QUALITY CARE AT YOUR NEIGHBORHOOD SCHOOL ENRICH your child s early educational experience by EXTENDING his or her learning and fun right at your home school! PROGRAM FEES: (Due at registration) $392 per month tuition for all day care. $50 per family annual registration fee. $13 per student for personal mat. (Total due with application $ in a check made payable to Jackson Board of Education. Registration fee is non refundable; Tuition non refundable after August 1st) Tuition fee includes any combination of childcare before or after the kindergarten session - between the hours of 7 a.m. and 6 p.m. Certified teachers will use fun and engaging activities to further develop fundamental learning and socialization skills. Activities include: - Music Appreciation - Science Experiments - Geographical Habitats Program also includes rest time and an opportunity for children to socialize with their c classmates in a relaxed educational setting. AVAILABLE FOR THE SCHOOL YEAR* * Space is limited to 20 students per class. Program must meet a minimum enrollment to run at each home school. An informational Extended Enrichment Kindergarten meeting for parents only will be held on Thursday, March 20th at 7 p.m. in the All Purpose Room at the Holman Elementary School. (Snow date-tuesday, March 25th) Registration forms with payment will be accepted at the close of the meeting and continue through April 30th on a first come first serve basis. Breakfast* and lunch are available at reasonable rates. 3 EEK TUITION AGREEMENT I, (parent/guardian) certify that presently resides with the Name of child/ren undersigned at the designated address and is a student enrolled in the Jackson Township School District. All the regulations and provisions offered by the Jackson Township School District in connection with its Jackson Community School /Extended Enrichment Program are posted on their website and I hereby agree to abide thereto. I further acknowledge that the Jackson Township Board of Education may terminate this agreement for any failure of the undersigned, or the child, to abide with the regulations and conditions referred to in the above documentation and for any other cause which the Jackson Township Board of Education feels, in its sole discretion, is not for the best interest of the child or the district, provided the undersigned receives five (5) days prior written notice. The undersigned agrees to pay all tuition payments in accordance with the Jackson Community School, between the 15th and the 25th of the month PRIOR to services rendered (See payment chart). I understand that the child/ren shall not be permitted to participate in this program if the tuition and any late charges are not received in full on the date set forth above. As per Jackson School District policy there will be no credit or refund of fees given for unused time, i.e. cancellation, vacation, illness, absences, snow day interruptions including delayed openings and early closings, etc. Due to limited program space, any delinquent account will result in cancellation of services and loss of program placement due to active waiting list. Any default in the payment of tuition or other related charges shall result in the undersigned being responsible for the same, plus interest, as well as any cost, including reasonable attorney fees, that may be necessary for the Board to expend in collecting the same. Payments are due on the 25th of each month beginning September 25th for October. You will not receive monthly bills. You may pay the tuition through our free online payment option. Please note: If you were registered for the online payment option through the Pre-K or Childcare program last year you must request your old account to be closed in order to use the same for EEK this year, or provide us with a different to be used with the EEK program this year. PLEASE NOTE : Any Pre-Kindergarten or Childcare Academy accounts must be up to date in order to participate in the EEK program. ALL accounts must be fully paid by June 30, 2014 in order to secure your registration in the EEK program. In acknowledgment of the terms of this agreement, please sign below. REMINDERS: Monthly tuition is based on 180 scheduled school days each year; divided into 10 months of the school year for an average of 18 days per month. Therefore, tuition for each month is identical from September to June, inclusive. Account Holders will NOT receive monthly invoices. Account holders are responsible for monthly payments due as per the above detailed chart. Although not the desired outcome, when warranted Jackson School District will exercise its right to place a child s enrollment in jeopardy and/or be cancelled due to non-payment of tuition/other charges for services rendered. Effective immediately we will more closely abide by the agreed upon/contracted regulations stated on your signed application as we can no longer allow accounts to go unpaid for extended periods.
