2 Ola Middle School Cheerleading Tryouts The dates and times for the OMS Cheer Clinic for all cheerleading candidates are as follows: WednesdayApril 30 th 4:00pm 5:30pm Thursday May 1 st 4:00pm 5:30pm Friday May 2 nd 4:00pm 5:30pm Monday May 5 th 4:00pm 5:30pm Tuesday May 6 th FINAL CUTS May leave after you tryout! All paperwork must be turned by Friday, April 25 th in order to participate. All candidates should have a ride home NO LATER than 5:30pm everyday of tryout week. Late pickups could result in dismissal from tryouts! Tryouts will be judged by OMS coaching staff and outside judges. Cuts will NOT be posted until everyone has cleared the premises! The cuts will be made on Tuesday, May 6 th. All candidates making a squad will be listed on the door after tryouts. Do not expect a list to be up before 8:00pm!!! Lists will be posted on the doors that lead to the commons area of the building. This tryout is for: 7 th and 8 th grade Competition Squad 7 th and 8 th grade Football Squad 7 th and 8 th Basketball Squad The competition squad will be determined based on the highest scores. For example: the top 16 scores regardless of grade level will be chosen for the Competition Squad (will cheer some home football games). If you have no desire to cheer for teams and only want to be considered for the competition squad, please mark competition only on your application. The football squad will be determined based on strengths in jumps, tumbling, voice, and stunting. If you are interested in playing basketball at OMS, you may be a football or competition cheerleader and play basketball. If your desire is to only cheer during football season, please mark FOOTBALL ONLY on your application. The basketball squad will be determined based on strengths in voice, tight motions, and dance ability. If you are interested in playing softball at OMS, you may play softball and be a basketball cheerleader. If your desire is to only cheer during basketball season, please mark BASKETBALL ONLY on your application. If candidates are interested in cheering during both FOOTBALL and BASKETBALL seasons, please mark Football/Basketball on your application. If you just want to be a cheerleader on ANY SQUAD at OMS, please mark all three: Competition, Football, and Basketball.

3 Please note: If you only mark the competition squad and your score is not high enough to make the competition squad, you will not be on a squad period. Each candidate must: Requirements for Practice and Tryouts 1) Meet eligibility requirements according to state standard. (Must pass 5 out of 6 classes the previous semester). 2) Pass a physical examination. (Attached form) 3) Submit proof of insurance. (Attached form) 4) Parental Permission form/medical release form for tryouts. (Attached form) 5) Cheerleading Application (Attached form) 6) Cost Agreement (Attached Form) 7) Teacher Recommendation Form (Attached) 8) Complete Alternate Agreement if trying out for Competition Squad (Attached) 9) Attend ALL practices in order to tryout. 10)Have a way to be picked up everyday of tryout week by 5:30pm. 11) NO VISTORS WILL BE ALLOWED DURING THE PRACTICE SESSIONS OR TRYOUTS. ALL CHEERLEADING PRACTICES AND TRYOUTS ARE CLOSED Dress: 1) Cheerleading/Athletic Shorts, T shirt (NO SPAGEHTTI STRAPS), athletic shoes, and socks. 2) HAIR UP! 3) NO JEWELRY! 4) NO GUM! 5) NO cell phone use during practice hours! 6) NO false nails or long nails! (Safety purposes) The following conduct is strictly prohibited while attending all phases of tryouts: 1) Smoking 2) use of alcohol or drugs 3) use of profanity 4) public display of affection to the opposite sex 5) unsportsmanlike conduct 6) undisciplined behavior 7) Absences from school and tryouts

4 Demands of Cheerleading: Cheerleading is a demanding sport. An individual must be prepared to devote time in the summer, after school and on weekends for practices, clinics, special projects and games. All squads will be required to attend a summer camp that will be held at the school. Therefore, it is very important that each candidate for Ola cheerleading realize the amount of time and effort that must be put into this sport. A competition choreography camp will be held this summer as well. Dates to follow ASAP! A tentative summer practice schedule is attached. Please realize the amount of dedication and commitment this sport takes. Cheerleaders will be required to follow specific rules of conduct and participation established by the administration and the cheerleading coaches. Failure to follow these stated rules will be grounds for suspension or dismissal from the team. Uniform Fitting: On Wednesday, May 7 th, the new squads will be fitted from 4:00 5:30pm at OMS. On the same night, the parents of the new squads will be required to attend a meeting at 6:00 at OMS in the commons area. At the meeting, a complete set of rules will be provided for each cheerleader chosen. We will discuss the rules and practices. Parents and cheerleaders will be asked to sign a statement stating that they have read and agree to abide by the rules. We are looking forward to continuing the strong cheerleading tradition at Ola Middle School that has been established the past two years.

5 OMS Cheerleading Cost Information Competition Squad (Season begins early August and ends late October not including summer camps and practices) We feel that it is important to notify you of the cost required in cheerleading. Please note that this only an estimate and actual costs may vary slightly. These items are required of all competition cheerleaders. 7 Ola Middle School Alternate Agreement (Only for the Competition Squad Candidates) The competition squad may have at least two alternate positions for competitions. These positions are EXTREMELY important throughout the year and especially during competition season. The only reason alternates are designated is that only 16 members can compete (if deemed necessary, the number of competitors can be dropped lower than 16). During the sports season, (any optional football and basketball games that we choose to cheer at) will hold the same duties, but ALTERNATES WILL BE NAMED AS COMPETITION SEASON GROWS NEARER AND ARE SUBJECT TO CHANGE DEPENDING ON THE STRENGTH OF THE ENTIRE TEAM. Alternates will be chosen based on what is BEST for the squad. The alternates chosen are at the coach s discretion. No one is guaranteed a competition spot. Alternates are still a part of the squad and expected to attend all competition practices and functions. By signing below, you are acknowledging that you understand the policy for alternates, their importance and that it is entirely possible that you may become an alternate. 8 Ola Middle School Cheerleading Cost Agreement The cost of cheerleading will be handled through a payment plan. If your child is selected to be a part of the OMS Cheerleading Squads, you will be responsible for all payments being made on time. This is necessary for the girls to have the equipment needed to perform. The following is a rundown of the proposed payment plan. We have attempted to get these costs as close as possible to the actual cost. You will be notified of any cost changes before 3 rd and final payment. Remember we have scheduled a fundraiser that will help with your individual cost. We hope that this will help in some way. **Alternative payment plans are available for extenuating circumstances. Please see the Head Coach. I/We understand that the cost for cheerleading is required for my child to participate in the Ola Middle School Cheerleading Program. I have read the above estimations and agree to make all above payments before or on the dates due. I/We understand that camp is mandatory for all team members. I give permission for my child to attend camp on the dates specified by the coach(es). 9 Parent/Guardian Permission for Participation and Adherence to OMS Cheerleading Tryout Guidelines, Rules and Consequences I hereby give permission for my child, to participate in OMS Cheerleading tryouts, realizing that such participation involves the potential for injury which is inherent in all sports. I acknowledge that even with the best observance of the rules, injuries are still a possibility. On rare occasions, these injuries can be so severe as to result in total disability, paralysis, or even death. I acknowledge that I have read and understand the warning presented herein. I agree not to hold the school or anyone acting in its behalf responsible for any injury occurring to my child in the proper course of such athletic activities.
