Forms may be left in the mailbox outside of the building if the office is closed. DO NOT SEND REGISTRATION FORMS WITH YOUR MAINTENANCE FEES! IF YOU RENT YOUR UNIT, YOUR LANDLORD MUST COMPLETE THE REVERSE SIDE OF THE REGISTRATION FORM.

2 LA MER 2015 POOL REGISTRATION We are pleased to announce that the La Mer Community Pool is scheduled for weekend openings, beginning Saturday, May 23 th, Sunday, May 24 th and Memorial Day, Monday, May 25 th. Beginning on Monday, June 22 nd and thereafter, the pool will be open daily through Labor Day. Pool hours are 11:00 AM until 8:00 PM on weekdays and 10:00 AM until 7:00 PM on weekends. PLEASE CAREFULLY READ ALL OF THE INSTRUCTIONS FOR REGISTRATION! 1. The Rules and Regulations of the pool will be available at the pool sign-in table. 2. Recreation Registration Form this form must be completed and returned to The Management office in the La Mer Recreation Center Building, 100 Nathan Blvd. Please make sure to check off which community you live in! 3. Relinquishment of Recreation Privileges Form Investor owners must complete this form in order for their tenants to be issued passes. The form is on the back side of the registration form. 4. Guest Pass Purchase Form this form and payment must be used for the purchase of guest passes. The photo ID s issued last season are to be used again this year with year 2015 validation sticker! To obtain validation stickers, you just complete the registration form and return it to the site office. You do not need to return the passes with your registration form. Proof of residency is not required for passes being validated unless specifically requested. Please check Validation at the bottom of the form. For new residents or if you have not previously obtained passes, you need to complete the registration form and include a photo (passport size) for each person listed on your registration form. Please label the back of the photos with names and addresses. The cost for replacement of lost passes is $5.00 for the first pass and $2.00 for each additional one for the first occurrence. For example; if you lost 4 passes, the cost would be a total of $ $6.00, equaling $ Additional replacements after the 1 st occurrence will be at a cost of $5.00 per pass. If you are requesting replacement passes, do not forget to include new photos for each person you are registering. An emergency contact person and telephone number must be listed on your registration form or passes will not be issued.

3 If your child s age puts them into the next age category from last year, they will need new passes. You must include a new photo if you are requesting a child s new badge. There is no charge for these passes. The age categories are as follows: 4 years old and under do not need passes years old pink passes years old yellow passes 18 and over green passes Investor owners who are turning over their owner rights to use of the recreational facilities to their tenants, must complete the Relinquishment of Recreation Privileges Form. Either the owners or the tenants may have access and use but not both. A copy of a current lease cover page must be on file with the Management office for badges to be issued. Guest Pass Form Guest books with 20 coupons are available at a cost of $20.00 per book. Guest fees are: 2 (two) coupons per day, per person, Monday through Friday, and 5 (five) coupons per day, holidays and weekends. One book contains twenty (20) $1.00 coupons and the coupons do not expire so that they can be used from year to year. Passes can be purchased by check or money order made payable to: La Mer Community Association NO CASH WILL BE ACCEPTED! Forms and payment must be sent to: La Mer Community Association, 4-08 Towne Center Drive, North Brunswick, NJ Guest passes are not available from the site office located in the Recreation Building or at the pool. Due to the large amount of requests for additional pool passes for persons or relatives who are in residence in your home part time, or for the summer months, those requests will not be granted. This does not apply to non or joint-custodial parents, childcare provider situations who will be allowed seasonal passes if a request is presented in writing. Just a reminder all residents must be in good standing to utilize the amenities. All fees and charges must be current before any passes or validations will be issued.
