2 Our Story The idea for Harken Health grew from a series of conversations among a small group of health care industry veterans who, collectively, had seen it all. They wrestled with the fact that so many patients were frustrated with the current system and so many of their needs were left unmet. With many obvious opportunities to improve health care, they narrowed in on two core elements. These served as the foundation for Harken Health: Most insurance services are designed specifically for employer customers, which leaves the individual experience sorely lacking. Over the last few years, the industry focus has been shifting to prioritize experiences for individuals who use insurance. We believe we can make that experience even more positive by creating a meaningful health care experience for them. A Care Team health care model delivers the best, highest level of care to an individual. We know that many talented, well-intended health care professionals work in the system today. But we also know that the system prevents these professionals from delivering compassionate care to patients in a consistent and high quality manner. 4 Tom Vanderheyden CO-FOUNDER AND CHIEF EXECUTIVE OFFICER With a leadership background in early-stage business building and professional services, Tom Vanderheyden is a co-founder and the Chief Executive Officer of Harken Health. He is excited to be leading the company s launch, combining his professional background and personal passion for helping people. Prior to Harken, Tom was Vice President of Development and Commercialization at UnitedHealth Group, a company he joined in He held leadership roles as Chief Operating Officer of UnitedHealth International, in divisions involving mergers and acquisitions, strategic sourcing and the launch of a large, global operation in India. Before that, Tom spent 20 years creating an IT infrastructure outsourcing business, managing a technology and consulting firm, and held sales and consulting roles at IBM. Tom s passion for caring for others began at an early age. The youngest of eight children, he left home at the age of 13 to enter the seminary to pursue interest in a life of service to others. Tom believes people do generally well on their own most of the time, but that they also experience times of struggle. It is in those moments that we are called to provide compassion, empathy and care to others. This is the foundation of Harken Health. EXPERTISE Health care operations Technology systems engineering

5 Ryan Armbruster CO-FOUNDER AND CHIEF EXPERIENCE OFFICER A recognized pioneer and leader of human-centered service design in the health care industry, Ryan Armbruster is a co-founder of Harken Health and is designing health services that will provide better experiences for people that need care. Prior to Harken Health, Ryan built enterprise-wide programs to improve innovation as Vice President, Innovation at UnitedHealth Group, including creating and leading their Design Studio. He also created and directed the SPARC Design Lab at Mayo Clinic, a groundbreaking center for innovation in health care services. The comprehensive program and physical laboratory developed new health care services and experiences for patients and care providers. For the past decade, Ryan has taught health care service design and innovation at the University of Minnesota, the school that granted him a Masters in Health Care Administration and the inaugural Alumni Innovator Award in Ryan grew up in the countryside near Ellsworth, Wisconsin, best known as the state s Cheese Curd Capital. When not photographing wildlife, (ask him about capturing a tender moment between a mama bear and her cub in British Columbia) he loves to fish or waterski with his wife and two children. 6 Krista Nelson CO-FOUNDER AND VP OF STRATEGY AND PROGRAM OFFICE An experienced health care professional with product development and consulting experience, Krista is a co-founder and VP of Strategy and Program Office for Harken Health. She leads business strategy and manages all program office functions. Prior to joining Harken Health, Krista was a Product Director of Innovation at UnitedHealthcare, responsible for developing new products, services and capabilities for members, employers and providers. She also worked for Ingenix and Deloitte Consulting where she guided Fortune 100 companies on new product development, strategic planning, mergers and acquisitions, and customer experience strategies. Krista earned her MBA from the University of St. Thomas and received a B.S. from the University of Minnesota. To say Krista grew up with a large family is an understatement. Her parents fostered numerous children at their home in Mankato, Minnesota, and each child they welcomed became a genuine member of the family. Watching her parents provide love and care to those who needed it most shaped the way Krista lives her life. It also became part of the inspiration for Harken Health, a place where caring is the most important aspect of health care. 7 Marcus Robinson VICE PRESIDENT OF SALES A professional experienced in sales strategies and products for the health care industry, Marcus Robinson is the Vice President of Sales for Harken Health. He oversees sales operations development, leads and formalizes all sales distribution strategies in commercial segments, directs the recruiting and hiring of sales and account management staff and cultivates relationships with distribution partners. Marcus also facilitates work across the company s divisions to drive sales growth and retention. Prior to Harken Health, he was the Director of Sales and Account Management for the Small Business division at UnitedHealthcare, a company he joined in In each of his four roles during his career there, he focused on small business insurance sales strategies, products and services. Marcus earned his B.S. in Marketing and Information Management from the University of Tennessee. Growing up, Marcus s family moved around throughout the Midwest, and he finished high school in Fayetteville, Arkansas. He and his wife have their hands full with their toddler son, but they do make time to enjoy an active lifestyle by participating in Crossfit as well as outdoor activities. Marcus leads a Sunday School class at the First Baptist Church of Atlanta, and if he is lucky enough to steal any spare time, he likes to ride motorcycles around the city. 8 Chandra Torgerson CLINICAL AND POPULATION HEALTH LEADER A 30-year veteran of the health care industry with expertise in clinical operations, outcomes and quality measurement, Chandra Torgerson is the Clinical and Population Health Leader for Harken Health. In addition to partnering with Harken providers to improve members quality of care, she develops policies, procedures and guidelines that impact medical and clinical operations. Prior to joining Harken Health, Chandra served in clinical operations leadership roles (including Chief Nursing Officer) at UnitedHealthcare. She also worked as a Quality Improvement Management Consultant and as the Director of Nursing for Fairview Health Services in Minneapolis. She completed her postgraduate studies at the University of Minnesota and earned her B.S. in Nursing at Minot State University in North Dakota. Notably, she has also received a Six Sigma Black Belt certification and regularly applies those principles to her management work. Chandra grew up in Crosby, North Dakota. On weekends, Chandra and her partner drive a few hours from their home in Minneapolis to experience nature at their cabin in Northern Minnesota. 9 Hogai Nassery, MD MEDICAL DIRECTOR, ATLANTA A primary care physician with almost 20 years of family medicine experience in both patient care and academic settings, Hogai Nassery is the Medical Director for Harken Health in Atlanta. She oversees the operations of all six Atlanta Metro Harken Health Centers. Prior to joining Harken Health, Hogai served as Deputy Vice President of Ambulatory and Community Medicine, Medical Affairs at Atlanta s Grady Health System. She advocated access to quality health care regardless of income, insurance or immigration status. Hogai simultaneously served as Director of the Division of Community Medicine at Emory University School of Medicine, where she held a teaching position for 18 years. Hogai received a B.A. in Biology from the University of West Georgia and an M.D. at Medical College of Georgia. She completed her residency in Family Medicine at the University of Cincinnati/Providence Hospital. Born in Kabul, Afghanistan, Hogai s family immigrated to the U.S., where she was raised in Georgia. Hogai returned to her birth country at the age of 18 to volunteer in Afghan refugee camps an experience that drove her to become an advocate for social change. Hogai enjoys photography and knitting, and along with her husband, spends much of her free time caravanning her teenage sons to soccer matches. 10 Carolina Pavese, RN DIRECTOR, PRACTICE OPERATIONS, ATLANTA A registered nurse and experienced manager, Carolina Pavese serves as Practice Operations Director of Harken Health Atlanta, where she leads overall strategies that focus on clinical development, business operations, medical management, patient engagement and enrollment. Prior to Harken Health, Carolina was the Director of Clinical Operations at Iora Health where, among other duties, she served as the liaison to practices, business and medical management teams within the organization. She also worked at the Culinary Health Fund, where she helped to found a comprehensive maternity program designed to provide support and education to women and their families. Carolina s childhood passion to help people led her to pursue a Bachelor of Science in Nursing, which she earned from Chamberlain University. With an extended Colombian family she is fluent in Spanish and spent many of her formative years in Colombia as well as Guatemala learning about the cultures. Her family eventually settled in Cape Coral, Florida, where she finished high school. Carolina, her husband, and three children currently live in Atlanta. When they aren t spending time outdoors together, they are cooking, reading or cheering for their favorite sports teams. 11 Jaizon Plaga DIRECTOR OF ACCOUNT MANAGEMENT, ATLANTA & CHICAGO A sales leader with expertise across numerous industries, including health care, Jaizon Plaga is the Director of Account Management in Atlanta and Chicago for Harken Health. In addition to creating and maintaining productive relationships with the company s broker partners, he guides policy, procedure and process workflows for effective sales strategies. Prior to joining Harken Health, Jaizon worked as a Regional Sales Manager and Sales Supervisor for Assurant Health, consulting with brokers in numerous states on individual medical, ancillary and small group health insurance products. Jaizon earned a B.S. from the University of Minnesota Twin Cities after completing its Inter-College Program (ICP), where he double-majored in Applied Business and Accounting. Jaizon was born and raised in a tight-knit Filipino family in Milwaukee and considers himself a true Wisconsinite because he loves cheese, anything fried at the state fair and all the state s sports teams especially the Packers, Brewers and Bucks. He has volunteered extensively with the Filipino American Association of Wisconsin s Free Medical Clinic as well as the AIDS Resource Center of Wisconsin. He and his spouse devote a lot of their energy to keeping up with their four rambunctious dogs. Jaizon is also a singer/songwriter in his spare time and actually made it all the way to Hollywood Week during Season 10 of American Idol. 12 Bruce Doblin, MD-MPH CHICAGO REGIONAL MEDICAL DIRECTOR A primary care physician with experience starting up and running both for-profit and not-forprofit health care programs, Bruce Doblin is the Regional Medical Director for Harken Health in Chicago. In addition to caring for his own patients in one of Chicago s Harken Health Centers, he will oversee the opening of all four of the Chicagoland Harken Health Centers. Prior to joining Harken Health, Bruce practiced community-based primary care for more than 25 years in his own practice. He was the founding director of three hospice and palliative care programs, served as a health care consultant for the state of Illinois and directed a notfor-profit health care advocacy organization. Bruce received a B.A. in Economics from Williams College and an M.D.-M.P.H. (Master of Public Health) at Northwestern University. He was awarded clinical fellowships at Northwestern University as well as fellowships in health services research at UCLA and medical ethics at the University of Chicago. Bruce was born and raised in Chicago and has practiced Vinyasa yoga for the last 20 years. He enjoys cooking with his wife and teenage son from healthy homemade granola and robust soups to not-quite-as-healthy (but equally delicious) double chocolate brownies.
