3 HEALTH PLANS Medical - BCBS Choice of 3 plans HDHP PPO PPO Buy Up higher monthly cost but with lowest annual deductible Dental -MetLife Basic Dental Plan Ortho Dental higher monthly cost but includes ortho benefit for dependents up to age 19 Vision Insurance - MetLife OPTUM BANK HSA OR FSA FORMS 3

5 LIFE, ACCIDENTAL DEATH & DISMEMBERMENT INSURANCE METLIFE Basic Life Gordian paid; 2x your salary up to a maximum of $500K Supplemental Life Employee paid; Statement of Health is not required in the initial 30 days of employment for coverage up to $100k; Late enrollees or EE requesting coverage over guaranteed issue amount must complete and submit PHS to MetLife and wait for approval before life coverage can be added; Dependent Coverage Spouse coverage available in increments of $5,000 up to 50% of EE coverage EE must enroll in supplemental life in order to insure dependents (spouse or dependent children) Dependent Children coverage available - $1k, $2k, $4k, $5k or $10k LIFE, ACCIDENTAL DEATH & DISMEMBERMENT INSURANCE METLIFE FORM 5

6 DISABILITY Long-Term Disability Employee Only Coverage - Dependents are not covered. Employer Paid Employee has option of paying taxes on premium - LTD benefits picks up after 90 days of disability Short-Term Disability Employee paid Elimination Period coverage picks up 7 th day of approved illness; 1 st day for accident Benefit 60% of weekly earnings with cap of $1,500 Plan will pay up to 13 weeks (LTD would pick up after STD is exhausted) OPTIONAL PLANS METLIFE FORMS 6
