2 Billing Information Service is billed quarterly in advance and is due upon receipt. The first billing will be collected upon activation of your account and will reflect the amount due from your effective date of service through the current quarter. Future invoices will be mailed directly to the billing address given during setup. Please send payments to: Republic Services PO Box Louisville, KY To ensure timely and proper posting of your payment, please be sure to include your remittance advice and/or your 12-digit account number. Late fees are assessed to all accounts not paid within 30 days of your invoice date. A $35.00 service interruption fee will be assessed to all accounts 60 days outstanding. There will be a $30.00 processing fee for any returned checks for insufficient funds. As an added convenience our customers may now pay online. We Also Provide payment services by phone via credit card (Visa, MasterCard, Discover, or American Express) by calling Cart Placement Please make sure to place your cart(s) with the wheels facing towards the curb/ your home. Ensure that there is nothing blocking the service of your cart. Our drivers need six feet of open space in either direction. Please place the cart the appropriate distance away from light poles, mailboxes, fire hydrants, etc.

3 Garbage Collection Please have your 95-gallon rolling cart(s) at the curb by 6:00 a.m. on your pick-up day. All garbage and/or boxes must be inside a company provided container additional bags (other than yard waste) on or around container will not be serviced. Republic Services provides service to your neighborhood until 7:00 p.m. If a missed pick-up should occur, please call us immediately at (404) If reported more than 24 hours after the scheduled pick-up, you may have to wait until your next scheduled pick-up date. If your cart/bin/yard waste is not at the curb when the driver comes by, a $25.00 return trip fee may be assessed. A 95 gallon container is provided free of charge during the time of removal for waste removal. Where available, an additional 95 gallon container for recycling service will also be provided free of charge. Republic Services property not returned at the end of this time will result in additional charges. Holiday Collection Schedule Residential Holiday Schedule Memorial Day Independence Day (July 4) Labor Day If your scheduled collection day falls on these holidays, there will be no collection. Collection will be on the following day. For example, if your scheduled collection day is Thursday on Independence Day, collection will be on the Friday, and Friday customers will have collection on Saturday. Commercial + Residential Holiday Schedule Thanksgiving Christmas New Year's Day Residential customers will have their collection day moved to the following day. Commercial pick-ups may be modified during those weeks so as to accommodate the holidays. In all cases, customers will be given service at their contracted service levels. *These are the only Holidays that will effect collection days.

4 Recycling Collection Place your recycling bin at the curb by 6:00 a.m. on your pick-up day. Recyclable items should be placed inside your bin. Please refer to the Recycling Checklist the listing of items that can be recycled. Recycling Checklist Yes. 5 Yard Waste Collection Yard waste includes grass clippings, leaves, tree and shrubbery trimmings, branches and limbs. (No logs or dirt) Bags of yard waste must weigh less than 40 pounds each Trimmed limbs should be no more than 4 inches in diameter by 4 feet long and tied in bundles weighing less than 40 pounds Please do not combine yard waste with household garbage *Please note that Recycling and Yard Waste Collection Services are not available in all areas.
