2 2016 Doctoral Course of Graduate School of Nutrition and Bioscience, Tokushima University Application Guidelines for the 3rd Admission In the Graduate School of Nutrition and Bioscience, we make efforts to train professionals who can be active in their fields (medical service and local society or industry) and researchers for educational research institutions. We give lectures of extensive education through teaching human nutrition including basic life science and various knowledge of medical care to students and also try to do research so that they can lead their field and contribute to society. We have a goal of contributing to the nutrition field through achievements of research to train students to contribute to society. <Admission Policy of Department of Human Nutrition Science, Graduate School of Nutrition and Bioscience> We admit applicants from Japan or overseas who have sufficient and high technical knowledge base in life science and medicine and who are able to contribute to the maintenance and promotion of health. Nutrition managers who can apply their high technical knowledge to develop clinical field. Japanese or overseas public officers who have demonstrated their capacity to apply their knowledge for planning, drafting and social welfare within a nutritional institution. Japanese or overseas applicants with professional capacity and able to play an active role both in research and education in life science and nutrition. I. Number of Students to be admitted Department Strength Recruitment Human Nutrition Sciences 9 a few II. Qualifications for Applicants The following individuals are eligible to apply for admission. (1) Holders of a master or vocational degrees or those expected to complete the degree by March 2016 as prescribed by the Japanese Ministry of Education Regulation 9, Article 5. 2 of 1953 on vocational degrees 2

3 (2) Applicants who have completed or are expected to complete their master, vocational or any other equivalent degree overseas by March 2016 (3) Applicants who have taken correspondence courses from a foreign school and completed their master or vocational degrees in Japan by March 2016 (4) Applicants who have followed a relevant curriculum in a well accredited foreign institution and completed or expected to complete by March 2016 a master or vocational degree or any other equivalent course as specified by the Minister of Education, Culture, Sports, Science and Technology of Japan. (5) Those who have graduated from the United Nations University program with a degree equivalent to a master s degree, or who are expected to graduate by March 31, 2016 (6) Those who have completed a course of a foreign school, an educational institution designated in item (4) above or the United Nations University; passed an examination or a screening which corresponds to those prescribed in Article 16, paragraph 2 of the Standards for the Establishment of Graduate Schools (Ordinance of MEXT, 1974); and are qualified to have academic standard equivalent to or higher than those who hold a master s degree (7) Applicants designated by the Minister of the Education, Culture, Sports, Science and Technology based on Article 156 Paragraph No. 6 of the Ordinance for Enforcement of the School Education Act. (8) Applicants 24 years of age and older as of March 31, 2016, holding a master degree or more and accepted by our department on the merit of their scholastic accomplishments, regardless of the entrance examination.(refer to notes.) (Note 1) Application Qualification (8) 1. "Holder of a master degree or more with scholastic accomplishments" means that the applicant should meet the conditions specified in the paragraphs 1 and 3,or, 2 and 3. 1 Hold a bachelor degree and satisfy the requirements of the Chapter 104 of the School Education Law that requires the applicant to have at least a 4-year experience working in a nutrition related field. 2 Applicants who dropped out of junior college, professional high school, vocational school or any other educational institution (after studying for more than 2 years) or have graduated from one of the above-mentioned schools, and worked in a nutrition-related field for more than 6 years. 3 Author of a valuable academic paper, lecture, report or, at least, a master thesis 3

4 as the results of outstanding research. 2.Applicants who fill the requirements should submit the following documents for prior screening to Graduate School Section, Educational Affairs Division, Faculty of Medicine, Dentistry and Pharmaceutical Sciences, Tokushima University: Recent resume and graduation certificate Application for entrance examination (Nutrition School format) Report of research achievement (Nutrition School format) Reprint (s) of your paper(s) The deadline for submission of the above documents is Friday, February 5, In addition, applicants who meet the requirements of the above paragraphs 1 and 2 should submit a document signed the Head of section or President of the employing institution certifying that the candidate has more than 4 or 6 years work experience. (Note 2) Entrance Examination for Employed Students - Any applicant who fills the requirements of articles (1) to (8) - Is presently working for a hospital, school, public institution, a company or an education and research institution - Has ability for personal financial support after admission or has a permission for taking the entrance exam from the Head of a future hiring institution (Note 3) Entrance Examination for Privately-Funded Foreign Students Applicants who fill the requirements of the above paragraphs (1) to (8) and have Japanese or English proficiency. III. Term of Application and Place to Apply 1. Term of application Monday, February 29 to Thursday, March 3, :00 am to 5:00 pm *We cannot receive applications on weekends. Application Procedure 1. Documents to be submitted Application Form Ticket for Examination Your master thesis Official Transcript/Grade Report Please fill in all required information on the form provided. (with a picture taken these days at the size of 4 5cm without wearing a cap or hat.) Please attach your identification photo. The photo must be recent, and must be of your upper body, facing forward with head uncovered. The size must be 5 cm height and 4 cm width. Your master thesis and summary indicating the main results of your research (about 700 words, one A4 page in English) Official transcript prepared by the Head of the Department of your previous university Graduation Certificate Only individuals eligible based on qualification (2) must obtain and submit a graduation certificate issued by a national certification institution indicating that the applicant has completed or on the way of completing the first term of the doctor or master courses. In addition, applicants qualified under the provision of paragraph (2) must submit a certificate delivered by an academic certification institution. Proof of Examination Fee Payment The examination fee is 30,000. After paying by postal transfer, please attach the receipt to the Proof of Examination Payment form attached below. For details, refer to (1) of 2. Procedure below. 5

6 If you are an applicant residing outside Japan, payment by credit card is available by making an online application through International Student Portal of the University`s website. isc.tokushima-u.ac.jp//english/ Government funded international students and the applicants who have completed or are expected to complete the Master`s program in this university are exempted from the examination fee. Acceptance Letter Individuals who are currently enrolling in a graduate school or working in a company must submit an Acceptance Letter by an advisor or manager. Copy of Certificate of Alien Registration/Passport Stamped Envelope for Admission Ticket For other adult workers taking the entrance examination, please fill in the fixed format. Foreign nationals residing in Japan must submit a resident registration certificate and a recommendation letter from a research instructor. Fix a 82 stamp to the envelope and write your name, address and postal code clearly. (unless to bring directly ) 2. Procedure (1) Please hand them in with other required documents by the deadline. (2) Please choose registered mail and write Application for the Graduate School of Nutrition and Bioscience, Doctoral Course on the envelope with a red pen and General Foreign Student, Employed Student or Privately-Funded Foreign Student. V. Selection Method Selection of applicants is based on the overall results of the interview, achievement test and official transcript provided by an academic authority of your previous university. 1. Oral exam Oral examination consists of a 15-minute presentation focused on your master degree thesis, followed by questions related to your field of proficiency. (Note) Presentation slides for the oral examination (1) Each presentation will be displayed on a liquid crystal display monitor and the university will provide a computer, so no need to bring your own. (2) All computers used for presentations operate on Windows (OS Windows 7). A Mac PowerPoint file may display weird characters on Windows. So make that your document is saved on Windows configuration and that you have spare copies of your presentation document. (3) After the morning academic test, you are required to save the original of your research subject file on an USB flash drive or a CD-Rom bearing your name and examinee number, and then submit them to the responsible officer. 1 Date : The interview will be held in Graduate School of Nutrition and Bioscience at 1pm on Friday, March 11, Location : Tokushima University, Graduate School of Nutrition and Bioscience Kuramoto-cho, Tokushima Japan 3. Transcript Sealed transcript issued by the principal of your previous university will be used as a material for the selection. VI. Applicants with Disabilities Applicants for Graduate School of Nutrition and Bioscience with disabilities may need special consideration on the examination and need to contact the person in charge at the Graduate School 7

Announcement of Successful Candidates The results of the examination will be reported to all applicants on Friday, March 18, The results will be posted at the School of Medicine at 10 am and the applicant will also be notified by postal mail. Results by telephone inquiry will not be responded. VIII. Admission Procedure Friday, March 25 and Monday, March 28, 2016 IX. Entrance Fee and Tuition Fee 1. Entrance fee is estimated as 282,000 yen. 2. The tuition for the first semester will be 267,900 yen and annual tuition 535,800 yen (both are estimated amount). Notice: (1)In the event of a revision in the tuition fees, the new rate will be applied from the date of the revision. (2) The above fees are the currently estimated amount. If revisions are made, the new fees will be applied. X. Miscellaneous (1) After an application is submitted, fees cannot be returned. (2) Other costs (such as medical support fees) of 37,000 yen will be necessary at the time of entrance. Part of fees is not applicable for applicants who are from Tokushima University. (3) If sending application materials by postal mail please include self-addressed return envelope (33cm 24.0cm) with a 250 stamp (specifying Name, Address and Postal code) addressing to Graduate School Section, Educational Affairs Division, Faculty of Medicine, Dentistry and Pharmaceutical Sciences, Tokushima University.
