We offer a wide range of senior care and living options, including: Skilled Nursing Care Rehabilitation Memory Care Assisted Living Independent Living Condominiums You ll find the housing and healthcare resources you need including our new, state-of-the-art Wasen Rehabilitation Center. Brethren Care Village is conveniently located on 28 beautiful wooded acres near Samaritan Hospital, Ashland University, hotels, dining, and shopping. You ll come home to all the comforts and privacy of a delightful Bloomfield Condo nestled in a picturesque neighborhood. The single floor design with two bedrooms and two baths is popular with independent living residents. It s perfect for a couple, yet not too big for a single person. All the major appliances provided in your kitchen and laundry room are included. Make use of the many amenities, activities, social events and outings, or you can snuggle up at home with a movie, a good book or your favorite crossword puzzle. There are no community fees, only a simple monthly service fee.

4 Assisted Living Belmont Tower, Brookwood Place, Bradford Houses A Friendly Family Atmosphere; Caring and Understanding Staff Belmont Tower: Elegant setting and excellent staff, a charming, residential atmosphere designed for those who need some assistance with daily activities (dressing, bathing, medication management). Residents have a choice of apartment sizes with rates based on the size of apartment and level of care. Residents at Belmont Tower have the opportunity to select a level of care uniquely individualized to meet personal needs and desires, and a choice of apartment floor plans. We look for ways to encourage residents to be active members of our community, including friendly interactions with many staff members, visitors and other residents. We have an array of stimulating activities to inspire personal interests and share life stories. Brookwood Place: A charming, residential setting designed for those who need assistance with daily activities (dressing, bathing, medication management). The building is also equipped to provide a safe and secure environment for residents who may be prone to wandering. You can feel the friendly family atmosphere as soon as you arrive. Brookwood Place offers a full and active life enrichment calendar, comfortable gathering areas, a library and reading room, and other welcoming areas for the pleasure of the residents. Residents on our campus always have priority admission to the nursing center or assisted living.

5 Bradford Houses: Unique assisted living specializing in Memory Care. One of the unique features of the secure Bradford Houses is that they are specifically designed to provide assistance and support to older adults with memory impairment challenges, who can no longer live safely on their own but who do not need all of the medical treatment and services offered in our Skilled Nursing Center. Each Bradford House is designed for five residents allowing each resident to have his/her own bedroom with personal belongings and decor. The house size and number of residents keeps the atmosphere as close to just like home as possible. Our unique setting is the only one of its kind in North Central Ohio. Residents and staff can dine together at the family table. The living room and den offer a place for family and friends to visit. Each resident is encouraged to participate in the everyday functions of their home. Safety, security and structure encourage each resident s involvement in the day s activities; promotes personal relationships, and communicates the importance of respect and dignity. The Bradford House Club program consists of two sessions that are two hours long. There is a session in the morning and one in the afternoon. Because the attention span of each resident is different, the activities during each session last approximately 20 minutes. Frequently changing activities keeps participants more attentive, focused, and involved. Families have been pleased with our personal approach and the interaction available for their loved ones. Residents on our campus always have priority admission to the nursing center or assisted living.

6 Nursing Center Providing Skilled Nursing Focused on Individual Care and Respect Our Nursing Center provides personal nursing care, monitoring and assistance with activities of daily living that many people require as they face health challenges. The Nursing Center at Brethren Care Village is open to Village residents as well as the general public, certified by Medicare and a Medicaid provider. Our professional and compassionate staff provides 24-hour care and offers a full range of social services, and therapies. Post Acute Care Post Acute Care teams offer nursing care 24/7 following discharge from a hospital after surgery, a major illness, or when an individual cannot either safely return home, or function adequately in independent or assisted living. Personalized services administered by our professional and compassionate staff may include: Regular and advanced IV therapies Pain management Advanced wound care Diabetic management Pulmonary care Palliative or hospice care Nutrition and psychological counseling Wasen Rehabilitation Center The state-of-the-art Wasen Rehabilitation Center offers a supervised program to help improve the participant s health and well-being. The goal is to return to home to an active and independent lifestyle as soon as possible.
