2 Welcome to the 26th Health Care Risk Management Conference, Communication for Patient Safety. Effective communication is essential to improve patient safety and quality of care, and this conference is designed to provide attendees with relevant, actionable information about initiatives and training methods that are being implemented to help improve communication among health care professionals and their patients. In addition to presentations by our faculty of nationally- recognized leaders in patient safety, we have brought together leading practitioners from our member hospitals to share insights from their work on HIC s patient communication and provider communication initiatives to improve patient safety and clinical outcomes at our member hospitals. 5 General Information Conference Goal Effective communication among health care practitioners and between practitioners and patients is essential to improve patient safety and quality of care. This conference will provide participants with an overview of current best practices and contemporary training methods to improve communication in the health care setting. In addition, participants will hear about communication initiatives at HIC member institutions that have resulted in improved patient safety and quality of care. Educational Objectives At the conclusion of this CME activity, the participant should be able to: Identify and discuss ways in which poor communication affects patient safety and quality of care. Identify and discuss ways to communicate effectively with patients to improve patient safety. Understand the role simulation plays in improving communication among health care providers. Accreditation This activity has been planned and implemented in accordance with the Essential Areas and Policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint sponsorship of Maimonides Medical Center, FOJP, and HIC. Maimonides Medical Center is accredited by ACCME to provide continuing medical education for physicians.
