DAKOTA COUNTY - Back to School Bash, Inver Grove Heights Location: Church of St. Patrick, nd Street East, Inver Grove Heights Eligibility: Pre- K through 12th grade students needing school supplies residing in Inver Grove Heights and Dakota County Intake: Walk-in; no pre-registration Distribution: Saturday, August 20, am - 1 pm - Hastings Family Services, Hastings Eligibility: K - 12 students living in the boundaries of Hastings School District #200; May attend a school outside of the district if they reside in the district Registration: Begins August 15, Register by phone at Intake: Parent or guardian must have a photo ID and a piece of mail postmarked within the last 14 days or the most current utility bill with an address in District #200. Parent must have a knowledge of household income and expenses. Each child must have proof of enrollment in school, such as a piece of mail from the school, a current year print-off from school view or a document from the school with the child's name and address. Distribution: August and for registered participants -Ready, Set, Achieve at Lakeville Public Schools, Lakeville Location: Crystal Lake Education Center, Ipava Avenue West, Lakeville Eligibility: Pre-K - 12 students registered at Lakeville Public Schools (ISD 194) Intake: Walk-in; no pre-registration Distribution: August 24, 2016 from 1 pm - 7 pm; Students must be registered at their school prior to the distribution date. 2 HENNEPIN COUNTY - CROSS Location: Weinand Circle, Rogers, MN Eligibility: Current CROSS clients only; CROSS serves residents of the Osseo- Maple Grove area, western Champlin, Rogers, Dayton and Corcoran Registration: No specific registration; see below Distribution: School supplies are distributed beginning August 1, 2016 and in to September as supplies last during the client's month food shelf appointment; call with questions - Interfaith Outreach & Community Partners (IOCP), Plymouth Location: 1065 County Road 101 North, Plymouth Eligibility: Current IOCP clients with children in grades K Individuals living in the service area may become IOCP clients. Agency serves those living in Wayzata, western Plymouth, Long Lake, Orono, Hamel, Minnetonka Beach, Medina and Medicine Lake. Registration: Wednesdays August 3, 10, 17 and 24, 2016, between the hours of 9 am and 8 pm, by calling OR on the day of distribution, although supplies not guaranteed for day of distribution registrants Distribution: Tuesday, August 30, 2016, 10 am - 6 pm -Luxton Park Summer Celebration, Minneapolis, MN Location: 112 Williams Avenue SE, Minneapolis Eligibility: Residents of the Prospect Park neighborhood ONLY this year- most of zip code with an emphasis on Glenwood Townhomes public housing, with a child enrolled in grades K - 12 Registration: Pick up a flyer from Luxton Park, 112 Williams Avenue SE, Minneapolis, MN between 12 pm - 8 pm Monday - Friday and return by the end of July Distribution: Registered participants only on Tuesday, August 9, 2016; Registration also provides participants with food voucher to use at the community celebration and concert taking place during pick-up - PROP, Eden Prairie Location: Martin Drive, Eden Prairie Eligibility: K - 12 students residing in Eden Prairie or Chanhassen in need of school supplies Registration: No pre-registration required; Do not need to be a client for other services 2

3 Intake: Walk-In Tuesday, July 5 - Friday, August 12, 2016, Monday - Friday 9:30 am - 12 pm, and Wednesdays 4 pm - 6 pm; Must have photo ID for head of household, proof of residency for head of household in form of a utility bill, county statement, medical bill, mortgage statement or lease dated in the last 30 days, and verification for each child in the form of a birth certificate, medical card, social security card or school record -ResourceWest, Hopkins Location: st Street South, Hopkins, MN Eligibility: K - 12 students attending schools in the Hopkins and Minnetonka public school districts who have an income qualifying them for the Free or Reduced School Lunch Program; the family does not have to utilize the program Registration: Must register August 8-12, 2016, between 10 am - Noon either in person or by calling Distribution: Registrants will receive an appointment between August 22-27, 2016; Families must bring photo ID for adults, ID for each child (school ID, medical insurance and social security cards are accepted), and a utility bill dated within the last 30 days -STEP, St. Louis Park Location: 6812 West Lake Street, St. Louis Park, MN Eligibility: K - 12 students residing in St. Louis Park; Students may attend school outside of the district Registration: In person during regular food shelf appointment or on a walk-in basis through the end of August on the following days and times: Monday and Wednesday 8 am - 11:30 am and 12:30 pm - 3:30 pm; Tuesday 12:30 pm - 3:30 pm and 5 pm - 7:30 pm; Thursday 8 am - 11 am; Friday 8 am - 11:30 am Distribution: At food shelf appointment or during walk-in; Parents need to bring photo ID, current utility bill or apartment lease, and official original documentation that contains the child's birth date unless they received school supplies from STEP in Callers may contact Karen at ext. 12 if they have questions. -VEAP, Bloomington Location: 9600 Aldrich Avenue South, Bloomington Eligibility: K - 12 students residing in VEAP service area of Bloomington, Richfield or South Minneapolis (south of 50th Street and west of Cedar Avenue); financial need (self-reported). Registration: Must call VEAP at before August 15, 2016 to register between 8 am - 5 pm Monday - Friday Intake: Parents must bring a photo ID and a piece of business or utility mail dated within the last 60 days. Children will need an original ID showing a date of birth if one has not been provided to VEAP within the last year; acceptable ID include 3

Distribution: Each family will receive a distribution date and time when they call to register - WeCAN, Mound Location: 5213 Shoreline Drive, Mound Eligibility: K - 12 students living in Greenfield, Independence, Loretto, Maple Plain, Minnetonka Beach, Minnetrista, Mound, Western Orono, Rockford, Spring Park, St. Bonifacius and Tonka Bay with self-reported financial need Registration: No pre-registration; registration and distribution are on the same day Intake: Adults must provide a photo ID and proof of address with a piece of business mail or utility bill from the last 30 days; Must also provide an ID for each child receiving supplies, such as a school ID, medical card, report card or birth certificate. 5 RAMSEY COUNTY - Family Values for Life Back to School Celebration, St. Paul Description: Food, family fun, haircuts, community resource booths, games and school supplies Location: Bruce Vento Elementary School, 409 Case Avenue, St. Paul Eligibility: Residents of St. Paul with children in grades K - 12 Registration: No pre-registration; registration and distribution are on the same day Intake: Walk - in Wednesday, August 31, 2016, 4:30 pm - 7:30 pm; Adult must bring photo ID; Child must be present SCOTT COUNTY Community Action Partnership of Scott, Carver & Dakota Counties - backpack distribution Location: Shakopee office 712 Canterbury Road S, Shakopee, MN Eligibility: Must live in Scott County and have children in kindergarten 12 th grade Registration: Sign up at the Shakopee office and git a date, time and locatin to pick up school supplies. Distribution: August 9 th & 10 th by appointment. Must bring photo ID; proof of residence such as a recent piece of mail; proof of each child s age. WASHINGTON COUNTY - Friends in Need Food Shelf, 535 4th Street, St. Paul Park Eligibility: Students in grades K - 12 who reside in Cottage Grove, Grey Cloud Island, Newport and Saint Paul Park Registration: There is no pre-registration; registration and distribution are on the same day Intake: Head of household must bring photo ID and utility bill Distribution: Thursday, August 18, 2016, 4 pm - 6:30 pm OTHER Metro Area Salvation Army The Salvation Army will be distributing school supplies through their local offices.
