1 Life Insurance The Company offers different life insurance coverage options to protect you and your family in the case of your death or accidental injury (AD&D insurance only applies to full- time employees). The life insurance is issued and administered by MetLife. Full- Time Employees Overview of Life Insurance Coverage Options for Full- Time Employees Coverage Amount Your Cost Basic Employee Term Life One times your salary, up to $1,500,000 Provided automatically at no cost to you AD&D One times your base salary Provided automatically at no cost to you Supplemental Employee Term Life* One to five times your salary, up to $1,500,000 including the company- paid basic life Spouse Term Life* Up to $200,000 in $25,000 increments Dependent Term Life* $10,000 per enrolled child Do You Have to Make an Election to Participate? No No Yes Yes Yes * Maximum coverage amounts are not guaranteed and are subject to approval of our provider, MetLife. You may be required to provide Evidence of Insurability You must be actively at work present at your job on your first day of coverage in order for life insurance benefits to take effect. For open enrollment elections, this means you must be actively at work on January 1, 2016 for your coverage to take effect that day. If you re absent on January 1, 2016, your coverage will not take effect until you return from work. For all other enrollment elections, your coverage will take effect on the first of the month following your date of hire. If you are absent on that day, your coverage will not take effect until you return to work. Basic Employee Term Life Insurance and AD&D The Company automatically provides you with basic employee term life insurance. The basic life insurance plan pays a benefit of one times your salary, up to $1,500,000. In the event of accident- related death, Accidental Death & Dismemberment (AD&D) insurance pays an additional benefit of one times your 1
2 salary. Life insurance benefit amounts are reduced when you reach age 65 and again when you reach age 70. The Company pays the full cost of basic employee term life and AD&D insurance. You do not need to enroll or contribute toward the cost of this coverage. Imputed Income on Basic Employee Term Life Insurance The Company pays the full cost of basic employee term life insurance. You may be subject to imputed income for the value of some of your life insurance. If the Company provides you with life insurance coverage of more than $50,000, an additional amount of your income will be subject to Social Security and federal income taxes. The IRS considers this imputed income when determining the amount of your taxable income for the year. This imputed income amount will be shown on your pay advice and on your W- 2 earnings statement at the end of the year. For example, if your basic life insurance coverage is $60,000 and you are 40 years old, the value of $10,000 ($60,000- $50,000) of insurance is $12 annually. This is the amount that will be subject to taxes because the IRS considers it as additional income. Be sure to complete a beneficiary designation form, and to verify that any forms you may have completed previously are up to date. If you have only one beneficiary on file and you are enrolled in more than one plan, MetLife will recognize the most current beneficiary information on file for your company- paid basic term life insurance plan. Definition of Salary for Basic Employee Term Life and Supplemental Employee Term Life Your salary for purposes of Life Insurance is defined by your Basic Annual Earnings. Basic Annual Earnings means your gross annual rate of pay as determined by Your Policyholder for performing the duties required of your job. "Basic Annual Earnings" includes draw and the annualized average commissions earned during the shorter of: (i) the 12- month period just prior to your date of loss; or (ii) your period of employment. Bonuses, overtime pay, earnings for more than 40 hours per week, and all other benefits are not included. Supplemental Employee Term Life Insurance You may elect coverage amounts from one to five times your salary, up to $1,500,000, including the company- paid basic term life insurance. Your final coverage amount is subject to approval by MetLife. New employees may enroll in supplemental term life insurance of one, two, or three times your salary, up to $750,000, without providing Evidence of Insurability. Supplemental life insurance coverage rates depend on your age and how much coverage you elect. The rates shown in the following chart are monthly costs per one thousand dollars of coverage. 3 45-49 $ $ $ $ $1.053 $1.708 Dependent Spouse Term Life Insurance You may elect life insurance coverage for your spouse in increments of $25,00 as long as he or she is not an employee of the Company. This coverage cannot exceed $200,000. The amount for which you are responsible depends on the coverage amount you choose and your spouse s age. You may be required to submit Evidence of Insurability for your spouse if you elect life insurance for him or her. Spousal rates depend on your spouse s age and how much coverage you elect. The rates shown in the following chart are monthly costs per one thousand dollars of coverage. Dependent Spouse Term Life Insurance Rates $ $ $ $ $ $ $ $ $ $1.27 $2.06 Dependent Child Term Life Insurance Dependent child term life insurance coverage pays a benefit to you if one or more of your covered children die. You enroll for this coverage and pay one premium amount to cover all the children you identify as dependents. If one or more of your children dies, the benefit will pay $10,000 per eligible child. The cost of this insurance is $0.70 per thousand dollars of coverage per month. 3
