7 Kidz Kollege / UIL / Tutorials / Other after school activities If a child will be participating in any after-school activities that require him/her to leave Kidz Klub, the Community Education Office must receive written permission from parent/guardian prior to the student being released, either hand-written or by to [email protected]. The procedure for the release of students in the program is: Students must report to Kidz Klub for attendance and receive their snack prior to release. The teacher will then pick the student up from Kidz Klub and return the student to Kidz Klub after the activity. DISCIPLINE We want our program to provide a safe, healthy, friendly and fun environment for your child/children. In doing so, we will issue discipline referral forms for unsatisfactory behaviors, including children who repeatedly hurt themselves, others or property, are continuously defiant or disrespectful or refuse to follow the rules and guidelines. The parents will be informed each day their child receives a discipline form and will be asked to sign it. If a child receives 3 discipline referral forms, he or she will be unable to attend child care for one week. After the child returns, if he or she receives 2 more forms, they will be unable to return to child care for the remainder of the year and no refund will be given*. NOTE: The director may, at any time, remove a student immediately from the program for any of the following reasons: 1. Refusal by a student, after being warned, to obey the stated rules. 2. Verbal abuse, physical abuse, or sexual harassment of students or staff members by students or their parents. 3. Failure of parents to pay fees in a timely manner. 4. Continual late pick-up. * DSISD understands that the removal of a student from the after-school program may create a hardship on the parents. When a student is removed from the program and the parents have no alternative care solution, the parents may request an assigned aide to work with the student. This type of one-on-one arrangement is subject to the following terms. 1. One-on-one care is contingent upon the availability of personnel. 2. Should the assigned employee be absent, all efforts will be made to secure a substitute. In the event no substitute is available the parent(s)/guardian will be notified as soon as possible that the student must be picked up immediately after school. 3. The parent/guardian will be responsible for the additional cost of the individual care provider at a rate of $800/month for 5:00 pick-up or $1200/month for 6:30pick up. 4. If the behavior that warranted removal from the program continues or becomes unbearable, the student will be removed from the program and the one-on-one care arrangement with no further options. 5. Payment of the additional fee is due with the regular after-school care tuition. 6. An agreement for one-on-one care in no way alters the terms applicable to after-school care.
