2 THE RED CROSS WAR MEMORIAL CHILDREN S HOSPITAL The Red Cross Hospital is principally tasked with providing care to the children of Cape Town but has both a regional and national referral base. The Department of Paediatric Surgery is a busy surgical unit that sees an outstanding array of surgical pathology. It is staffed by 4 general surgical consultants, 1 traumatologist, 1 urologist and 2 Emiratis Professors. Trainees include 2 Paediatric Surgery trainee registrars, 4 General Surgery registrars rotating on a 3 monthly basis and 5 Surgical Interns. In addition, there are 2 to 3 super-numery registrars in the department at any one time. Infants and older children are admitted to a 28 bedded surgical ward which is shared with urology. There is a separate 28 bedded surgical speciality ward to accommodate patients from other surgical disciplines, a day ward for ambulant surgical patients and a 22 bed burns unit. Surgical intensive care patients and surgical neonates are accommodated in a combined medical and surgical 18 bed ICU. Groote Schuur Hospital, Mowbray Maternity and Somerset Hospital in Cape Town accommodate the neonatal units. While all operating is performed at Red Cross, Registrars are required to attend to patients in these other units as is required to meet service needs. In addition to neonatal surgery, paediatric gastroenterology, transplant surgery, hepato-billiary surgery, trauma and oncology, a broad range of general paediatric endoscopic surgery is undertaken by the unit. Outpatient facilities are excellent and involve 2 large clinics per week for general surgical cases and 2 burns clinics per week. The operating theatre complex has 8 theatres; trauma unit has 2 theatres and the outpatient service 3 theatres. JOB PURPOSE The APFP Senior Registrar post in Paediatric Surgery in Red Cross Children s Hospital, Cape Town is a two year training program that will form the basis for the degree of MPhil in Paediatric Surgery from the University of Cape Town. This is an academic qualification and will not be sufficient for registration with the South African College of Paediatric Surgeons. Your local medical council will determine the weight it holds with respect to your training and qualifications in your home country. Responsibilities will include a service commitment to the general and specialist paediatric surgery service, a service that will continue after hours when on call. The post includes voluntary training opportunities in a number of paediatric surgical sub-specialities including urology, ICU, burns, trauma and surgical skills training. This post is suitable for an applicant who wishes to gain broad general and specialist paediatric surgical experience with a view to a career in paediatric surgery. EXPECTATION OF THE DEPARTMENT IN REGARD TO APPLICANTS Experience The APFP registrar will be expected to carry out clinical duties at the level of an advanced trainee. The appointee will be expected to hold MRCS or equivalent, and to have some previous experience in paediatric surgery, preferably at Senior Registrar level. It will be desirable for candidates to have undergone training in paediatric resuscitation (APLS/PALS), have some experience in teaching and clinical research. It will be expected that the appointee will be able to demonstrate understanding of the principles of clinical governance, and he/she should be able to show evidence of familiarity with clinical audit. Clinical service

3 Paediatric Surgical Senior Registrars are expected to oversee the ward management of patients for which they carry responsibility, including supervision of interns. They are responsible for ensuring that discharge information about the patient is written within an appropriate timescale. They are expected to attend ward rounds, academic and operational meetings, clinics and theatre lists as time-tabled, having prepared appropriately. Registrars perform calls on a maximum of a 1:3 call rota, with 1:3 weekends on duty. This normally extends to a 1:4 or 1:5 call and weekend rota if the unit is fully staffed. Normal hours extend from 07:00 am until approximately 18:00, after which the on call registrar and intern continue to cover the unit. Post call is a normal working day, only ending early if the unit is adequately covered and outstanding issues are handed over to the on call team. Registrars do calls from home and are covered by a consultant when on call. Teaching The Senior Registrar is expected to participate in the undergraduate teaching programme for University Of Cape Town medical students rotating through Paediatric Surgery. The Senior Registrar will be actively involved in the post-graduate surgical and paediatric teaching programme. Research/Audit/Study It is expected that the trainee will involve themselves in new clinical research and it is desirable that they produce at least one peer reviewed publication by the end of their two years with the department. The Department of Paediatric Surgery holds regular morbidity and mortality meetings and an on-going programme of regular clinical audit. The Senior Registrar will be required to attend these meetings and play a full role in the process of clinical audit. Facilities for study are excellent. The main University Medical School Library is situated in the Medical School at Groote Schuur Hospital and is readily accessible. In addition there is a small Medical library on the Red Cross Hospital site in the Institute of Child Health Building. Active participation in registrar tutorial groups is expected. These groups focus on preparing the South African Trainees for their final exams and are led by Emiratis Professor H. Rode. LEAVE: Leave is granted only when acceptable to the department and if staffing allows. Twenty two days (excluding weekends and public holidays) will be granted for an annual contract, and 11 days for a 6 month appointment. ASSESSMENT: Registrars visiting the department will be assessed every 3 months by consultants in the department. The assessment will take place in the form of continuing day to day observation of the candidates progress and skills. SPECIAL WORKING CONDITIONS The appointee will be expected to travel between the 4 hospital sites whenever the clinical duty requires it. The appointee will have to use his/her own car as there is limited public transport facility. REQUIREMENTS: All Registrars working in South Africa are required to get full registration with the Health Professions Council of South Africa as a supra-numerary trainee in Paediatric Surgery. PERSONAL ISSUES: There may be staff accommodation available at a nominal cost, in the nurse s home. There are 2 apartments with basic furnishings and their own facilities, as well as 4 single rooms, each equipped with bed, wardrobe, Fridge, Microwave, toaster and kettle. These have shared bathroom facilities. These facilities are generally booked up well in advance, so it is advisable to book early. Alternatively you may find your own accommodation. Public transport is poor in South Africa and car hire/purchase is advisable. It is advisable for you to get some form of medical insurance for yourself for the duration of your stay as the state will charge you full private medical rates in the event of illness or injury. Being on a training fellowship is a massive personal, financial and professional undertaking that has huge rewards and many challenges. It is strongly recommended that you make contact with previous APFP fellows in an effort to gain as much insight into the program as possible.
