11 Cigna Employee Supplemental Life Insurance Premiums for Supplemental Life will now be calculated as of 1/1 of the plan year Removal of the disability clause; Employees insured after 11/1/80 who become disabled prior to age 60 will no longer have a reduction in benefits. New combined maximum for Employee Basic & Supplemental Life ($1,150,000)
12 Cigna Dependent Life Insurance May now elect Spouse Life coverage in increments of $10,000, up to $250,000 (does not include AD&D) Spouse s coverage is not limited to 50% of employee s coverage amount Rates based on spouse s age as of 1/1 of the plan year New 10,000 Child Life policy for $1.00/month May keep the bundled dependent life option in 2013 ($4.70)
13 Cigna EOI: Open Enrollment Employee may increase 1 level, up to 3x salary with no EOI requirements (includes no coverage to 1x annual earnings) Employee may elect up to $30,000 in spouse life coverage without EOI New electronic EOI process; after the close of OE, employee has 31 days to complete online EOI application
14 Life Insurance Portability Option Employees may Port Life Insurance in 2013 Portability allows employee to keep the group term life coverage with most of the optional benefits at about the same premium (does not include AD&D, Will Preparation or Healthy Rewards) o Only available up to age 70 o Available for Spouse & Child life coverage o EOI is not required for keeping same coverage amount
15 Hyatt Legal/AFLAC Insurance Hyatt Legal Plan o No Plan Design Changes o 0% Increase AFLAC Group Critical Illness/Accident & Cancer Plans o No Plan Design Changes o 0% Increase
16 Donated Sick Leave Program Enrollment on an annual basis during the Benefits Open Enrollment period o Manual form available to enroll in program Eligible Requirements to Participate: o o o Employee who earns or accrues sick leave including PT employees Employed for a minimum of 6 months Must have a minimum of 48 hours of sick time accrued (pro-rated for PT employees) Donation of 8 hours leave every two years Must be experiencing a life-threatening or emergency medical condition (excludes maternity) Participants eligible for up to 12 weeks of donated leave within a two year period.
17 Retirement Contribution Changes Teachers Retirement System (TRS) o Effective July 1, 2012 Employee Contribution: 6% Employer Contribution: 11.41% Optional Retirement Plan (ORP) o Effective January 1, 2013 Employee Contribution: Increase from 5% to 6% Employer Contribution: Remaining at 9.24%
