5 Sign this form to confirm that you understand and agree to the HungerRush Terms of Service. HungerRush Service Agreement The HungerRush Service Agreement terms are twelve (12) months, and will automatically renew when expired unless the customer submits a signed cancellation notice to Revention, Inc. 30 days in advance. The monthly service fee for the HungerRush module is $59 per month via ACH or for a prepaid annual fee of $708 (payable by check or credit card). Texas residents should add 8.25% sales tax. In the event of a transfer of ownership or change of merchant information, Revention Credit Server license is not transferable and must be repurchased. Revention, Inc. reserves the right to change the HungerRush service fees at any time. Customer Requirements: The customer agrees and understands that in order to maintain a HungerRush website, the following conditions must be satisfied: o The customer must maintain an active Revention Service Agreement. o The customer s account with Revention must be current. Any outstanding or unpaid invoices may result in the termination of the customer s HungerRush service. o The customer must utilize a static IP address a dynamic DNS service. o The customer must obtain high-speed Internet access. o A completed ACH form and signed HungerRush Service Agreement must be submitted to Revention, Inc. o The customer must use Revention Credit Card Processing software. High-Speed Internet Maintenance: The customer is responsible for maintaining high-speed Internet access. Revention, Inc. is not responsible for any data transmission issues that arise as a result of an interruption in internet connectivity by the customer s Internet service provider. All troubleshooting related to internet connectivity is the responsibility of the customer. Technical support: HungerRush customers are eligible to request technical support for the HungerRush module in accordance with the terms and conditions outlined in the official Revention Service Agreement. By signing this document, the customer acknowledges that he or she has read all of the terms and conditions of this agreement.
