5 NOTES If using a single form for more than one child, be sure to include the name, entering grade, and birthday for each child on the front page of the registration form. Please clearly indicate the programs selected for each child by writing the child s name or initials on the inside pages A confirmation will be ed. [Check for the date on your confirmation of the scheduled tour for children who would like an opportunity to become familiar with the USM prior to the first day of classes.] Students in the workshop program will pick up their individual class cards on the first day of each session in the Middle School lobby. (Signs will be posted.) They will receive a map and any guidance that may be needed in order to comfortably and easily find their way to their different programs. Check with guides in the Lower School wing for children attending the Discovery Camp and Clubhouse Capers programs. Assistants will be in the halls to help children locate their rooms. Students in grades JK through grade 3 who stay for lunch prior to their afternoon programming bring a lunch that is refrigerated in the morning. Children eat lunch with teachers and assistants. Students in grades 4 and above who stay for lunch may bring a lunch that can be refrigerated in the morning. They may also stop by our Snack Bar. (Our Snack Bar is open 6/20-7/29.) These older students are supervised by assistants who oversee the dining room and snack bar areas. See p. 2 of the program booklet for an explanation of the $8 cancellation fee, $5 change fee, and the timing-of-refunds policies.
