2 HOW TO FILL IN THE PRE-ENROLLMENT APPLICATIONS Applications for the Corso di Perfezionamento Muscle-fascial anatomical dissections must be compiled within of the 13 th November 2014, by connecting to the website After the expiry date the connection will be terminated and it will no longer be possible to compile the application form. Instructions for compilation: Phase 1 1) Before filling in the application, log into the system following the instructions on the Uniweb homepage. a) Click on Registrazione (Sign in) (if you have never been enrolled in the University of Padua ) or b) Click on Gestione username e password (Manage username and password) (if you do not remember them or if they are expired). In case of difficulty with the username and password, you are advised to use the Modulo Uniweb Post Lauream username/password which you will find in the website under Modulistica Corsi di Perfezionamento. 2) After logging in with your credentials (if your career is displayed, select to continue), click on Didattica and select Preimmatricolazione ad accesso programmato. 3) Click low down on Preimmatricolazione corsi ad accesso programmato and select the type of course: Corsi di perfezionamento. 4) Select the course. 5) Fill in the application form. 6) Print the application by clicking on Stampa riepilogo preimmatricolazione (the form for the payment of euro as pre-enrollment fee will be printed with it). The euro pre-enrollment fee (including euro virtually paid stamp duty) is nonrefundable, regardless of the outcome of the selection process or of whether the course is started or not. The fee indicated on the payment slip may be paid in one of the following ways: - At Cassa di Risparmio del Veneto SpA - Gruppo Intesa Sanpaolo branches (this is the University s bank), and other banks on Italian soil; - Through the Home Banking service for customers of Cassa di Risparmio del Veneto SpA - Gruppo Intesa Sanpaolo, Gruppo Intesa Sanpaolo banks and through other credit institutions where this service operates; - At all ATM machines of Cassa di Risparmio del Veneto SpA - Gruppo Intesa Sanpaolo banks, with a cashcard issued by any credit institution and at the ATM machines of other banks that provide a similar service(to pay through an ATM, the MAV identification code shown on the payment slip must be entered); - Through the Home Banking service and ATM machines for Superflash prepayment card holders (to pay through an ATM, the MAV identification code shown on the payment slip must also be entered in this case). In case that it is not possible to pay with MAV you can use the form for the bank transfer (enclosed 1). In case of difficulty arising in compiling the application form, a telephone helpdesk will be operational on / from to from Monday to Friday. There will be a computer workstation available at the Servizio Formazione Post Lauream, Via Ugo Bassi 1,35131 Padova, for those who require it. It will be open Monday to Friday from to 2

Phase 2 The pre-enrollment application, signed in the original, must reach the office (by post or hand delivered): Università degli Studi di Padova Servizio Formazione Post Lauream Via VIII Febbraio, Padova Italy within and not beyond on the expiry date 13 th November The post office stamp will not be considered valid as proof of compliance with the expiry dates. Pre-enrollment application hand delivered: Post Lauream office hours Monday to Friday from to 12.30; Tuesdays also from to and Thursdays continuously from to Applications may also be sent by certified (PEC posta elettronica certificata) within and not beyond on the expiry day 13 th November 2014, to the certified address amministrazione.centrale@pec.unipd.it, according to the following methods: With a digital signature appended on it and on all the documents that require an autograph signature in traditional form; By sending a copy of the application with an autograph signature together with the signatory s identity document; similarly, signed copies of all the documents that require an autograph signature in traditional form must be transmitted. The copies of these documents may be scanned; By means of the candidate s personal certified for messages from and to the authorities (CEC-PAC comunicazione elettronica certificata tra pubblica amministrazione e cittadino) together with the required documents. To send documents on screen, static formats that are not directly editable must be used, without macroinstructions or executable codes and preferably in pdf. Finally, it is not necessary to call the University offices to inquire whether the application has arrived, nor is it necessary to send any hard copy message because the acknowledgement is sent automatically by the certified operator. Failure to comply with the time limit specified for the presentation of the pre-enrollment application, whether sent online or in hard copy, entails exclusion from the selection procedure. The following must be sent with the application: 1) CV (use the format that you can find at - PDF2); 2) Any documents required for admission to the course; 3) A photocopy of the receipt for the payment of the euro pre-enrollment fee; 4) A photocopy of a valid personal identity document; 5) A photocopy of a valid passport if the applicant is a non-eu citizen. 3

4 When enrolling in more than one course, the application form must be sent with the abovementioned documents and the Euro enrollment fee. Applications must contain a declaration in lieu of the certification of the educational qualification required for admission to the course. The University of Padua reserves the right to make the appropriate inquiries to the institution that granted the qualification. Documents, if any, may be produced in the original or in a copy that is declared to be a true copy of the original (Modulo dichiarazione sostitutiva autocertificazione titolo, available in the website Pursuant to Article 40 of Presidential Decree 445/2000, as amended by Article 15 of Law 183/2011, in dealing with public authority offices and operators of public services, certificates and self-declared certificates are always replaced by the declarations referred to in Articles 46 and 47 of the Decree. Citizens of non-eu States, duly authorized to stay in Italy, may only use the declarations referred to in Articles 46 and 47 of Presidential Decree 445/2000 for status, personal qualities and circumstances that can be certified or attested by Italian public bodies (see Article 3, paragraphs 2, 3 and 4, of Presidential Decree 445/2000). The University Administration declines all responsibility for any communications that go astray resulting from incorrectly stated addresses, places of residence or contact addresses by the applicant, delay in changing such particulars or resulting from any postal, screen-based technology or telegraphic problems for which the University Administration is not to blame. Within six months after the conclusion of the competition and 120 days after the publication of the admission list, candidates must take steps to recover any publications (if submitted in the original) from the office to which they have been sent. After this time limit has expired, the University Administration will not be answerable for the preservation of these documents. Dyslexic candidates, pursuant to Law 170 of 8 October 2010, and disabled candidates, pursuant to Law 104of 5 February 1992 and Law 17 of 28 January 1999, may ask to use aids and/or to be given personalized tests in order to take the admission tests, according to their disabilities. Requests of this kind must be specified during the compilation of the pre-enrollment application in Uniweb; in order to confirm the request, the form has to be printed and delivered to the Servizio Disabilità dell Università degli Studi di Padova at Via Portello, 23, Padova. Candidates that hold an educational qualification obtained abroad should refer to the instructions in Article 6, below, of this Selection Notice. SELECTION RESULTS, ENROLLMENT OF THE WINNERS, REPLACEMENTS The list will be available to the public from the 1 st December 2014 in the following ways: - Affixed to the notice board of the Servizio Formazione Post Lauream, Via Ugo Bassi 1,35131 Padova; - Published in the Padua University website: The publication of the list is valid as an official notification to those concerned. Enrollment of EU and non-eu candidates with qualifications obtained in Italy. In order to complete their enrollment, candidates must hand in the documents listed below, within and not beyond the date 19 th December 2014, to the Servizio Formazione Post Lauream office at Via Ugo Bassi 1, Padova (office hours Monday to Friday from to 12.30; Tuesdays also from to16.30 and Thursdays continuously from to 15.00), or send them to: Università degli Studi di Padova,Servizio Formazione Post Lauream, Via VIII febbraio 1848, 2, Padova: - A photocopy of the receipt for the payment of the enrollment fee, to be made by bank transfer (using Annex 1) to Università degli Studi di Padova, IBAN code IT81V , Cassa di Risparmio del Veneto SpA - Gruppo Intesa Sanpaolo - Ufficio di Tesoreria. 5 to be paid is 740,50. The enrollment fee includes the premium for accident insurance, third party liability and stamp duty at the current rate. Transfers sent by home banking will only be considered valid for enrollment if the Servizio Formazione Post Lauream receives the receipt for the bank transfer complete with transfer code; - Female candidates only: a declaration to the effect that the person concerned has read the Regolamento per la frequenza dei laboratori da parte delle studentesse in stato di gravidanza ( Regulations for expectant mother students attending laboratory sessions ), in Annex B. Copies of bank transfer receipts may also be sent by fax to The candidate, however, must in any case see that he or she lets the Servizio Formazione Post Lauream have the remaining documents necessary in order to complete enrollment. Those who have not completed their enrollment within the required date will be considered to have given up their application. Candidates that hold an educational qualification obtained abroad should refer to the instructions below, of this guidance. Any places that become available after express or tacit waiver or the loss of the right to enrollment, will be allocated to the persons in the following places on the admissions list in order of ranking. For this purpose, those entitled to a place in the list will be contacted orally by the Servizio Formazione Post Lauream office, which will give them all the information required regarding the formalities for completing their enrollment within the date 9 th January Should other places remain, this office will continue contacting applicants until all the free places have been taken. EDUCATIONAL QUALIFICATIONS OBTAINED ABROAD AND REGULATIONS FOR FOREIGN CITIZENS Italian and foreign citizens holding an educational qualification from a University abroad may submit an application for selection for admission to the 2014/2015 course; their qualification must be equivalent to one of the degrees required for entry to the chosen course in accordance with international agreements and conventions. The holders of other qualifications obtained abroad must apply to have their qualification recognized as equivalent. The Course Regulatory Board (Consiglio di Corso di Perfezionamento) will decide whether to recognize such qualifications solely for the purposes of entry to the course. Enrollment is subject to the academic recognition of the qualification and passing the selection procedure. Pre-enrollment of candidates that hold educational qualifications obtained abroad. Applicants holding an educational qualification obtained abroad must arrange for the preenrollment form to reach the Servizio Formativo Post Lauream within the time limits specified. The following must be sent with the application: 1) CV (use the format that you can find at - PDF2); 2) Any qualifications and documents required for admission to the Master course; 3) A copy of the degree diploma with declaration of value if available; 4) A certificate stating examinations taken and marks obtained; 5) All other documents deemed helpful for the evaluation of the admissibility of the qualification held (diploma supplement, European format CV, etc.); 6) Translation of the documents into Italian or English (if not already drawn up in a widely diffused language); 5

6 7) A photocopy of the receipt for the payment of the euro pre-enrollment fee; 8) A photocopy of a valid personal identity document; 9) A photocopy of a valid passport if the applicant is a non-eu citizen. It is suggested that foreign students that are not citizens of EU countries consult the contents of Ministry memorandum 602 of 18 May 2011, available on website and, specifically, with regard to the documents necessary for enrollment and obtaining a stay permit. 7
