Defibrose The tissues to restore their elasticity STEP 5 Firm Up The skin to improve its tone Classifying Cellulite ( WATER+ FAT+VASCULAR FACTORS) Compact cellulite 1 st stage Between the skin and the muscle fascia there are cells that form the so-called adipose tissue that is crossed by numerous blood vessels. An alteration in the permeability of the capillaries leads to a loss of the watery part of the blood, known as serum, with a consequent build-up and accumulation in the interstitial tissue located between the adipose cells. If the process is not stopped, this seepage of water into the interstitial tissue leads to a separation and disassociation of the adipose cells with a consequent break-up of the cells themselves and the formation of gaps. 3 adipose cell is wrapped by a network of very fine reticular fibres. A repetition and duration of the oedema phenomenon leads to a defensive reaction by the fibres themselves that react by increasing in number (hyperplasia) and in thickness (hypertrophy). All this results in an alteration to the normal (lobe) structure of the adipose tissue and a dilation of the micro vessels. The circulation of the blood is compromised and if this phenomenon continues for a long period, the adipose tissue hardens (see sclerosis) and, in other words, becomes harder and more painful. Soft cellulite 3 rd and 4 th stages (formation of micro nodules). With the passing of time, the reticular fibres become collagen fibres; the latter tend to encapsulate the degenerated adipose cells with the formation of true micro nodules, that is of rounded structures of microscopic dimensions. 4 th stage (formation of macro nodules). When several micro nodules combine together, they give rise to the formation of a macro nodule, which is evident to the touch and painful when pressed. When talking of cellulite, it is always thought that only women are affected; however, there is a symptom similar to cellulite that affects men, although to a lesser extent and it is known as male cellulite. There are basically two differences between the female and the male phenomenon: the origin and the location of the pockets of fat deposits. Male cellulite is caused by a tendency to retain liquids in the tissues: therefore, it is a problem of constitution. For men the problem is characterized by rolls of fat in the area from the waist upwards: arms, back, stomach, but above all, the waist. Sagging Tissue STEP 1 : Two Basic concept need to be kept in mind - Cutaneous tension of the skin - Flexibility of the skin - These two factors determine the elasticity and its capacity to adapt itself to anatomical controls as a function on movement STEP 2: Physiologic Balance - The fibroblasts of the dermis synthesize 2 essential proteins that create the fibres that organize the dense networks -The collagen function is to support the tension forces within the skin -The elastine contained in the elastic fibres can shrink or stretch in order to create and control cutaneous flexibility - Problems, stemming from either a lack of the 2 proteins or poor quality in the 2 proteins, will result in sqgging of the cutaneous tissue. Anterior tibial vein) -Common Linking Point: Thigh Ring (At the Scarpa level) which becomes the external iliac vein STEP 2: Diagnostic Procedures 1. Find the oedema areas With the tissue impression test to determine the origin of the oedemas (venous or lymphatic) 2. Palpation of the saphen areas From the bottom to the top 3. FIG 3 place the probe for 40 seconds on every spot following the order: from 1 to 23 on the right leg and from 1 to 23 on the left leg. Then, gently and slowly brush from above knee to foot (A) and after from hip under knee (B) the concerned area. Only brushing may be done on every other day.
