Tom Peterson is the contact person at this location. Attached, is a list of safety observations and recommendations based on conditions present at the time of my visit. Observations and recommendations are not intended to include every loss or accident potential. Inspection: Booker Elementary Location: Omaha, NE Site Contact: Tom Peterson Position: Superintendent # EE's/site: 22 Inspection Date: May 08, 2013 Scope of Work: New Construction, structural steel and precast panels. Score: 95% Observations Aerial Lifts Observation Workers in aerial lift tied off to boom or basket ( ) Observation Workers keep both feet on floor of basket, not allowed to climb/sit on edge ( ) Observation Aerial lift controls are plainly marked as to their function ( ) Cranes/Rigging Observation Crane(s) inspected and found to be in safe operating condition and properly maintained ( ) Observation Power lines are de-energized and grounded, and/or 20 foot clearance maintained and/or crane, line and load maintain clearance requirements in Table A Section ( ). Observation Accessible areas within swing radius of crane are not barricaded to prevent workers from being struck or crushed ( ) Recommendation Make sure all accessible areas within swing radius of crane are barricaded to prevent workers from being struck or crushed (reference ). Additional safety training and/or stricter enforcement may be needed to encourage more consistent compliance with this requirement. Photo(s) The radius guard is not installed on this crane. 1

Electrical Observation Electrical equipment installed/used as listed or labeled ( ) Observation Extension cord(s) have ground pin attached ( ) Observation GFCI used on 120V single-phase, 15 and 20 amp receptacle outlets ( ) Observation Extension cords in good condition/not worn or frayed ( ) Observation Breakers, switches and/or other disconnecting means are legibly marked to indicate purpose ( ) Fall Protection Observation Employees are protected from falls when working 6 feet or more above lower level ( ) Observation The PFA System is rigged such that the employee does not contact lower level ( ) Observation Top rail is installed 42 inches (+/-3in.) above working level and mid-rail is located half way between working surface and top rail ( ) Observation Top rail is capable of withstanding 200 lb. force applied downward and outward ( ) Observation Toeboards are erected where required to prevent objects from falling from higher levels ( ) Observation Harnesses maintained in good condition, no obvious damage, deterioration or defective components ( ) Observation Harnesses fitted/worn properly ( ) Observation Lanyards maintained in good condition/no obvious defects ( ) Observation Suitable anchorage provided for personal fall arrest system ( ) Excellent compliance! 2

4 Fire Safety Observation Fire extinguishers are maintained in operating condition ( ) Observation Portable fire extinguishers are inspected annually ( ) Observation Fire extinguishers are readily accessible/within 100 feet of work area ( ) Observation Only approved/ul listed fire extinguishers are provided ( ) Observation Flammable/combustible liquids are stored in approved/closed containers ( ) Housekeeping Observation Housekeeping is acceptable/reasonably maintained in work areas considering nature of work performed ( ) Observation Combustible scrap/construction debris removed at regular intervals ( ) Observation Material staging area kept reasonably organized ( ) Ladders/Stairs Observation Ladders are inspected/no visible defects ( ) Observation Side rails extend at least 3 feet above landing surface ( ) Observation Ladder(s) set up at proper angle ( ) Observation Ladder(s) set up on firm/level base ( ) Observation Side rails are secured to prevent side shift ( ) Observation Ladders are used for intended purpose ( ) Observation Handrail(s) not provided as required on stairs with 4 or more risers ( ) Recommendation Stairways having four or more risers or rising more than 30 inches, whichever is less, shall be equipped with at least one handrail or two if both sides are open (reference ).
