3 Section 1. Plumbing & Gasfitting requirements: Plumbing & Gasfitting Services 1. The service will be contracted from 1 st January 2012 to 30 th June 2013, subject to satisfactory annual performance. 2. The service contract will include all regular maintenance and incidental plumbing services including out of hours and emergency callouts. The service contract will not necessarily include new capital developments. 3. A review of the service will take place in June of each year to ensure that both parties are satisfied with the service provision. 4. Services are calculated on the rate advised on the tender dated / / (tender application). 5. The rate includes wage recoupment, travel expenses, insurance, equipment, equipment maintenance and administration costs. 5. Cooinda Village will be invoiced monthly. 6. Three month notice of withdrawal or cessation of service required. 7. All work undertaken is to be itemised on account paperwork to allow for tracking of work undertaken at Cooinda Village.

4 Section 2 Plumbing & Gas fitting Services The contractor to carry out such works and services as referred to in the schedules and as requested by Cooinda Village from time to time in a conscientious, expeditious and professional manner. Cooinda Village requirements: 1. The successful tender to be registered business with an ACN or ABN (Copy to be held by Cooinda Village.) 2 The successful tender to hold current professional and public liability cover. (Copy of cover details to be held by Cooinda Village.) 3 The successful tender to have current Workcover insurance. (Copy of cover details to be held by Cooinda Village.) 4 Service technicians and tradesmen to hold current relevant qualification in their field of work. (Copy of qualification details to be held by Cooinda Village.) 5 A current (dated within the last 3 years) police check is required by all contractors who work within the Cooinda Village site. (Copy to be held by Cooinda Village.) 6 New service technicians and tradesman allocated jobs at Cooinda Village to undergo an orientation/induction program. 7 All residents are to be treated in a courteous and respectful manner 8 The successful tender will comply with the Occupational Health & Safety guidelines for contractors and service personnel as set out in the Contractor Induction and Safety Program Orientation Manual. 9 The successful tender is to obtain work permit from administration if works to be done are likely to interfere with the fire detection system (smoke detectors & Sprinklers.) 10 The successful tender is to advise the CEO/DON if there is a need to switch off the water or gas supply to any part of the facility. 11 Contractors working at Cooinda Village are required to sign in and wear personal identification Tag. Temporary identification tags are available at reception. 12 High standard of privacy, dignity and confidentially toward Residents must be maintained at all times.

5 SCHEDULE 1 Maintenance of Thermostatic Mixing Valves. Twice Annually (April & October) Note: TMV maintenance logbook and manual located in administration office in which work is to be documented. Service includes: Complete dismantling of the valve so that it can be inspected and cleaned Replacement of any faulty parts Conduct fail-safe test several times on each valve. Recording details in maintenance log book Signing off logbook when service completed. Recommended temperature settings are between 40.5 degrees to 43.5 degrees C.
