8 Physicians in Ontario 2015 What s New This is the twenty-fourth annual publication of Physicians in Ontario produced by the Ontario Physician Human Resources Data Centre. The primary geographic entity of physician data reporting is the Local Health Integration Networks (LHINs). Governed by the Ministry of Health and Long-Term Care, the LHINs are responsible for coordination of planning and financial management of health care in Ontario. The 2015 Active Physician Registry reports the Ontario physician compliment by Local Health Integration Network. To ensure the greatest level of accuracy, a Geographical Information System (GIS) was used in conjunction with the 2015 Postal Code Conversion File, which was the latest file available, for mapping the practice location of all physicians in the Active Physician Registry in LHIN Population data was not available at the time of PIO printing. As a result, tables 5.a & 5.b Population per Physician in ONTARIO by LHIN and Specialty of Practice in 2015 have been excluded from this publication. When the population data becomes available at a later date the Population per Physician in ONTARIO by LHIN tables for 2015 will be published online at the OPHRDC website, The 2015 PIO has 2 new RCPSC certified specialties; Neuroradiology Developmental Pediatrics The 2015 PIO has 1 RCPSC certified specialty name change; Pediatric General Surgery is now Pediatric Surgery Each year the OPHRDC verifies functional specialty for a select group of specialties by using the College of Physicians and Surgeons of Ontario s certified specialty list, conducting targeted telephone interviews and incorporating data from the Ontario Physician Hospital Appointments List. For details, refer to About the Dynamic Physician Interview on page ix. OPHRDC welcomes comments about the report and suggestions for improvements. 9 About OPHRDC The Ontario Physician Human Resources Data Centre (OPHRDC) is the definitive source for information on physicians and postgraduate medical trainees in Ontario. Located at McMaster University, OPHRDC is a collaborative project of the College of Physicians and Surgeons of Ontario (CPSO), the Ontario Ministry of Health and Long-Term Care (MOHLTC), the Ontario Medical Association (OMA), and the Council of Ontario Faculties of Medicine (COFM). Funding is provided by the Ontario Ministry of Health and Long-Term Care. The contract to develop and operate the OPHRDC was awarded to the McMaster University Faculty of Health Sciences in Since 1992, OPHRDC has maintained a registry of all licensed physicians practicing in Ontario, the Active Physician Registry. From this registry the Centre produces numerous reports and analyses, including an annual report, Physicians in Ontario (PIO) and numerous special reports based on the annual PIO dataset. OPHRDC also maintains a complementary Ontario Postgraduate Medical Trainee Registry. Each of the Ontario medical schools provides data on postgraduate trainees and transfers data to the OPHRDC central registry for analysis and reporting. An annual report, Postgraduate Medical Trainees in Ontario (PMTIO), and a variety of special reports are prepared from this Registry by the OPHRDC. 10 About Physicians in Ontario Physicians in Ontario (PIO) reports are designed to be the most accurate and comprehensive source of physician information in Ontario. The best sources of data for each element of physician information have been identified and integrated into a common database, the OPHRDC Active Physician Registry. Agreements to acquire data for this registry have been negotiated with the College of Physicians and Surgeons of Ontario (CPSO), The Ministry of Health & Long-Term Care (MOHLTC), the Ontario Medical Association (OMA), the College of Family Physicians of Canada (CFPC), and the Royal College of Physicians and Surgeons of Canada (RCPSC). In all cases, data transfer to the OPHRDC is predicated upon legally stringent agreements covering the confidentiality and use of data. In addition, OPHRDC conducts its own dynamic interviews of Ontario physicians' practice locations and specialties. The telephone interviews serve to validate location, specialty and activity status data in the physician registry and to collect information about specific sub-specialties being practiced. The Registry is renewed annually at an arbitrary census date of December 31. As of that time, physicians who have ceased practice in Ontario are removed, new physicians and physicians who have returned to practice are added and new physician location and specialty information is integrated. In order to validate these additions, attritions and changes, the data are carefully screened and subjected to a number of criteria. From this final dataset, the Physicians in Ontario annual report and other reports are prepared. For more about the Registry preparation process, see About the Active Physician Registry. 11 About the Active Physician Registry Physicians in Ontario (PIO) reports are prepared from the annual Active Physician Registry. The following describes the methodology used to select physicians for inclusion in the Active Physician Registry. The Registry is derived substantially from The College of Physicians and Surgeons of Ontario (CPSO) data, records of the Ontario Health Insurance Plan (OHIP), information collected from physicians by the OPHRDC dynamic interview, and a number of secondary data sources. First, records of licensees of the CPSO are screened for anomalies and duplicates and rules are applied to exclude physicians who are on the CPSO list but are unlikely to be active. These rules include: Removal of deceased physicians. Removal of licensees with birth years 85 years prior to the report year. Removal of physicians with expired licenses. Removal of physicians with educational, short term or academic visitor license class. Removal of physicians not practicing in Ontario. The remaining CPSO records are then matched against OHIP demographic data. Duplicates and suspicious matches are contacted and interviewed, explained using other data, or referenced to the owners of the source data. The next step is a record-by-record reconciliation between the Ontario Postgraduate Medical Trainee Registry (also a product of the OPHRDC) and the Active Physician Registry to eliminate postgraduate medical trainees holding a general license who had not completed training by July 31. Location and activity level data from OPHRDC dynamic physician interview is then overlaid on the Registry dataset. A number of secondary sources of data are then used to validate and in some cases to supplement the data used. The final Registry then includes only licensed physicians verified as actively in practice in Ontario. Data elements regarding geographic location, specialty of practice, age, gender and other variables are then appended so that the PIO reports can be prepared. 12 About Physician Data Elements The following describes how OPHRDC creates the four data elements used to prepare Physicians in Ontario reports. Physician Specialty: Records of physicians passing the criteria for inclusion in the Active Physician Registry are appended with the specialty recorded by the CPSO for the most recent specialty or subspecialty of RCPSC certification or Functional Specialty as identified by the Dynamic Physician Interview. Certification data provided by the CFPC is used to identify Family Physicians and Family Physicians with certification in Emergency Medicine. Remaining physicians shown to be billing OHIP in "general practice" are included in the category Family Medicine/General Practice. Location of Primary Practice: Geographic location is derived from practice addresses on public record. These are obtained from the CPSO, as publicly reported on their website, the OPHRDC telephone interview. They are then verified by or supplemented with information from the OPHRDC interview. Since 2002, expansion of the OPHRDC physician interview and increased reliability of CPSO data have created the opportunity for practice location data to be based primarily on these sources. Physicians can have multiple practice locations including other provinces or countries. A physician is counted as active in Ontario if they have an Ontario practice address and make a regular contribution to health care in Ontario. Physician Age: Age data are calculated in the following way: Report Year DOB year = age OPHRDC compares birth date data from several sources. Physician Sex: Sex data is derived by comparison of a number of sources. Population Data: Prior to 2000, population estimates were based on Statistics Canada census data purchased from a commercial statistics company. In 2000 and 2001, data used were Ministry of Health and Long-Term Care estimates from Statistics Canada data based on the 1996 Canadian Census. The population figures for Physicians by Local Health Integration Network in Ontario 2015 are estimates based on the 2011 Census Data and were provided by the Ministry of Finance for the Ministry of Health and Long Term Care. viii

13 About the Dynamic Physician Interview Beginning with preparation for the first Physicians in Ontario report in 1992, OPHRDC has conducted a series of interviews. By contacting physicians or their office staff, focusing on particular geographic, specialty and age segments of Ontario s physician compliment. Each year OPHRDC verifies functional focus of practice for a select group of specialties. The Royal College of Physicians and Surgeons of Canada (RCPSC) certified specialty list is incorporated with data from the Ontario Physician Hospital Appointments List (PHAL) to target specific areas of interest for telephone interviews. These RCPSC Specialty changes and the OPHRDC Dynamic Physician Interview targets can result in significant changes within Specialties / Subspecialties and should be taken into consideration when using multi-year trending.
