1 Massachusetts Personal & Home Care Aide State Training Health Career Lattice Project Supporting Personal Care Attendants and Home Care Aides

2 PHCAST to Nurse Aide Bridge This project results from a collaborative relationship between the University of Massachusetts Medical School Commonwealth Medicine, Quinsigamond Community College, and Greenfield Community College. The purpose of this project is to develop a bridge curriculum from the Personal and Home Care Aide State Training (PHCAST) core competency curriculum to the Massachusetts Department of Public Health Nurse Aide Standards.

4 Team-based Approach UMMS Ongoing project leadership Committed to the goals of PHCAST including building career lattices GCC Since inception, has continued to offer PHCAST; including trainings offered through Tripp Established relationships with employers, rapid response with seven partners including home care and CNA Sustainable program QCC Has a long standing commitment to DCW and NA training Consistently offers PHCAST in Spanish and English Committed to providing opportunities for diverse students, including limited English Speakers

5 History of Nurse Aide Training Prior to 1987 few states had training standards for NA s 1987 Fed. Nursing Home Reform law enacted (OBRA 87) New standards to address quality of care issues and promote resident-focused care Major part of the law mandated state-approved training for NA s Minimum 75 hours with minimum of 16 hours clinical State competency evaluation (written/oral and skills) Establishment of state nurse aide registries Ongoing education requirement - minimum of 12 hours per year Massachusetts requirements for NA training remain the same 27 years after OBRA, most states have increased NA training requirements

6 Historical Context In 1987 the Federal Nursing Home Reform Act or Omnibus Budget Reconciliation Act (OBRA) was signed into law. One major provision of the law was that nurse aides have a minimum of 75 hours of initial training including 16 hours of clinical. The training outline that was developed at that time primarily focused on the basic skills and knowledge needed to provide support with activities of daily living (ADLs) to elders in the nursing home setting. Ongoing education requirement - minimum of 12 hours per year

7 Impacting Change The modern health care industry has changed dramatically in the decades since OBRA Changes include: scientific, surgical and technological advances cost-containment, and current economic realities in the U.S.

8 Current Reality The role of the nursing assistant in today s modern healthcare environment is no longer purely a supportive one involving (ADL s). The nursing assistant role has become more varied and complex, and the work environment includes not only nursing homes, but many other practice settings, including hospitals, hospice, rehabilitation facilities, assisted living, home care, adult day health programs, and other community-based settings. As a result of these changes, many states have established additional training requirements for nursing assistants, however Mass has not.

11 PHCAST to Nurse Aide Bridge: Offers opportunity to support the culture shift from task or staff directed to resident directed care that is underway in the LTC community. Patient Centered Care/ resident engagement Increase worker autonomy Team-based model Aging out of facility and into home care Process: Bridge 30-hours* of knowledge and skill from PHCAST into the MA DPH Nurse Aide training Enhance the NA training by building onto PHCAST knowledge and skill gain * While not all modular activities directly align with the MA DPH NA standards; acknowledgement of the entire curriculum should be made

13 Culture Change Movement in LTC Culture change transformation supports the creation of both nursing home environments as well as home- and communitybased settings, wherever older adults and their caregivers express choice and practice self-determination in meaningful ways at every level of daily life Taken from Pioneer Network

14 Project Summary The PHCAST to Nurse Aide Bridge offers: An educational lattice for DCW s to advance to nursing assistant; thereby retaining these valuable workers in the healthcare workforce, and promoting the self-esteem, ongoing professional development, educational advancement, increased wages and benefits for the workers Opportunity to increase the diversity of the health care workforce in that nearly 50% of the direct care workers self-report as minorities with the majority identifying themselves as Hispanic/ Latino. An increased level of quality trained, competent nursing assistants to support complex healthcare industry needs in a variety of practice settings Opportunities to increase consumer outcomes, by providing experienced and well-educated nursing assistants who are prepared to support the transitions, admissions and discharge of residents and patients with increasingly complex medical, surgical, emotional, social, and personal care.
