Bob Fiala, aia, ncarb certified Bob joined the design firm of Huston Associates in 1976 and in 1989, he formed his own company by acquiring Huston and establishing ThenDesign Architecture, Ltd. (TDA). His extensive experience and leadership has resulted in great project success in many different facility types including healthcare, residential and hospitality in both public and private sectors. 4 Altenheim Shurmer Place / Strongsville, Ohio owner / West Side Deutscher Frauen Verein project type and size / 25 unit addition to Shurmer Place Assisted Living and redesign of campus grounds / 35,000 total SF architecture, landscape architecture 25 studio, one bedroom and two bedroom units were added on two residential floors with a partial basement that has a large gathering room, catering kitchen, spa, and mechanical spaces. The existing unit designs had been very popular and so became the basis for the design of the new units activity spaces such as a library, TV room, parlor, lounge areas along with staff spaces and resident laundries were included in the planning of this addition. The appearance of the existing facility was repeated for continuity the original site had circulation and functional issues so it was decided to completely reconfigure and improve the circulation and parking system

5 Astoria Assisted Care Residence / Meyers Lake, Ohio owner / MSTC Development, Inc. project type and size / 60 unit assisted living facility, new construction / 62,500 SF this 60 unit assisted living residence is to be built on the Astoria mini campus adjacent to the 83 bed nursing facility and will share common drives, utilities, etc. It will have areas for assisted living residents and for those who need a memory care setting studio, one bedroom, and two bedroom units will be placed on three fl oors with large living, dining, and activity areas that will be arranged into an enjoyable indoor and outdoor living environment for seniors

6 Astoria Skilled Nursing and Rehabilitation / Meyers Lake, Ohio owner / MSTC Development, Inc. project type and size / 83 bed nursing facility, new construction / 46,600 total SF this facility is planned with private and semi-private rooms and two nursing centers, two dining rooms, two resident lounges and a large therapy area for inpatient and outpatient use there is a large central activity area and administrative suite along with all the necessary support functions needed in a freestanding nursing home

7 Bath Creek Estates / Cuyahoga Falls, Ohio owner / Bath Creek Healthcare Holdings, LLC project type and size / assisted living facility: 82,000 SF, nursing facility: 32,000 SF, and condominiums a 3-story, 80 unit assisted living facility will be joined to a 50 bed skilled nursing facility by shared support functions such as the main kitchen, laundry and other common facilities amenities include lounges, activity areas, dining functions, library and computer center, media room, gift shop, chapel, exercise and therapy areas and other related support functions throughout the ALF and skilled nursing units

8 Brecksville Assisted Care Residence / Brecksville, Ohio owner / McCabe Company project type and size / 80 unit assisted living facility, 90,000 total SF the site for this facility is a heavily treed site close to the downtown area of Brecksville and across the street from The Oaks nursing facility. Next door to the north is St. Basil s church which will provide many interactive opportunities for the residents. the architectural style for this facility includes red-brick, stone, white trim and other traditional features of buildings found in this Western Reserve community. maximum use will be made of the grounds around the building including a large wooded area to the south which will be the location of outdoor open and covered patios, walking areas, etc. the units will be spacious and the facility will have all of the dining and activity spaces necessary in this up-scale residential setting

9 Concord Assisted Living / Lake County, Ohio owner / Vista Springs project type and size / 100 unit assisted living facility, new construction / 111,000 SF this facility is to be built on a heavily treed site which is adjacent to the recently built regional Tri-Pointe hospital in Concord, Ohio. It will be a 3-story building housing assisted living units and will have a one-story memory-care unit adjacent on the site which will be served out of a main kitchen and other service support functions a mix of studio, one-bedroom, and two-bedroom units will be offered in the assisted living portion of the the building while interesting and dynamic activity, lounge and dining spaces throughout the facility will be provided to create a desirable living environment the memory-care area will have smaller living units that will focus on dining, activity and program spaces designed to offer a secure setting which will foster a high quality atmosphere for living

10 Covenant Village of Middletown / Middletown, Ohio owner / Health Care Management Group project type and size / 99 bed short term rehabilitation nursing facility / 95,500 total SF upscale, large short-term rehab facility is designed with spacious private rooms therapy area is designed as a therapy main street concept in the center of the facility providing physical, occupational, speech, and aquatic therapy services in a spacious area with areas for relaxing and getting refreshments between therapy sessions

11 Covington Health Campus / East Palestine, Ohio owner / Jilltin Management, LLC project type and size / 19 unit independent living facility, new construction / 15,750 total SF 19 unit independent senior residence component of the Covington campus, complementing the 50 bed nursing facility and is designed for future additions resident units inlude studio, one bedroom, and two bedroom units offering a range of options this facility has a central lounge, activity space, and a large dining room served from the kitchen in the nursing facility

12 Covington Health Campus / East Palestine, Ohio owner / Jilltin Management, LLC project type and size / 75 bed nursing facility, new construction / 38,000 SF mix of private and semi-private rooms arranged in four wings with one central nursing center, central dining room, resident lounge, activity areas, and administrative suite this facility is the fi rst built on this campus which is intended to be dedicated largely to health care and senior living uses

13 Darby Glenn Care Center / Hilliard, Ohio owner-operator / Provider Services project type and size / 100 bed nursing facility, new construction / 70,000 SF 100 large private rooms arranged in smaller 12 to 13-bed wings for resident privacy and to create smaller, more intimate living areas within the larger facility each of two nursing centers is surrounded by two dining areas and two lounges to provide a more intimate atmosphere for the residents to relax and dine there is a large central activity and therapy area along with other smaller activity areas such as a library, computer center, personal care and area. All of the elements of the plan are arranged around a large central courtyard providing a landscaped focal point and a secure outdoor area for the residents use

14 Edwin Shaw Rehabilitation Institute / Cuyahoga Falls, Ohio owner / Falls Village Realty, Ltd. project type and size / Inpatient Rehabilitation Hospitals / 29,500 total SF this is a re-purposing of the ground fl oor assisted living section of the existing Falls Village, a mixed use health care setting the plan provides sections for private rooms for patients undergoing treatment of traumatic brain injury, spinal injury, and general orthopedic cases. Nurses stations and all necessary support functions are placed in support of the care program the original plan had a large activity and dining area with skylights which was converted to a large therapy area with speech, occupational, and physical therapy treament areas along with an offi ce for 14 therapists

15 Good Shepherd Home / Fostoria, Ohio owner / Good Shepherd Home project type and size / 14 unit apartment building (HUD 202) / 12,500 total SF this HUD 202 facility was planned to complement the other senior care and living options on the extensive Good Shepherd campus All resident units are provided with a living room, open kitchen, and one bedroom with a full bath. The building common areas include a lobby/lounge with adjacent kitchen and a resident laundry the central corridor has several wider areas which can be furnished with seating groups

16 Good Shepherd Home / Fostoria, Ohio owner / Good Shepherd Home project type and size / phase 2 nursing wing and administrative area renovation / 36,650 total SF phase 2 of this project includes a new 32 bed nursing wing with a dining room and spacious lounge and activity areas. The main entrance of the original nursing home was improved by reconstructing the main entrance and administration offi ces a spacious reception area leads to the new Main Street and to other resident s areas. Main Street provides amenities such as a General Store, post offi ce, snack area, lounge area, beauty/barbor shop, bakery, bank, and activity areas

17 Good Shepherd Home / Fostoria, Ohio owner / Good Shepherd Home project type and size / assisted living additions and improvements / 14,250 SF 15 existing units were doubled in size and 10 new one bedroom units were added assisted living dining area was more than doubled and a servery was added for providing the quality of dining desired also included in this phase was the renovation of the entrance to Brethren Court and a nursing center was added for this wing

18 Good Shepherd Home / Fostoria, Ohio owner / Good Shepherd Home project type and size / inpatient/outpatient therapy addition / 11,400 SF this therapy addition to Good Shepherd will increase the capability of serving residents inside and outside of the home services offered include Speech Therapy, Physical Therapy, Occupational Therapy, and Aquatic Therapy in new spaces and are readily accessible from the center of the home s nursing areas as well as from an outside entrance

19 Graceworks Enhanced Living / Wyoming, Ohio owner-operator / Graceworks, Inc. project type and size / 10 bed ICF/MR Group Home / 4,852 total SF this Group Home includes two private bedrooms and four shared bedrooms for the residents, each including a full bath common areas include a large kitchen and adjacent great room, living room, central bathing room, 1/2 bath, laundry, and mechanical room

20 Legacy Place of Twinsburg / Twinsburg, Ohio owner / Legacy Health Services project type and size / 30 bed nursing/rehab addition / 21,000 total SF 30 bed nursing wing being added to an existing facility that has 26 nursing beds on the fi rst fl oor and 28 assisted living units on the second fl oor 10 of the 30 private rooms are large enough to receive an additional bed in the future to create a 40 bed addition the new wing connects at 2 corridor ends of the existing building and hall all of the necessary functions such as dining and activity, bathing, resident lounge, nursing center and a large inpatient/outpatient therapy suite the addition has a separate exterior entrance and will feature up-scale decor and design reminiscent of hospitality facilities

21 Mapleview Country Villa / Chardon, Ohio owner / Legacy Health Services project type and size / 100 bed nursing facility, new construction / 70,000 total SF the 100 large private rooms are all arranged in smaller 12 to 13 bed wings for resident privacy and to create smaller, more intimate living areas within the larger facility each of two nursing centers is surrounded by two dining areas and two lounges to provide a more intimate atmosphere for the residents to relax and dine a large central activity area and therapy area along with other smaller activity areas such as a library, computer center, and personal care center are provided. 22 North Canton Project / Canton, Ohio developer / Senior Living Solutions Group, LLC project type and size / 105 unit residential care and 30 bed skilled nursing facility, 137,500 total SF concepts are being develped to join this residential care facility with a memory care area can be paired on site with skilled nursing facility specializing in short-term rehabilitation the residential care facility will be a 3-story building with memory and some assisted living units; ground fl oor functions will be focused around a protected landsacped courtyard the nursing facility is designed on one fl oor for 30 large private rooms and will be suitable for a short-term rehab program with large therapy area for speech, physical and occupational therapy; possible aquatic therapy as well the nursing facility will be joined by a connector to the ALF buildign to share the kitchen and laundry functions and to facilitate staff movement between buildings

27 Vista Springs ALF and Memory Care / Macedonia, Ohio owner / Vista Springs, LLC project type and size / 96 unit assisted living and memory care / 102,500 total SF This site is in an ideal location in Macedonia, has great visibility from a major freeway and is close to the business area of town. The site, however, is small and provided site engineering challenges including setback variances and grading issues The Vista Springs Company has a number of facilities and wished to have some of their existing themes including trim color, a sign style, a specifi c bay window, etc. used in the development of the design for this building The interiors of the facility will be upscale and vibrantly hospitable in feel and will have all of the features and amenities desired in this upscale market for the benefi t of the residents

28 Woodland Country Manor / Somerville, Ohio owner / Woodland Country Manor, Inc. project type and size / 50 bed nursing facility / 40,150 total SF the original 50 bed home was outdated, very small and lacked the necessary amenities and features to be effectively marketed in the current environment. The large site is wooded and has adequate space for future expansion and additional buildings for assisted living, independent living and other related uses to form a CCRC Additional administrative spaces, expanded dining, an expanded kitchen, a new therapy suite, new activity spaces, more private rooms, etc. were added in the nursing wing with a central courtyard as a main focus. A small assisted living wing with a seperate entry was also added; it is served from the main kitchen and laundry
