1 Forest Side Assisted Living for those with Dementia Repositioning/Adaptive Reuse Constructed in the mid-twentieth century as a home for elderly women, this structure is located in an upscale Washington neighborhood adjacent to Rock Creek Park. The Methodist Home of DC purchased the property for purposes of conversion to an assisted living for those with dementia within small household settings. Restricted in exterior design by the DC Fine Arts Commission because of the adjacency to the park, the design successfully retains the exterior design feeling while adding necessary area to accommodate contemporary care provision. The result is an example of attention to detail in renovation and a comprehensive understanding of how household design in aging care must integrate with the care program and staff convenience. 2 Arbor Springs Alzheimer s Assisted Living/Alzheimer s Nursing Charged with providing a versatile design that would allow dementia specific residents to age in a place, maintain their independence and allow them a maximum amount of freedom, security and privacy, we prepared a concept that would place the residents in six distinct neighborhoods, each with its own living and socialization areas. The neighborhoods then come together into two communities where group activities and dining take place. Public and service areas are isolated from the residents in order to minimize disturbances and to conduct administrative functions without bothering the residents. 3 House of Hope Alzheimer s Assisted Living Facility The topography allows for 6 households, each with 10 or 11 residents, on two floors maintaining grade access for all. The upper level contains four, and the lower level two, resident households containing a serving/resident kitchen as well as a bather, dining, living and family spaces. There is an audio room in each household that allows residents to watch television with a high volume yet still not disturb other residents. There is a staff preparation area outside each household where staff work can be accomplished without affecting the residents or the staff concentration. A desk within the household allows staff to work directly with residents or family members. Each household has a laundry area where residents can do personal laundry. The adult day care and every household opens onto a courtyard with covered and open areas for the residents to enjoy. The sidewalk cafe and family dining room are provided with a protected deck. The community space on the upper level brings the households and campus together and offers a meeting room/chapel, a beauty salon/barber shop, a library, a sidewalk cafe, and a private family dining room. This is where the residents and the community come together for celebrations and everyday social interaction. Directly below this community space is an adult day care that accommodates up to 15 clients and whose program will integrate with the remainder of the new residence households. The true repositioning came with a culture change approach to care provision from an institutional model to a resident oriented and household model. The success of this project can be attributed to a full team collaboration with Crepidoma Consulting handling the master planning, conceptual design and culture change consulting in partnership with the financial, marketing and development consultants. A very intricate phased plan allowed for no disruption of housing availability for current residents and minimized revenue disruption while attracting new residents to the community. The new model integrates built revisions along with new construction, both with an eye to sustainability and an improvement of indoor environmental quality. The design also maximizes the utilization of exterior spaces and the efficiency of staff. 5 Legacy Gardens Alzheimer s Assisted Living Facility Conceived as a compliment to an existing health-care campus developed in conjunction with the University of Iowa, this assisted living residence serves early stage Alzheimer s residents who are unable to live alone or do not have day-to-day familial care provision. Divided into two distinct and separate households, the environment serves forty residents with an emphasis on the inwardly focused community spaces as well as nooks near resident rooms that provide sensory cues to residents past. The community spaces provide ease of access for the residents but a visual delineation from the circulation and personal areas with residential iconography. Iowa City, Iowa Complete: ,000 sq. ft. 6 Elm Crest Retirement Community Continuing Care Retirement Community This not-for-profit client realized the existing 1948 facility would not adequately service the needs of the community s elderly into the next century. They also realized that a new facility would not be financially feasible if, during construction, there was any loss of revenue. We were retained to provide a long range plan for the facility campus. New construction was designed which could be completed prior to demolition of the older structure so as not to interrupt the operations of the existing facility. The new facility included three independent living apartments, a 40 unit assisted living wing, a community center, 16 bed specialized dementia unit and a replacement wing administrative offices and expanded dining. 7 Gables at Breeze Park Nursing Care Recognizing the need to have a facility that was market competitive and resident friendly, Lutheran Senior Services called upon Crepidoma Consulting s national design expertise and the local capabilities of Grubbs & Associates for their skilled nursing addition to the Breeze Park retirement campus. The 67 resident long-term care facility was divided into four distinct neighborhoods each with ready visual and physical access to a secured exterior courtyard. The community comes together in spaces where the neighborhoods intersect, providing country kitchen and dining facilities for each neighborhood. The entire campus can take advantage of expanded community space developed into a chapel, ice cream parlor and convenience store, wellness center and family room. 8 Salem Lutheran Home Nursing Care/Alzheimer s Nursing After having completed an extensive long range plan for this health care provider, Mr. Anderzhon was commissioned to provide 60 replacement nursing beds for the aging 1948 structure. Because the rural facility could not withstand loss of revenue during construction, the new facility had to be built around the older building. When the residents were relocated, the demolition of the older structure took place, and finally, completion of the new construction was accomplished. While the design did not add new long term healthcare beds, it did include a high percentage of private rooms, a 16 bed specialized dementia unit, hospice rooms and rooms to accommodate overnight visitors. 9 Nebraska Masonic Home Alzheimer s Nursing Design Work Having experienced a significant increase in the number of residents with dementia, The Nebraska Masonic Home came to us for the conversion of a wing to a secured specialized care unit. The administration and staff of the home were determined to improve the quality of life of the residents and to use the built environment to help mitigate the symptoms of dementia. This resulted in a true team approach to the design of a complete unit, and not simply the placement of secure doors at the entry of the wing. A small addition was added for appropriate space for an interior wandering path, ADL kitchen and activity area and access to a secure outdoor courtyard. Within the courtyard, areas were provided for gardening and respites from chronic wandering. The common spaces were designed for resident use with minimal intrusion from staff. The resident rooms were converted from semi-private to fully private rooms, complete with daily wardrobe display areas, lockable closets and memory link shelving.
