles. Advise adaptability of practice to other settings The course can be adapted and implemented for use in other regions and the candidate selection process modified to target candidates with specific specialties to meet identified faculty shortage needs. Partnership with a nursing school to provide the program and grant course credit is advised to support the transfer of elective course credit toward the ongoing education of nurses enrolled in or considering starting a Masters program. Partnerships with nursing schools in the region to provide faculty as preceptors during their eisting clinical rotations is essential to providing the clinical practicum portion of the course. Provide time frame funded (from month/year to month/year) and plans for sustainability Jan 009 to Dec 0 with combined grant funding from three sources. The course could be offered with some or all of the registration fees paid by course participants if needed. The program can be replicated in other locations as an etended education course offering for a fee, and the materials and services provided in the program adjusted to cover costs. Other Comments Communication and networking with both service and schools in the communities served by the course is advised to assure awareness of the program, provide support for working nurses to attend, and ultimately to link schools with open faculty positions with nurses available to teach. A needs assessment survey of clinical faculty vacancies indicates an ongoing need for courses like this to be provided to both generate interest and support the preparation and role transition of new faculty.
